DAH is a rare but catastrophic manifestation of SLE with high mortality, requiring early, intensive therapy. Case Presentation: A 31-year-old female presented with low-grade fever and joint pains for six weeks associated with alopecia, anorexia, and weight loss. She had pale and had cervical lymphadenopathy, moderate hepatomegaly, and splenomegaly. Hb level was 9.2mg/dL, with a platelet count of 144 × 103/µL. The ESR was 65mm/hour, and the CRP level was 36 mg/L with a UPCR of 332mg/g. She had low C3, C4 levels, positive ANA, and dsDNA titer. A renal biopsy revealed class 3 lupus nephritis. Twenty days after, she was readmitted with acute onset of pleuritic chest pain, cough, and dyspnea, where she was hemodynamically unstable with SpO2 of 85% on air. The Hb level was 8.6 g/dL with a platelet count of 106 × 103/µL. Her condition deteriorated despite standard medical care, where the Hb level and SpO2 dropped to 5.6 d/dL and 65%, respectively, even after blood transfusions, intubation, and artificial ventilation. The chest X-ray revealed bilateral large whitish hazy shadows, and the HRCT scan revealed diffuse bilateral pulmonary hemorrhages. Then she was transferred to the ICU, and there she was started with broad-spectrum antibiotics, methylprednisolone, and plasmapheresis. However, after one week of ICU stay, her renal functions worsened, and she was initiated on CRRT. However, despite all the resuscitation efforts, she succumbed following a cardiac arrest. Conclusion: DAH is a rare catastrophic complication of SLE, which usually presents in patients with an established diagnosis of SLE, even on medical therapy. The diagnosis of DAH is problematic because it mimics a severe pulmonary infection. HL is a common hematological malignancy, mostly seen in young and middle-aged people. According to NCCN guidelines, common treatment options include ABVD, BEACOPP, StandfordV, etc. Most HL have a good prognosis and even can be cured. However, some Refractory / Recurrent (R/R) HL, continue to progress during treatment or even relapse after the completion of a complete chemotherapy course 3 months. At this time, the best treatment for R/R HL is high-dose chemotherapy / Autologous Hematopoietic Stem Cell Transplantation (HDC/HSCT). This paper reports a case of Recurrence/Refractory (R/R) mediastinal Hodgkin's lymphoma that relapsed after Allogeneic Hematopoietic Stem Cell Transplantation. After receiving a variety of treatments, including chemotherapy, radiotherapy, autologous hematopoietic stem cell transplantation and so on, no long-term sustained remission was achieved. Traditional chemotherapy regimen has not brought ideal therapeutic effect to patients, but the rapid development of immunotherapy in recent years has brought new hope to patients. Finally sustained remission was achieved under the treatment of Brentuximab Vedotin combined with PD-1 inhibitor. This paper combined with the literature reports and discussed the clinical characteristics, treatment process and related experience and thinking methods of immunotherapy related to PD-1 inhibitors in this refractory and easy-to-relapse case. The poor prognosis of MM is always closely related to the immunosuppression of T cells. Restore the immune function of suppressed T cells may reverse the immunodeficiency in the MM microenvironment and improve prognosis. In recent years, immunosuppressive receptors such as programmed cell death receptor-1 (PD-1), T cells immunoglobulin and ITIM domain (TIGIT), Lymphocyte-Activation Gene-3 (LAG-3), T cell immunoglobulin and mucin domain–containing molecule 3 (Tim-3), Leukocyte immunoglobulin-like receptor B1 (LILRB1) and Cytotoxic T- lymphocyte antigen 4 (CTLA-4) have been discovered playing a key role in the tumor immunodeficiency microenvironment. For patients with solid tumors and some leukemia, immunotherapy targeting such receptors can significantly improve the T cells immunodeficiency. However, similar positive results were not found in MM patients, which is related to the complex immunosuppressive mechanism. At present, the understanding of immunosuppressive receptors in MM is insufficient. Aims: To investigate the function of ERK1 in regulating DNA hypomethylation and explore the potential molecular mechanisms involved in lupus mice. Methods: CD4+T cells were isolated from MRL/lpr and BALB/c mice, activated in vitro in the presence or absence of 5-azacytidine (5-Aza) (a DNA methyltransferase inhibitor)/U0126 (a selective inhibitor of the ERK signaling pathway) respectively. The positive rate of CD4+T cell was measured by flow cytometry, and expression levels of CD70, ERK1, DNA transferase 1 (DNMT1) were estimated by RT-PCR and Western blot. The methylated DNA is detected using the Capture and Detection antibodies, then quantified colorimetrically. Results: The expression level of p-ERK1 in MRL/lpr mice was significantly lower than healthy control (P<0.05); Meanwhile, down regulation of DNMT1 and DNA hypomethylation were found in MRL/lpr mice T cells. In contrary, the expression levels of CD70 increased when compared with healthy control (P<0.05). The same changes were seen in healthy CD4+T cells treated with U0126. Conclusions: DNA hypomethylation regulating mechanisms in lupus T cells were associated with ERK1 signal pathway. The measurement of APLs (IgG and IgM anti-cardiolipin (aCL) and anti-β2-glycoprotein-1 (aB2GP1) was performed on the 1st day and after 15 days of admission, using the chemiluminescence assay (threshold =19 CU). The average age of patients was 64.7 ± 20, 44 years (ranges: 30-88 years), with a sex-ratio of 1.25. On day-1, APLs were positive in two cases, the first of which was positive for IgG aβ2GP1 (94.9 CU) and IgG aCL (24.8 CU), and the second was positive only for IgG aβ2GP1 (31.4 CU). On day-15, APLs showed negative results for both aβ2GP1 and aCL for the first case, and decreasing titers of aβ2GP1 for the second one. Interestingly, these two cases showed no thromboembolic events and had a good clinical outcome. Conversely, APL positivity occurred at day-15 in two cases, corresponding to IgG aB2GPI (49.3 CU) in one case, and IgG aCL (76 CU) in the other. Both cases presented with a prolonged activated-partial-thromboplastin-time, high levels of D-dimers and fibrinogen, associated with increased levels of ferritin and interleukin-6. Our series has shown that IgG aB2GPI or IgG aCL can be either transient or appear secondarily with significantly high titers. According to the classification of the World Health Organization (WHO) 2016. Our study is described on a series of cases that were studied using standard histopathological techniques, before undergoing specific immunohistochemical techniques for the detection of anti-IDH1antibodies in gliomas and biomolecular ones in the search for 1p19q deletion. Central nerve tumors account for 0.18% of all conditions, over a period of time. Meningothelial-type meningioma was the most common (60.9%). The most affected age group is over 50. The distribution of tumors by sex shows a predominance of woman (67.1%). The gliomas had constituted 29.6% of all nerve tumors recorded over the period considered and whose predominance was constituted by oligoastrocytomas with 42.1%. The immunohistochemical profile is characterized by a strong positivity with the anti-IDH1antibody in case of grade II oligoastrocytoma with 6.6%. Our study showed that central nerve tumors account for 0.18 of all conditions, gliomas are made up of 29.6% of all nerve tumors listed with a predominance of oligoastrocytomas.
