The surgical approach to this clinical scenario is even more challenging when coupled with anatomical anomalies, like horseshoe kidney and the presence of abnormal vessels. Materials and Methods: In this video, we will show a case of a large left renal tumor in a horseshoe kidney with double renal vein and endo-caval thrombus that reaches the suprahepatic veins (T2b—third level). The video describes, step by step, the piggy-back surgical technique, which allows to completely expose the inferior infrahepatic vena cava, splitting it from the liver that remains connected just to the three suprahepatic veins. This technique, borrowed from the liver transplant, allows, without increasing the risk of intraoperative complications and maintaining a high oncologic efficacy, to have an excellent vascular control that, in cases like the presented one, may avoid recourse to the use of extrac...","PeriodicalId":92923,"journal":{"name":"Journal of endourology. Materials and Methods: A retrospective study of prospective data on 118 TURP procedures performed at our urology department in Saudi German Hospital–Jeddah, from October 2011 to October 2013, was conducted. Exclusion criteria were prostate cancer, previous prostate surgery, and preoperative urinary incontinence. Surgery was conducted under spinal or general anesthesia. The sphincter irrigation test was routinely performed before the start and at the end of the resection, and at any time needed during the resection (e.g., if the prostatic tissue exceeds the level of the verumontanum) to sharpen the surgeon's perception of the normal structures in this region, which enables contractile elements to be spared from resection. However, performing the urinary diversion by this approach requires the development of special skills, and the learning curve is yet to be determined. Combining the robotic and the open approaches may allow the surgeon to maintain the outcomes while developing the technique for the intracorporeal urinary diversion. Materials and Methods: In our present case series of seven patients, most of the patients were having T1 or early T2 disease. We present a video of a male patient, aged 59 years, with a history of recurrent urothelial bladder carcinoma with squamous differentiation (T1G3). CT shows large bladder tumor in right lateral wall. Additionally, MRI/TRUS fusion platforms have demonstrated improved detection of clinically significant prostate cancer compared to standard of care systematic 12-core transrectal ultrasound biopsies.2 Materials and Methods: This video demonstrates the MRI/TRUS fusion biopsy procedure of a 62-year-old male with a history of elevated serum prostate specific antigen (PSA) in the setting of multiple negative TRUS biopsies. The patient was initially biopsied under the standard of care 12-core TRUS with no evidence of prostate cancer with subsequent TRUS biopsies also demonstrating no evidence of cancer.
