It is estimated that 35–40% of patients diagnosed with RA have got nodules localised in various body regions. Rheumatoid nodules constitute a fairly common problem in the hand surgeon’s practice. This study presents the results of operative treatment of 30 patients with rheumatoid nodules in the upper limb – 21 women and 9 men with a mean age of 51 years. Only 6 of the 30 patients (20%) had RA diagnosed prior to surgery. All but 1 nodules were localised on the hands. The follow-up assessment was performed in a form of a phone interview on average 1.5 years following the surgery. All patients declared satisfaction with the results of the treatment, 5 (17%) had a slight pain in the postoperative scar, and 6 (20%) complained about the esthetic effect. Two cases (7%) of recurrence at the operation site were noted. {"title":"Smoking and hyperglycaemia in blue-collar workers in Lubelskie region in Poland","authors":"K. Jankowski, E. Diabetes and pre-diabetes states are responsible for many cases of ischaemic heart disease. Among blue-collar workers, circadian misalignment may elevate total cardiovascular risk. Materials and methods: A retrospective analysis of data obtained by the National Health Fund from the Program for the Prevention of Cardiovascular Diseases in the Lublin Province in 2008–2018 was carried out; 26,226 visits were analyzed. The assessment included sociodemographic variables, data on nicotinism, and fasting glucose. The χ2 test was used to test the relationship between the 2 qualitative features, and the Pearson linear correlation for the quantitative features. The analyses were performed using IBM SPSS Statistics for Windows, version 25, with a significance level of α = 0.05 for 2-tailed tests. Results: Hyperglycaemia was observed in 14.07% of the research group, 1.37% of the whole group was diagnosed with glucose level ≥126 mg/dL; 54.34% of patients reported smoking history (n = 14,250). Current smokers constituted 33.04% of the study group (n = 8,665) and 21.30% of patients were past smokers (n = 5585). The mean age of smoking initiation was 24.68 years and the mean cigarettes use was 15.66. The correlation between the incidence of analyzed risk factors and age and sex was statistically significant (p < 0.001). Conclusions: Men and respondents of more advanced age have a higher burden of cardiovascular risk factors. The prevalence of smoking and hyperglycaemia in surveyed workers is relatively high. The stay-at-home orders also contributed to a change in the volume of the emergency admissions as well as their causes. Materials and methods: The available electronic databases were searched for papers concerning pediatric trauma admissions, referrals, and visits during the coronavirus outbreak and in the previous year. The quality of included papers was assessed via National Institutes of Health National Heart, Lung, and Blood Institute (NIH NHLBI) Study Quality Assessment Toolbox. Results: The overall workload in pediatric emergency medical care during the COVID-19 outbreak declined by 40.13% (95% CI 18.7–61.6) compared to the previous year. A massive decline in the pediatric trauma admissions was observed during the coronavirus outbreak. The sport-related accidents were supplanted mainly by those that occurred at home. Gestational diabetes mellitus is the cause of abnormalities in pregnancy and fetal development. It can be diagnosed by analyzing blood glucose levels and performing mandatory screening tests. This paper describes the general concept of GDM, its risk factors and complications that may occur in the newborn. The aim of this study was to assess the effect of GDM on the birth condition of full-term newborns. Materials and methods: The study included 104 women divided into 2 groups: study and control. A retrospective method was used to analyze the medical records of the women and their children. The research analysis was performed by calculating the number and percentage of occurrences of each response. The comparison of the qualitative variables between the groups was performed using the χ2 test or Fisher’s exact test where low expected numbers appeared in the tables. A significance level of 0.05 was adopted. Results: The majority of newborns in both groups got an Apgar score of 8–10 at 1 and 5 min. The results of a pulse oximetry test on the 1st day of life showed that all children had normal saturation of 95–100%, irrespective of whether their mother had diabetes or not. Considering the birth weight, it was shown that neonates from the study group significantly more often were born heavier than their counterparts from the control group (p < 0.001). Analysis of the newborns’ health on successive days of life did not reveal any statistically significant differences in the studied parameters between the groups (p > 0.05). Analysis of congenital anomalies revealed a heart defect in 1 neonate and a kidney defect in 2 neonates from the study group. Conclusions: 1. Newborns of mothers with GDM are usually born on time by Cesarean section, in good general condition, and with normal blood glucose levels and saturation. 2. Gestational diabetes mellitus affects neonatal birth weight. Babies of mothers with GDM were born heavier than those of healthy mothers. 