Changes in the lifestyle among dialysis patients – a descriptive study 透析患者生活方式的改变--一项描述性研究

This treatment changes patients’ lives, including limitations on physical activity. In this study, we highlighted the mobility aspects of patients’ lives compared to the control group (non-dialyzed). Materials and methods: Seventy-nine dialysis patients and 125 controls were studied. The study used an original questionnaire on socioeconomic status, physical activity, and lifestyle. All data were statistically analyzed between dialysis patients and controls divided into female and male groups. Results: Most of the dialysis patients were obese, and each had concomitant chronic diseases of other organs. Significantly lower levels of employment (p = 0.001 for women; p = 0.005 for men) and physical activity (p = 0.047 for women; p = 0.001 for men) were observed in dialysis patients compared with controls. Both groups had high motivation and knowledge about the health benefits of physical activity. However, dialysis patients had significantly more concerns about engaging in physical activity (p < 0.001). Conclusions: Dialysis patients are at risk for the negative consequences of occupational and physical inactivity. Healthcare professionals (physicians and physiotherapists) can take advantage of the high motivation of dialysis patients to overcome the fear of physical activity. Based on the current study and a previous study on the posture of dialysis patients, a universal set of exercises for dialysis patients has been created and is available on the YouTube platform: https://www.youtube.com/watch?v=Ng0sVOViQ-0&feature=youtu.be. White adipose tissue, one of the largest organs, spans the entire body and persists throughout an individual’s life, with the highest concentrations found in the abdominal cavity or subcutaneously. In obese individuals, the amount of WAT can reach up to 70% of total body weight. Today, glucagon-like peptide-1 (GLP-1) analogs have gained popularity in the treatment of obesity, insulin resistance, and related metabolic disorders. Patients using glucagon-like peptide-1 receptor agonists (GLP-1RAs) have improved lipid profiles, reduced visceral fat accumulation, and improved glucose tolerance. Polycystic ovarian syndrome (PCOS) is a disorder strongly associated with insulin resistance and obesity. It is the most common heterogeneous endocrine disorder, affecting an estimated 1 in 5 women of reproductive age. The introduction of GLP-1 analog treatment in women with PCOS could help to manage the disease, improve the quality of life of PCOS patients, increase their chances of conception, and maintain pregnancy until delivery. Nonspecific clinical symptoms caused by these bacteria include respiratory diseases, which often results in delayed diagnosis. This article discusses the spread, diagnosis, symptoms, and treatment of MOTT in the world. In view of the fact that NTM are prevalent in the environment, their isolation from the upper respiratory system of the respiratory tract does not necessarily indicate an active infection. To make an accurate diagnosis, clinical, radiographic, and microbiological factors must be considered. Clinical symptoms of NTM infection can be difficult to diagnose, even for experienced clinicians, due to their similarity to Mycobacterium tuberculosis infection. Based on the determined antibiogram, targeted treatment can be selected based on identification and drug susceptibility of the microorganism. There are a variety of therapeutic options, depending on the pathogen’s growth rate. Possible but rare adverse effects of statins include tendon disorders such as tendinitis and tendon rupture. The purpose of this systematic review is to summarize current studies relating to statins and tendinopathy. Materials and methods: A search was conducted of studies on the statin-related risk of tendon disorders, published in English until August 2022. It included randomized control trials, experimental studies on human tendon samples and animal tendons, cohort studies, case series, and case reports. The data was then extracted and qualitatively reviewed. The results were tabulated and structured into increased risk, reduced risk, and no effect of statin in tendon disorders. Results: Out of 48 studies, 17 articles indicated an increased risk of tendon disorders, 6 articles found no correlation between statin use and tendinopathy, 7 articles reported decreased risk, and further 6 articles merely mentioned a tendon disorder and statin use without drawing conclusions. The remaining 12 articles were case reports with bilateral quadriceps tendon rupture as the most mentioned tendon disorder. Conclusions: Overall, statins did not seem to increase the risk of tendon disorders in a clinical setting. However, in experimental studies, statins were shown to increase the risk. It is presumed that this is due to prescribing statins in hyperlipidemia, which is a risk factor itself for tendon disorders. Controversially, statins exhibit a tendon healing property in in vitro conducted studies. It is not necessarily the result of neglected or poor hygiene. Infection with head lice can occur not only in large clusters of children but also in salons providing cosmetic or hairdressing services. Despite easy access to various pharmacological agents, pediculosis can be a challenging problem to combat. Case report: The paper presents a case of a 4-year-old boy in whom the methods of eliminating lice, used independently by his mother at home, ended in failure each time. The family turned to a trichologist for help. Conclusions: Supporting the diagnosis with the use of a micro-camera can eliminate misdiagnosed cases of head lice in the presence of dandruff, scabs, or various types of scalp impurities. Among the most common diseases of the geriatric population, cognitive dysfunction, depression, urinary