Saurenmann","doi":"10.11648/J.IJI.20210902.11","DOIUrl":"https://doi.org/10.11648/J.IJI.20210902.11","url":null,"abstract":"Familial Mediterranean fever (FMF) is an autosomal-recessive autoinflammatory disease in the group of Hereditary periodic fever syndromes (HPFS), characterized by recurrent, self-limited attacks of fever and polyserositis, which last 2-4 days. It manifests mainly in childhood and often has early onset, resolve without sequels. Genetic testing of FMF is efficient and allows diagnosing of atypical cases. FMF is a significant health care problem in Armenia because of high frequency of carriers of MEFV mutations 1:3 - 4 (0.21) and marked FMF prevalence 54.7 per 10`000 of the total population. Articular symptoms are found in about 45% of cases. They usually manifest as acute recurrent arthritis (ARA) or arthralgia, but more rarely also chronic arthritis can be found. We aimed to investigate clinical and genetic characteristics of the joint manifestations in Armenian children with FMF. The charts of all 715 patients with proven diagnosis of FMF at the National Pediatric Center (NPC) for FMF were reviewed for joint manifestations. The diagnosis of FMF and disease severity were determined according to the Tel-Hashomer criteria and molecular-genetic detection of 12 MEFV mutations common for Armenians. The characteristics of joint manifestations and overall FMF disease were then compared to MEFV mutation analysis. There were 438 boys and 277 girls with an age at diagnosis between 3 months and 17 years (mean age: 8.64±0.17). Joint involvement was observed in 56.4% of all 715 cases. The manifestation was ARA in 30.5%, arthralgia in 21.2% and chronic arthritis (CA) in 4.7%, which would also qualify for a diagnosis of JIA. The frequency of ARA was associated with M694V mutation, mainly M694V homozygous and M694V heterozygous genotypes. The risk of CA depended on MEFV genotype of the FMF patients and was the highest in M694Vheterozygotes and M694Vhomozygotes. M694V heterozygous genotype was noticed significantly more frequently among FMF patients with spondyloarthritis in comparison to those without it. The probability of the development of CA among FMF patients without M694V mutation was significantly higher also in heterozygotes in comparison to compound-heterozygotes. We concluded, that the frequency of joint involvement among Armenian pediatric FMF patients was high - 56.4%, especially for CA. Chronic arthritis may be the first manifestation of FMF and occurred usually in patients with severe M694V mutations. The aim of the present study investigated the relationship between autoimmunity and lead pollutants in a cohort of children who lived in a mining settlement. Methods: Children were studied clinically and serologically. The serum lead concentration was measured by atomic absorption spectrometry, and antinuclear antibodies were analysed by immunofluorescence, ELISA and Western blot. Results: None of the studied children displayed any autoimmune disease. The lead concentration in the control group was negligible, at 0.10 ± 011 μg. L-1. In contrast, the study group had values of 12.72 ± 0.5 μg. L-1. The differences between the groups were significant (p < 0.0001). In addition, control sera were negative for antinuclear antibodies (ANA), while the sera of children living near the mining settlement showed ANA-positive titres (p < 0.001). These autoantibodies mainly recognized antigens associated with dividing cells, such as the mitotic apparatus and centrioles. Conclusion: The present results show how metallic pollutants induce environmentally associated autoimmunity and are reflected by antinuclear antibodies. Objectives: To assess the factors associated with pattern of skin disorders among HIV infected children in Aminu Kano Teaching Hospital Kano, Nigeria. Materials and methods: A cross-sectional study was conducted to determine the factors associated with pattern of skin dermatoses among HIV infected children attending Paediatric Infectious Disease Clinic of Aminu Kano Teaching Hospital, Kano, Nigeria. A total of 223 HIV infected children aged 6 months to 14 years were included in the study. Chi-square test or Fishers exact test where necessary were used to determine the difference between proportions of categorical variables and level of significance was set at 0.05. Binary logistic regression was used to further analyze significant relationships from the univariate analysis and the confidence interval was set at 95% and p < 0.05 was considered statistically significant. Results: A significant association was observed between low socio-economic class with Infections and infestations more specifically the bacterial skin infections. And an association between severe immunosuppression and fungal skin dermatoses was observed. Our study considered isolation of dental infection bacteria and quantitative evaluation of serum. IgA, IL-4, IL-7 and CD4 and CD8 molecules among dental plaque patients and control group. Oral bacteria from dental infection patients were isolated in appropriate media and diagnosed by biochemical tests and in vitro quantitative determination of serum IgA, IL-4, IL-7 and CD4 and CD8 molecules using ELISA