Ponmani","doi":"10.19080/crdoj.2021.14.555887","DOIUrl":"https://doi.org/10.19080/crdoj.2021.14.555887","url":null,"abstract":"Background: An increase in the incidence and the severity of diabetic ketoacidosis (DKA) in children with new onset type1 diabetes has been reported in the COVID-19 pandemic. Studies have linked this to SARS-CoV-2 although causality could not be established. Methods: We report two children with new onset type 1 diabetes and serum autoantibodies against islet cells and glutamic acid decarboxylase who developed severe DKA following infection with SARS-CoV-2. Both tested positive for COVID -19 PCR from nasopharyngeal swabs and developed symptoms of severe DKA, shock and cerebral oedema. Both children deteriorated rapidly illustrating the combined synergistic effects of the inflammatory pathways of COVID -19 and DKA. Results: Both children needed admission to intensive care. The first child had raised inflammatory and cardiac markers and was suspected to have Paediatric multisystem inflammatory syndrome temporarily associated with COVID-19 (PIMS-TS). The period of type 1 diabetes symptoms was short in the second child, she also presented with clinical and biochemical features of pancreatitis which improved with supportive management. She also presented with biochemical thyrotoxicosis at the time of diagnosis. Conclusion: Our report shows the association between infection with SARS-CoV-2 and rapid decompensation to DKA possibly accelerating the development of new-onset type 1 diabetes in genetically predisposed children. Additionally, the second child had pancreatitis illustrating that SARS-CoV-2 can affect both the endocrine and the exocrine function of the pancreas. Herein, we report the case of a sixty-year old female, with a history of non-complicated type 1 diabetes and well-substituted Hashimoto thyroiditis who had a myocardial infarction at the age of 48 year-old. Our patient did not have classical risk factors for coronary heart disease, clinical and laboratory investigations showed the diagnosis of rheumatoid arthritis. In a cross-sectional study 310 older adults (55.7% of women) were completed Pittsburgh Sleep Quality Index (PSQI) questionnaire. It was designed to assess sleep duration and sleep quality. Body-mass index (BMI) was measured and dichotomized as normal and overweight or obesity status. The mean age (SD) of participants was 65.9(3.1) years. More than 30 % of participants were overweight or obese. The findings of the current study demonstrated statistically significant relationships between sleep duration and sleep quality with BMI. Different medications are used to regulate dyslipidaemia. Efficacy and safety are questionable on treatment with lipid lowering drug for long period. Need of time is to search for new lead compound from phytoconstituents of plants which is equal in potency and efficacy with present oral hypolipidemic drug but with fewer side effects. Terminalia arjuna has hypolipidemic properties therefore this research is designed to evaluate the hypolipidemic effect of Terminalia arjuna on high-fat diet induced dyslipidaemia in Wistar rats. Methods: An experimental study was done. Total 36 Wistar rats divided into 6 group randomly. Group I given normal pellet diet. In remaining group dyslipidaemia induced by high fat diet given for four weeks. Group II (dyslipidaemia control) was given high fat diet. Group III, IV were given Terminalia arjuna 250 and 500mg/kg per oral and group V, VI were given Rosuvastatin 10mg/kg and 20mg/kg per oral for 8weeks. Blood samples withdrawn by retro-orbital puncture and analysed for total cholesterol, triglycerides, HDL by commercial kits. LDL, VLDL calculated by friedewald formula. ANOVA followed by post hoc analysis used for statistical analysis. Results: Terminalia arjuna showed reduction of total cholesterol, triglycerides, VLDL, LDL level at both doses and intergroup comparison were found to be statistically significant (p<0.05) at end of treatment. The change with Terminalia arjuna(500mg/kg) in total cholesterol, triglycerides, VLDL, LDL was comparable with Rosuvastatin(10mg/kg). This paper described the health effects of the Community-based interventions in the women’s park of Saeidabad village, East Azarbaijan on province, Iran. In this cross-sectional study, 109 women participated. The statistical population of the study was selected from women over 30 years in Saeidabad who were sure to experience the community-centered design of women’s park for physical activity. The participants mean age was 45.6 years. Most of them (98%) had one or more risk factor for cardiovascular disease. Thirty people were diabetic and used insulin. They lowered insulin level. Thirty-five other had hyperlipidemia whose blood lipid levels were significantly decreased. Ten people had depression. These ten stopped taking the anti-depressive pills. The Body Mass Index (BMI) of 85 of the studied people was reduced. The development of a women-specific sport area for women has been shown to have positive physical, psychological and social effects on them, where they make use of amenities in a safe and low-cost environment, and their ease of use and satisfaction from national institutions. Excess fat in visceral adipocytes impairs endocrine as well as immune response. The inflammatory and hormonal factors released from adipose tissue play a central role in inter-organ cross-talk. This review considers the current evidence on the role of Adiposopathy on cardiovascular disease and the importance of physical activity in restoring the function of adipocytes. Methods: Literature searches were conducted with keywords (e.g., Adiposopathy, cardiovascular disease, Exercise) using PubMed, MEDLINE, EMBASE, Science Direct, Google Scholar and DOAJ. All article types were included in the search. Results: The pathological mechanisms associated with up-regulated proinflammatory and down-regulated anti-inflammatory cytokines are responsible for the disturbed communication among muscle, liver and vasculature. In other words, Adiposopathy promote cardiovascular risk factors such as atherosclerosis, hypertension, insulin resistance, dyslipidaemia and prothrombotic state which in turn, directly and indirectly, promotes cardiovascular disease. Methods: A telephone survey was conducted among a sample of 552 registered patients with type 2 diabetes, aged 45-70 using The-Brief-Chronic-Illness-Resources-Survey to assess sources of support to health behaviors. Logistic regression analysis adjusted for age, sex, body-mass-index, and smoking status was used. Results: No association was found between support from the medical team and health behaviors. The personal resource subscale was associated with visiting a dietitian [OR=1.3, 95%CI (1.1-1.5)], engaging in physical activity [(OR=1.3, 95%CI (1.1-1.7)], consuming a healthy diet (OR=1.6 95%CI (1.3-2.0), and having a better health perception [OR=1.31, 95%CI (1.06-1.62)]. The family and friends’ resource were associated with dietitian consultation [OR=1.3, 95%CI (1.1-1.6)] and with physical activity [OR = 2.0, 95%CI (1.6-2.6)]. Conclusion: Personal support and support of the family and friends but not medical team support subscales were the most associated with healthy diet and physical activity behaviors in people with type 2 diabetes. In investigating the adults, it was decited to cover 50.1% males and 49.9% females to maintain the national level of sex ratio in the sample. The respondents were interviewed when they were visiting some diagnostic centres in urban and semi-urban areas. The percentage of diabetic respondents included in the sample was 67% and 25.5% of them were suffering for 10 years and above, 6.4% were disable against 4.8% total disable respondents in the sample, and obese adults were 30.2%. Diabetic disable adults in the sample were 4.3%. The most responsible variable for diabetes disability was obesity followed by longer duration of diabetes, sedentary activity, smoking habit, being housewife, being married, etc. These variables were identified on the basis of risk ratio and correlation coefficients of variables and discriminant function scores.