incontinence, sarcopenia and frailty syndrome are distinguished, which are collectively referred to as geriatric syndromes. The aim of this review is to discuss the phenomenon of frailty syndrome, taking into account its etiology, epidemiology, diagnostic criteria and treatment options. Materials and methods: This article contains an overview of publications from the PubMed and Google Scholar databases. Results: Frailty syndrome is defined as a multi-causal medical syndrome that increases the risk of loss of independence and/or death. Data on the incidence of this phenomenon remain inconsistent. An important role in pathogenesis is played by chronic inflammation, oxidative stress, sarcopenia and vitamin D3 deficiency, but also social aspects such as loneliness. One of the most important symptoms is sarcopenia, which is manifested by the loss of strength and muscle mass, leading to motor slowdown, reduced exercise tolerance or a feeling of weakness. It may also be accompanied by abnormal coagulation, anemia, malnutrition or affective disorders. Numerous scales have been developed that are used to diagnose frailty syndrome, enabling the individualization of the diagnostic process. Physical activity and diet play a key role in the process of prevention and treatment. Conclusions: Population aging is an unquestionable challenge for modern medicine and contributes to the more frequent occurrence of frailty syndrome. Its pathogenesis is complex, but the knowledge of risk factors allows it to select a group of patients who should be carefully monitored. Recently, infections caused by these bacteria have become more dangerous due to the acquired resistance to many antibiotics, severely limiting therapeutic options. The most common mechanism of resistance in K. pneumoniae rods is the production of extended-spectrum β-lactamases (ESBL). However, a more perilous mechanism is the production of carbapenemases. The course of the infection process is also influenced by various virulence characteristics, primarily enabling adhesion and aiding in avoiding host immune responses. Most genes encoding mechanisms of resistance and virulence are located on plasmids, facilitating their spread. The aim of the study was to detect selected virulence genes among clinical multidrug-resistant strains of K. pneumoniae. Materials and methods: We determined the drug susceptibility of strains and confirmed the presence of antibiotic resistance mechanisms using phenotypic methods. Additionally, we assessed the presence of genes encoding selected resistance mechanisms and genes determining selected virulence factors. A total of 134 strains from various hospital units were used for the study. Results: The highest percentage of strains was isolated from urine (46%). Among the isolates, 72% were from male patients. Fifty-seven percent of K. pneumoniae produced ESBL (KpESBL), while the remaining 43% carried the New Delhi metallo-β- lactamase (NDM) mechanism. The drug susceptibility of the KpESBL varied, with full sensitivity observed only in the case of antibiotics from the carbapenem group. New Delhi metallo- β-lactamase-producing K. pneumoniae showed sensitivity only to amikacin and gentamicin. In KpESBL strains, genes from the TEM family were most observed (74/76). Most of the strains had all 4 β-lactamase-encoding genes (61/76). In the group of strains producing carbapenemases, only the bla NDM gene was detected. Materials and methods: A cross-sectional study was held using an original questionnaire, implemented in 2019 and 2020, with 205 medical staff as respondents (including 170 nurses and 23 doctors) in 3 hospitals in Poland. Results: According to healthcare staff opinions, the most frequent reason for the application of direct coercive measures involved the patients’ aggression directed at themselves (active self-abuse, 70.73%) or others (67.80%). Other reasons for coercion (such as damaging or throwing things, disturbing the operation of the facility, and passive self-abuse) accounted jointly for 80.04% of the cases. The main reason for the significantly more frequent application of coercion (once a month or more often) involved alcohol consumption (25.6%) and consumption of other psychoactive substances (18.84%; p < 0.001). It was significantly more frequently used (once a month or more often) for mentally ill patients (24.15%) than for patients with intellectual disabilities (5.8%; p < 0.001). The most frequently used measure was immobilisation (once a month or more often: 37.68%), and the rarest one was seclusion (6.76%). Conclusions: The most frequent reason for the application of coercive measures involved self-abuse or aggression caused by the consumption of psychoactive substances. Frequent application of the measures for other reasons that do not directly threaten human life or health requires further research. In particular, the frequent use of immobilisation compared to the infrequent use of seclusion requires further research. To protect patients’ rights, it is urgently needed to clarify ambiguous terms used to describe reasons for the application of coercive measures by the legislator. Preventing violent behaviour through architectural solutions and preparing staff and wards for crisis intervention would benefit patients and staff. The diagnosis and management of scaphoid fractures are still troublesome and sometimes controversial. This article presents several aspects of diagnosing and management of scaphoid fractures which are a matter of scientific discussion. They include diagnosis of true fractures among suspected fractures, assessment of the range of displacement, methods and time of conservative treatment in a plaster cast, indications to surgical fixation, assessment of union of the fracture, diagnosis of nonunion, and the consequences of nonunion for the wrist function.
