The influence of nickel on intestinal microbiota disturbances 镍对肠道微生物群紊乱的影响

Its presence can cause dysbiosis, i.e., an increase in the number of harmful microorganisms at the expense of probiotic bacteria. The disturbed microbiota affects the organism’s metabolism and increases the risk of developing certain diseases. This study aims to analyze the available scientific literature on nickel and its effect on the disturbances in the intestinal microbiota. Materials and methods: The PubMed database and the Cochrane Library were used to find scientific articles with the following combinations of keywords: ‘nickel’, ‘nickel and microbiota’, ‘nickel and allergy’, ‘nickel and health’, ‘microbiota’, and ‘microbiota disturbances’. Scientific publications from the last 20 years were analyzed. Results: Nickel is an essential element for certain biochemical reactions that allow microorganisms (both beneficial and harmful) to grow and develop. Some strains of bacteria seem to have the ability to reduce the host’s exposure to this heavy metal. Excess nickel intake contributes to the disturbance of the proper composition of intestinal microflora, especially in patients diagnosed with systemic nickel allergy syndrome. This may have adverse health effects, possibly contributing to obesity. Conclusions: The wide use of nickel in consumer products, as well as its widespread presence in water and food, increases the probability of human contact with this metal. Degenerative changes alone affect 80% of the population aged 75 years and older. Currently, 2 public health trends are believed to be the dominant factors in the development of gonarthrosis: increasing body mass index (BMI) and increasing age of the population. Both of these factors are also significant risk factors in the development of life-threatening complications with COVID-19 infection. This retrospective study collected the number of patients admitted to a trauma and orthopedic clinic at one time between 2019–2021 along with isolating the number of patients with a diagnosis of gonarthrosis (M17). In both cases, the results show a significant decrease in the number of patients admitted in the 2nd quarter (Q2) of 2020 and a decrease in overall patient admissions compared to the COVID-19 outbreak in Poland (31.25% decrease from 2019 to 2020). A significant impact on the decrease in the number of patients in Q2 2020 may have been the closure of the outpatient clinics from March 16, 2020 to August 16, 2020. Other factors influencing the subsequent decrease in patients may have been people’s fear of contracting COVID-19, decreased physical activity, worsening dietary habits, and weight gain during the “lockdown”. These stimuli lie at the bottom of objective and persistent overstimulation leading to sensory or information overload. Materials and methods: This article is based on a review of the literature and statistical data related to the problem of chronic diseases in a socioeconomic context. Results: Overstimulation constitutes the core area of research of this article. There is no doubt that information overstimulation leads to information stress manifesting on a physiological, emotional, psychical and behavioural level. Progression to chronic conditions leads globally to increased incidence of mental disorders and severe somatic diseases. Conclusions: Since the range and rate of the transformation of civilisation accelerates, it should be envisaged that mental and somatic disorders underlain by information stress will take up a continuously increasing share of healthcare expenditures. This should call for the need for further medical specialisation in the field of treating the symptoms of diseases of affluence and primarily the need for preventive healthcare focused on balancing the excess stimuli. Success lies in their effective filtering and management. Whereas multiple myeloma (MM) occures primarly in bone marrow, solitary plasmocytomas present exclusively extramedullary or as a solitary bone plasmocytoma. Although typical first manifestation of MM are osseus lesions, there are only few case reports in medical world literature of first MM presentation as a single cranial tumor of the skull vault. Clinical presentation: A 63-year-old male with a 4-month history of painless scalp swelling was sent to our department after an unsuccessful attempt to remove the tumor. The patient was neurologically intact and his general condition was good. Bifrontal craniectomy was performed in order to obtain a total resection of the lesion. The tumor was histologically diagnosed as plasmocytoma. Postoperative bone marrow biopsy demonstrated myeloma cells. The subsequent combined therapy had been successful and the patient is in a good condition almost 3 years after the procedure. Conclusion: As an uncommon type of solid tumors, plasmocytomas may mimic