Striving for improved adherence and asthma control is of vital concern in today’s asthma management. In a recent document, the world health organization recognized lack of adherence as a major problem in management of chronic disease and concluded that improving adherence would have more beneficial impact on health outcome than improving specific treatment. But, study findings addressing its magnitude and associated factors are lacking in the country and hence current study was aimed to assess level of treatment adherence and factors affecting among asthmatic patients in Soddo Christian General hospital, from July to August, 2017. Methodology: A facility based cross-sectional study design was used. Data was collected through structured questionnaire to collect information on socio-demographic, socio-economic and information on medication adherence and disease related factors. Completeness of the data was checked every day and cleaned and analyzed. Descriptive analysis was computed as frequency, mean and standard deviation (SD) for continuous variables and for categorical data. The out puts of different study findings was presented using tables, graphs, & figures accordingly. Results: Among 106 patients participated in the study, 62 (59%) were males and majority of the respondents found within age range of 36-45, 36 (34%). It was found that 139 (59%) of asthmatic patients had poor adherence for which the most common factors affecting adherence were, forgetting to take the drug 25 (23.5%) and being busy with other tasks 18 (17%). Majority (79%) of asthmatic patients reported as they use alternative methods as means of treatment. Conclusion: The level of adherence to asthma medication was relatively lower when compared to others studies done in developing countries. The major reasons for missing doses were simple forgetfulness, absences of clear drug information and being busy with other things. Hence, health care providers especially pharmacists should provide accessible information to patients on their treatment plan to ensure patients keep their regular follow up, improve their patient’s confidence and to avoid drug abuse.","PeriodicalId":91516,"journal":{"name":"International journal of respiratory and pulmonary medicine","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-10-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"45084296","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

There are several well-recognized forms of vascular injury to the liver, including Budd-Chiari syndrome, ischemic hepatitis, hepatic infarction and passive congestion due to hemodynamic instability. We present a case of acute liver injury accompanied by hemodynamic and respiratory pathologies. Case: A 53-year-old lady was admitted to the emergency department with complaints of abdominal pain, nausea, vomiting for a week. Gastrointestinal examination was normal. Laboratory tests revealed mild thrombocytopenia, prolonged international normalized ratio (INR) and transaminases were ten-fold higher. In abdominal ultrasonography, no signs of chronic liver failure and vascular pathologies. Abdominal computed tomography (CT) revealed no pathology. Transthoracic echocardiography disclosed severe mitral regurgitation and dilatation of both atria and right ventricle. Pro-BNP level was increased. Thorax CT demonstrated filling defect compatible with pulmonary thromboembolism (PTE). The patient was accepted PTE, and enoxaparin treatment was initiated. Under the anticoagulant and diuretic treatments, the patient’s orthopnea disappeared, control blood values declined. Conclusion: Acute liver injury can be the initial presentation in a patient with severe valvular disease and acute PTE.","PeriodicalId":91516,"journal":{"name":"International journal of respiratory and pulmonary medicine","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-09-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48281548","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Material and methods: 142 eyes of 71 consecutive patients with COPD who are applied to ophthalmology clinic were included in the retrospective study from January 2012 to August 2013. Trauma, ophthalmologic surgery history, diabetic retinopathy, uveitis, chorioretinitis, retinitis pigmentosa, retinal detachment and tears in the anamnesis and ophthalmologic examination were excluded from study. The visual acuity of the patients was assessed using Snellen chart. Intraocular pressure was measured with applanation tonometry. Fundus examination was performed using +90D lens after pupil dilation with 0.5% tropicamide following biomicroscopic anterior segment examination. All patients underwent peripheral retinal examination with Goldmann three-mirror lens to detect the presence of retinal tear. A and B mode ultrasonography was performed to patients. Results: 142 eyes of 71 patients, 5 of whom were female (7%) and 66 of whom were male (93%) were included to the study. The patients were aged between 43-80 years and the mean age of the cases was 62.61 ± 8.34. The average visual acuity was found as 0.78 ± 0.25 (0.05-1). Mean intraocular pressure was found as 12.5 ± 2.26 mmHg (8-18 mmHg). PVD was detected in 87 eyes of 142 eyes. The prevalence of PVD was found as 61.26%. 46 (52.87%) patients of 87 cases were symptomatic. Conclusion: The PVD prevalence was higher in this age group in COPD patients than normal population. The reason of this may be concussion by increased cough reflex and amount in COPD. Pathologies that may cause increased cough reflex like COPD can be evaluated for risk factors and tendency to PVD. In conclusion, PVD is an important situation because it may lead to vitreoretinal pathologies like retinal tear, retinal detachment, intravitreal hemorrhage, macular hole. Therefore COPD patients should be evaluated by an ophthalmologist for early diagnosis and treatment.","PeriodicalId":91516,"journal":{"name":"International journal of respiratory and pulmonary medicine","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-08-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48846641","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Moreover, evaluation and management of persistent cough in pulmonary clinics account for roughly 40% of patient volume. The most common etiologies are: Upper airway cough syndrome (UACS), asthma, non-asthmatic eosinophilic bronchitis (NAEB) and gastro-esophageal reflux disease (GERD) all of which are treatable. However, despite avoidance of triggers and medical therapy, there remains an elusive diagnosis for some individuals with persistent cough. Further investigation warrants bronchoscopy, which is indicated for patients with persistent or unexplained cough. The purpose of this study is to assess the diagnostic yield of fiberoptic bronchoscopy (FB) with Broncho-alveolar lavage (BAL) for chronic cough in patients with normal chest roentgenogram. FB records were reviewed in cohort for a period of 3 years. Only patients with normal chest roentgenograms who remained refractory to medical therapy were included in the study. Fifty-two (52) cases encompassing ages 20-90 were reviewed. In 42 cases, BAL was negative for any diagnostic cause. Eight (8) cases were significant for infectious processes, one (1) case diagnosed bronchiectasis, and 1 case was lost to follow-up. Statistical analysis between the total cases in each group was statistically significant p < 0.001. Thus, though FB with BAL is indicated in chronic cough of unknown etiology, its diagnostic yield is low.","PeriodicalId":91516,"journal":{"name":"International journal of respiratory and pulmonary medicine","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-07-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"46208187","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Objective: To set an initial investigation in terms of study sample size and simple reference spirometric equations on the association of the set of conventional predictors in companion with two new predictors BMI and ABSI with spirometric lung function equations in Canadian First Nations Cree Populations. Methods: First Nations Lung Health Project (FNLHP) was conducted in rural Saskatchewan, Canada. For this analysis, we used data obtained on healthy non-smokers. There were 37 First Nations people (24 females, 13 males) from the FNLHP. Bootstrap regression technique was utilized to predict pulmonary lung functions in terms of age, weight, abdominal girth and body mass index (BMI), and a body shape index (ABSI). Main results: Controlled for age, BMI had significant association for FEF25-75 for women and no significant association for men in other outcomes. Controlled for age, ABSI had significant association for FEV1 for women and significant association for FVC and FEV1/FVC for men. In overall, ABSI was better predictor of spirometric outcomes compared to BMI. Conclusion: ABSI may be considered as a key predictor for spirometric lung functions in men and women for Canadian First Nation Populations with more significant results in men.","PeriodicalId":91516,"journal":{"name":"International journal of respiratory and pulmonary medicine","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-06-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"43937678","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