technique. Single and mixed bacterial isolates were noted, mixed infection were (59.25%), the nature of bacteria was Gram positive cocci Streptococcus viridans, Streptococcus pyogenes and Staphylococcus aureus, and Lactocacilli spps, in addition to Gram negative rods black- pigmented bacteria, Klebsilla pneumonia, and Esherishia coli. Serum IgA was higher in patients (368.8±182.5) ngml than in control group (319.92±79.26) ngml. Serum IL-4 was higher patients (285.33±86.12) pg/ml than in control group (257.7±94.14) pgml. Serum IL-7 was higher in control group (19.59±4.14) pg/ml than in dental plaque patients (17.98±3.18) pg /ml. Serum CD4 molecules was higher in control group (1.371±0.5242) ng/ml than in dental plaque patients (1.326±0.1292) ng/ml. Serum CD8 molecules shows non-significant elevation in patients group 0.5825±0.02717 (ngml) than in control group 0.51±0.01643 (ngml) P≤0.05. Mean of the CD4/CD8 ratio was higher 2.783±1.126 in control group while it was 2.355±0.24 in dental plaque patients, however the differences were non-significant (P≤0.05). The present study conclude that the bacteria isolated from dental infection patients were mixed more than single infection, there were non-significant elevation in IgA, IL-4, and CD8 in patients while IL-7 and CD4 was lower in patients group than in control group, while CD4CD8 ratio were lower in patients group, these result reflect the fact that mucosal antigen induce systemic tolerance to some extent since these bacteria present in oral cavity in early childhood. This is a prospective study with a descriptive and analytical aim which was conducted at the University Hospital Center (CHU) of Cocody (Abidjan). It took place from September 2017 to May 2018 on 117 atopic sickle cell disease patients, with no known history of allergy to aeroallergens, to any foods or to occupational allergens. These patients were interviewed and clinically examined for an atopic site. They were subsequently subjected to skin tests. Statistical analyzes were performed using Epi info 6.0 software using Fisher's test and odds ratio. The significance level was located at an odds ratio greater than 1. The comparison of the percentages was carried out by Fisher's test with a significance level p = 0.05. The majority of patients with sensitization to Aspergillus mix were over 15 years of age, with a predominance of women (Sex ratio: 0.56). Neither sex nor age appeared to influence the state of sensitization in our study (p> 0.05). The overall prevalence of sensitization to Aspergillus mix was 7.7%. Our results correlated asthma severity with sensitization to Aspergillus mix (p<0.05). However, there was no significant correlation between severity of allergic rhinitis and sensitization to Apergillus mix. This study confirms the low prevalence of sensitization to Aspergillus mix. So, they are subjected to a higher risk of infection compared to other medical specialties. This study aimed to assess the level of anxiety, depression, and post-traumatic stress disorder during COVID-19 pandemic among anesthesiologists. Methods: This is a cross-sectional, hospital-based survey conducted after getting IRB approval. The primary outcomes are to evaluate the prevalence of anxiety, depression, and post-traumatic stress disorder (PTSD) among anesthesiologists working in Saudi Arabia. The secondary outcomes include the association of the socio-demographic and occupational characteristics with mental health in the study sample. The survey sent to the target participants using a web-based software platform (https://www.surveymonkey.com/r/ZJ98BTJ) though the Saudi Society of Anesthesia. The study sample included anesthesiologists and their allied health care providers (nurses and technicians) involved in the management of COVID-19 patients. Data were analyzed using SPSS version 21. Descriptive analyses were conducted, and the ranked data for each level for symptoms of depression, anxiety, and PTSD were presented as numbers and percentages. Multi-variable logistic regression analysis was used to explore potential influence factors on the outcomes. P-values of less than 0.05 were considered statistically significant. Results: The survey sent to (1024) anesthesiologists and anesthetists, through the Saudi Anesthesia Association (SAS). A total number of 347 participants involved in the survey with a response rate (33.887%), while 296 of them completed the survey and included in the statistical analysis. Males represented (71.3%) of the sample, Saudis (58.8%), two-thirds aged 30-50 years, (83.4%) were married, physician anesthesiologists represent (81.3%). The prevalence of PTSD, anxiety, and depression among the included sample were (25.3%, 29.4%, and 30.7%) respectively. The following factors were associated with a higher prevalence of psychological stress: age categories below 40 years, being unmarried, those having relatives diagnosed with COVID-19, unsatisfied by the safety measures, smoking habits, and among female gender compared to males. Also, the prevalence of stress was higher among resident anesthetists compared to the nurses, technicians, and consultants. The association of psychological outcomes and other factors were inconsistent.
