Placing a suture in the posterior bladder neck is a common approach in patients with median lobes, whereby the median lobe is retracted with a figure-of-eight suture. Materials and Methods: The 30-degree down lens is utilized for the most optimal view of this dissection. Results: Multiple visual cues are utilized to determine the location of the bladder neck, including the limits of the anterior prostatic fat pad, excursions of the Foley balloon, and tissue response to pinching maneuvers. The initial dissection focuses primarily in the midline, aiming to get down to vertical fibers, which appear just before the catheter is exposed. If the lateral contours of the prostate are readily apparent, this tissue may be taken down as well, bearing in mind that bleeding is often encountered laterally as shown. Once the catheter is exposed, some lateral progression will help define th...","PeriodicalId":92923,"journal":{"name":"Journal of endourology. We aimed to compare its results with the classical ascending transperitoneal nephrectomy. Materials and Methods: The new modified “Tunc” technique1 includes a direct upper kidney pole access and early ligation of renal pedicle. The kidney preparation is performed in a descending and more direct access to pedicel. We compared 85 cases performed with the classical technique (group 1) with 98 cases (group 2) performed with the Tunc technique. Patient positioning and trocar sites were the same. Results: No significant differences were detected regarding mean patient age, intraoperative blood loss, and duration of hospital stay between the two groups. The difference became statistically significant in comparing the operation times. The mean operation time was 64.9±19.3 minutes and 28.2±7.7 minutes, respectively, in groups 1 and 2 (p=0.001). The mean operation time including right neph...","PeriodicalId":92923,"journal":{"name":"Journal of endourology. Currently, open surgery is indicated for failed percutaneous, ureteroscopic procedure and for calculus in anomalous kidney when other methods are not feasible or not available. In these scenarios, when expertise is available, laparoscopic approach has been proved to be more beneficial with regard to postoperative morbidity, recovery, and clearance of the stone.1,2 We are presenting a video demonstration of laparoscopic management of urolithiasis in different circumstances. Laparoscopic pyelolithotomy for left renal calculus was performed through transperitoneal approach using four ports in lateral decubitus. Double-J stent was placed in the lithotomy position. After reflecting colon, upper ureter was identified and traced proximally to reach the renal pelvis. Renal pelvis was exposed completely, and inverted V-shaped pyelotomy was performed. Calculus was identified and delivered, followed by pye...","PeriodicalId":92923,"journal":{"name":"Journal of endourology.
