NIRF imaging has previously been described in conjunction with the administration of the fluorophore indocyanine green (ICG). The principle urologic application of NIRF imaging has been to distinguish normal parenchyma from tumor during partial nephrectomy with intravenous ICG. We describe a novel use of NIRF imaging without ICG to identify intraluminal areas of interest during robotic-assisted laparoscopic (RAL) surgery marked by the white light (WL) of endoscopes. Materials and Methods: A single surgeon performed RAL surgery on six patients aided by NIRF imaging to visualize the areas of interest marked by the WL of endoscopic instruments. Recently, the safety and efficacy of ex vivo ureteroscopy for stone removal in donor kidneys have been described.1–3 Here, we present our experience with ex vivo ureteroscopy and laser infundibulotomy for the treatment of a stone contained within a calyceal diverticulum. Calyceal diverticulae are known to harbor bacteria and pose an infection risk for the immunosuppressed recipient,4 leading us to decide that treatment was prudent before transplantation. Materials and Methods: A 38-year-old woman with a previous stone episode presented for the evaluation as a living kidney donor. Preoperative computed tomography angiogram revealed two incidental stones in the left kidney, measuring 6 and 2 mm, with a density of 530 Hounsfield units. Excessive radiation exposure is associated with multiple short- and long-term consequences. This educational video demonstrates the important measures urologists could undertake to protect patients, themselves, and other healthcare professionals from excessive radiation exposure without compromising outcomes. Methods: As Low As Reasonably Achievable principles, including minimizing time, maximizing distance, and always using shields, are demonstrated. Intraoperative tricks to minimize radiation exposure are recommended, including using the foot pedal to activate fluoroscopy by the surgeon himself/herself, using pulsed fluoroscopy, and documenting fluoroscopy time. We previously described a robotic partial nephrectomy (RPN) technique with intracorporeal cooling and extraction (I.C.E.) for cold ischemia and intraoperative tumor assessment.1 We present a video of our I.C.E. technique.2 Recent evidence suggests a potential benefit of cold ischemia for preserving renal function.3,4 The total run time for our video is 8 minutes 2 seconds. During robot-assisted laparoscopic prostatectomy, patients are vulnerable to neuropraxia especially affecting the shoulders and upper and lower limbs while in the Trendelenburg position. Cases of compartment syndrome have also been described. We share our unit's experience on how to reduce the occurrence of intraoperative neurovascular complications. Methods: We have compiled a short but detailed video demonstrating how we position our patient preoperatively for a robot-assisted laparoscopic prostatectomy. Results: In the 4 years we have been running our robotic program, we have noted five cases of self-limiting postoperative arm and leg neuropraxia and two cases requiring fasciotomy for suspected compartment syndrome.