3. The effect of sex hormones on the risk and development of cancer among transgender people is not fully understood, although it is well known that steroid hormones affect changes in breast tissue, e.g., the deposition of breast fat and the formation of lobules and ducts in transgender women and the growth of the fibrous tissue. Matherials and methods: This paper reviews the recent literature (available on PubMed, NIH, Google Scholar, and Science Direct) from 2012 to May 2022 on the incidence of breast cancer in the transgender population. Seventeen case reports of breast cancer among transgender men were described (median age: 42; mean number of years of drug use: 7; most commonly diagnosed cancer type: invasive ductal carcinoma; most expressed receptors: estrogen and progesterone) and 15 cases in transgender women (median age: 53; average number of years of drug use: 16; most commonly diagnosed cancer type: invasive ductal carcinoma; most expressed receptors: estrogen and progesterone). Results: The discussed cases indicate a longer hormone therapy in transgender women without a cancer diagnosis and a later age of diagnosis compared to transgender men. The aim of the study was to examine the challenges orthodontists faced during the pandemic in Poland. Material and methods: An anonymous online survey using Google Forms was conducted among 104 orthodontists in Poland. The dependence of the variables was examined using the Pearson χ2 test or the Fisher’s exact test, additionally, the strength of the relationship, Cramer’s V measure of association, was estimated. Results: According to 84.6% of the respondents, the orthodontic practice was closed because of the COVID-19 pandemic. The interruption in patient admission lasted for 2 months in 52.3% of the orthodontic offices. In 35.6% of the orthodontic practices, after the pandemic outbreak, the practice was limited to accepting only patients with emergencies. The results of the Fisher’s exact test showed a significant relationship between the age group of orthodontists and the factor of using teleconsultation. Moreover, additional safety measures were implemented in all orthodontic offices during the pandemic. Interruption of orthodontic treatment due to COVID-19 had a negative impact on treatment outcomes. Conclusions: During the COVID-19 pandemic, it is very important for orthodontists to increase communication with patients. It includes the PHACTR1 and EDN1 genes that are considered genetic loci for cardiovascular disease risk. The present study aimed to verify the hypothesis that polymorphisms of the 6p24 region (rs5370, rs9349379) shape the risk of CAD in the Polish population. Materials and methods: The study included 490 Caucasian subjects divided into 2 groups. The 1st group consisted of 244 patients with angiographically confirmed premature CAD as CAD group and the 2nd group included 242 blood donors as controls. We performed serum lipid measurements and genetic analysis as well as statistical analysis containing biological interactions between genetic and environmental factors. Results: The analysis showed an association between carrying the G allele of the rs9349379 polymorphism and a higher risk of myocardial infarction (MI) in <50-years-old CAD patients (OR: 2.16; 95% CI: 1.03–4.53; p = 0.040). Furthermore, carrying the T allele of rs5370 decreased the risk of hypertension in non--smokers (OR: 0.21; 95% CI: 0.05–0.82; p = 0.046). Moreover, our analysis showed that cigarette smokers carrying the T allele of the rs5370 polymorphism had more than a 9-times greater risk of MI (SI = 9.86, 95% CI: 2.17–35.12, p = 0.003) compared to allele GG homozygotes. {"title":"Crush, degloving, and burns of the upper extremity: a case report","authors":"A. They are usually caused by various devices such as circular saws, grinders, millers, presses, conveyers, and agricultural machines. This article reports a case of mutilating injury to the entire upper extremity as a result of crush and burn of IV degree. The right upper extremity of a farmer who attempted to unblock the mechanism of a straw-press was pulled inside the machine. Despite resistance against the pulling force, the extremity was drawn inside the machine up to the elbow, where the cog-wheels started to glide on the exposed bones of the forearm and arm. The farmer first attempted to cut off the extremity with the pocket knife, following by firing the straw inside the press to draw somebody’s attention to his dramatic situation. Eventually, a person who passed by on the road nearby noticed the smoke flying out of the machine, came to the victim, and called for help. Despite the excessive injury of the entire upper limb and bleeding, the condition of the patient on admission was good. The extremity from the level of mid-arm was crushed, whittled, and burned, and the only option was the amputation at the proximal arm level. {"title":"Successful nonsurgical management of large periapical lesion – a case report","authors":"K. A patient was referred to a specialist endodontic clinic for apicoectomy of tooth 22. Despite the presence of extensive changes in the bone, it was chosen not to perform resection and instead to perform conservative root canal treatment to decompress the cyst through the root canal. The treatment was successful, as confirmed over an 8-year observation period. Cone beam computed tomography (CBCT) was used to diagnose the lesions and monitor the healing process. A CBCT scan performed after the treatment indicated that sealer had been introduced beyond the anatomical opening; however, this did not disturb the healing of the periapical tissue. The foreign bodies that are accidentally swallowed by adults frequently include materials associated with dental procedures, most of which do not result in any complications. This paper presents the case of a 60-year-old female patient who presented to her GP because of increased abdominal pain. Conservative treatment was unsuccessful, so the patient was referred to the hospital with a suspected bowel obstruction. Diagnosis in the hospital revealed a perforation of the jejunum. The patient underwent emergency surgery and fragments of a cohesive substance were extracted from the bowel, which the patient associated with a dental impression procedure that was carried out 7 days earlier. However, chemical analysis of the extracted plastic fragments showed that they were parts of the silicone impression material used for so-called functional impressions used in the later stage of prosthesis preparation, not the alginate material used in the 1st stage.