other neoplasms, which can lead to delay of diagnosis and proper treatment. The literature in this field is relatively rich, however there are several issues which are not referenced in the literature or are subject to divergent opinions. These concerns complaints caused by the tumours. The objective of this study was to investigate the distribution, symptomatology and histological analysis of benign tumours within the upper extremity. Materials and methods: The study group consisted of 346 patients – 234 women (68%) and 112 men (32%) at a mean age of 53 years who were operated on in the authors’ institution between 2015–2018. The criteria of inclusion were as follows: the presence of a tumour in the upper extremity which was not a ganglion cyst or malignant tumour, and available results of histopathological examination of the resected tumour. Results: Most lesions – 231 (67%) were localized in the digits, followed by the metacarpus – 46 (13.3%). The size of most of the tumours – 204 (59%) was 0.6–2 cm. The tumours were present for a mean duration of 2.6 years before surgical excision. For most patients, an unaesthetic appearance of the hand was the primary problem. Most of the tumours – 248 (72%), were symptomatic by means of pain at compression or caused mild dysfunction. Even though they contain complementary data, it is not possible to obtain comprehensive statistics based on any of the sources alone, while combining them is not obvious. Although this is possible for the first 2 sources – and only to a limited extent (as individual data are reported according to the mother’s identifiers), the statistical data from Statistics Poland are available in aggregated form only. Moreover, even after combining the data from the National Health Fund and the Department of Screening Tests and Metabolic Diagnostics at the Institute of Mother and Child and, hence, the acquisition of more comprehensive information on mothers and their newborns, these newborns are still not uniquely identifiable as they lack personal identification numbers, e.g., the Polish national identification number (PESEL). As a result, the statistics prepared for this paper are based on the estimated number of delivered newborns. As it has developed, there has been a reduction in mortality rates among prematurely born infants. The main problem faced by premature infants is respiratory disorders. The frequency of neonatal respiratory failure is inversely proportional to the gestational age and the maturity of the gas exchange system. The aim of this study was to evaluate the effect of steroid therapy in pregnant women on the occurrence of respiratory distress and the need for respiratory support in premature neonates. Materials and methods: The study covered 100 premature infants born in the Neonatal Intensive Care Unit of Zdroje Specialist Independent Public Health Care Centre in Szczecin. The newborns were divided into research groups, which consisted of newborns with prenatal steroid therapy, and a control group, which consisted of newborns without steroid therapy. The study was retrospective and was carried out based on the analysis of medical records. Results: Breathing disorders were more frequent in newborns who had received steroid therapy before birth than in children without steroid therapy. Respiratory failure occurred in children who had received both 1 or 2 doses of prenatal steroid therapy. A similar percentage of children, with and without the use of steroid therapy, required mechanical ventilation. Continuous positive airway pressure was applied both in children who had received steroids during pregnancy and those who had not. However, newborns who had received steroids during pregnancy required respiratory support for a shorter time. Conclusions: 1. Prenatal steroid therapy does not reduce respiratory disorders in prematurely born children. 2. In some cases, they are even the key tool in decision-making. However, the special characteristics of pediatric patients should be kept in mind. Examinations and procedures are routine and well-tolerated by adults, but often cannot take place without the presence of an anesthesiologist. Hydronephrosis is often diagnosed in prenatal ultrasound but is an significant socioeconomic problem. The selection of patients who absolutely require surgical intervention is an aspect to which special attention should be paid. In many centers, for children with hydronephrosis, ultrasound and scintigraphic tests play a dominant role in diagnosis and monitoring. The most common etiology of hospital-acquired diarrhea is Clostridium difficile infection (CDI). It has been demonstrated that probiotic use may be beneficial in the prevention of antibiotic-associated diarrhea. There is also growing evidence that a fecal microbiota transplant may reduce the duration and recurrence of CDI.
