Few numbers of publications have studied role of vitamin D in development of autism in childhood. Aim: The aim this work was to study the status of vitamin D in autistic children by evaluation of serum levels of 25-hydroxy vitamin D (25 (OH) vitamin D) in Autistic pediatric patients and to study the beneficial role of vitamin D therapy as adjuvant therapy for this disease. Materials and methods: This study was conducted on forty pediatric patients who achieved the diagnostic criteria of autism depending on the 4th edition of Diagnostic and Statistical Manual of Mental Disorders (DSM-4). Thirty age and sex matched children served as control group. They were selected from pediatric and neuropsychiatric departments of Tanta Faculty of Medicine. All subjects were subjected to complete history taking, full medical examination, childhood autism rating scale (CARS) assessment and serum 25 (OH) vitamin D assay by enzyme-linked immunosorbent assay (ELIZA). Results: This study reported that 25 (OH) vitamin D levels in children with autism was significantly lower than the controls and it had significant negative correlation with CARS which was improved after vitamin D supplementation at a dose of 400 units daily for eight weeks. {"title":"Intrauterine Bilateral Upper Limb Gangrene and Cerebral Infarction in Preterm Infant: A Case Report and Literature Review","authors":"A. The neonatal thrombosis needs to be distinguished from intrauterine arterial thrombosis as it is caused by catheterization of the umbilical artery or less commonly seen as complication of sepsis or coagulation disorders. The report describes the case of preterm infant with bilateral brachial artery thrombosis in utero and neonatal gangrene of the left forearm. The right forearm of the infant had a small patch of gangrene with irregular areas of edema and bluish discoloration, extending to the hand. The hand of infant was associated with thrombosis of left middle cerebral artery and acute infarction in left cerebral hemisphere. Both of the upper limbs in the patient were affected, including the left cerebral hemisphere; although in majority of the cases, the limbs are equally affected and the left side of the brain is preferentially involved. LMWH was administered in the present case, which significantly improved the upper right limb. Laboratory findings, including the coagulation markers within normal reference, ranges for the chronologic and gestational age. Design: Open labeled clinical trial. Setting: Level II obstetric ward of a teaching hospital in Southern India. Participants: A total of 1043 babies with a gestational age of more than 35 weeks were enrolled in the study. There were 510 babies in the probiotic Bacillus clausii intervention group and 533 babies in the control group. Intervention: Intervention group babies 37 weeks) twice a day for 3 days. Main outcome measure: The outcome measures were (i) Need of phototherapy and (ii) Duration of phototherapy. Results: A total of 32 babies in control group and 17 in intervention group required phototherapy. This difference in need for phototherapy was statistically significant between the two groups (p 0.04). Treatment with probiotic reduced the risk of need for phototherapy by 44% (RR 0.56, 95% CI 0.32, 0.99).The median duration of phototherapy in the intervention group was 18 hrs( IQR 16.50, 24.00) and that of control group was 24 hrs (IQR 18.00, 48.00). This difference in duration of phototherapy was statistically significant (p=0.027). No adverse drug reactions were noticed in the intervention group. What is already known: The management of neonatal jaundice depends on phototherapy and exchange transfusion. What this study adds: Prophylactic probiotic therapy appears to reduce the need and duration of phototherapy in neonatal jaundice. The objective of the study was to assess the effectiveness of awareness programme on knowledge and practice related to minor ailments among care givers of children under 5 years. A quantitative research approach was used for the study. Pre experimental one group pretest and posttest design was used in the study. The study was conducted in Dehradun. Systematic random sampling and consecutive sampling technique was used to select the study subjects. Data was collected from 57 care givers of under five children by conducting interview using structured knowledge questionnaire, self-reported practice check list. After that awareness programme was conducted on knowledge and practice regarding management of minor ailments among care givers of children under 5 years and the result shows that the mean posttest knowledge score and practice scores (20.74, 32.46) was higher than the mean pretest knowledge score and practice scores (14.37, 28.00) which was found statistically significant at p<0.05. Recently, it has been used to estimate myocardial iron overload in adult patients with acquired anemia. \u0000Objective: To assess the value of gradient-echo T2* in monitoring and screening of both liver and cardiac iron overload in non-transfusion dependent thalassemia (NTDT). \u0000Material and methods: This prospective study was conducted on children with mean age 11 years. Measurements were obtained in the same 1.5 MRI examination, followed by calculation with the reference spread sheet and then, the results were compared to the standard serum ferritin levels. \u0000Results: The study included 31 patients with non-transfusion-dependent thalassemia (NTT); 19 with thalassemiaintermedia, 6 with thalassemia-minor and 6 with hemoglobin-H. Mean serum ferritin was 201.8 ng/mL. Mean liver T2* was measured at 23.7 ms. Mean myocardial T2* was measured at 30 ms. Correlation analysis revealed significant negative correlation between hepatic T2* and serum ferritin (P<0.001*, R=-0.8). Week positive correlation was found for cardiac T2* (P=0.04, r=-0.37) and a week negative correlation was between hepatic and cardiac T2* values (P=0.4, R=-0.37). Statistically significant negative correlation with age was detected for hepatic T2* (P=0.001, R=-0.44) but not for cardiac T2* and serum ferritin. \u0000Conclusion: Liver and cardiac T2* measurement is the non-invasive modality of choice for monitoring and screening of both liver and cardiac iron overload in NTDT. We present a case of a 10 -year-old girl who was treated conservatively for inflammatory bowel disease for 6 years. She underwent a diagnostic colonoscopy after which she developed acute respiratory failure. Urgent bedside imaging series revealed pneumothorax, pneumomediastinum and pneumoperitoneum. A chest tube was immediately inserted, and then she was intubated and ventilated. Laparotomy revealed a 1 cm wide perforation of the transverse colon. The rupture was closed and a terminal ileostomy was performed. The postoperative course was uneventful. {"title":"A Study of Electrolyte Disturbances in a Child Presenting with Acute Gastroenteritis, with Special Emphasis on Hyponatremic Dehydration-A Hospital based Cross-Sectional Study","authors":"R. Electrolyte disturbances play an important role in the associated morbidity and mortality. Aims and objectives: (1) To study the electrolyte changes in moderate and severe dehydration in AGE in children. (2) To study the incidence and clinical features of Hyponatremic dehydration. Materials and methods: A cross-sectional type of observational study of 200 children admitted with AGE with moderate to severe dehydration was conducted at M R Bangur hospital, Kolkata. The cases were divided into two groups, one with Hyponatremia and the other with Isonatremia and the clinical features were compared. Basic crosstabulation and frequency distributions were prepared. Chi-square test and Fisher's Exact test were used to test the association between different variables. Z-test and t-test were used to test the significant differences. P ≤ 0.05 was considered statistically significant. Results: AGE constituted 18% of the total admissions. 22% had Hyponatremia, 71.5% had Isonatremia and 6.5% had Hypernatremia. Out of 30 children who were suffering from Hyponatremic dehydration and had ORS before admission, 83.3% were given diluted ORS. Clinical features significantly associated with Hyponatremia were increased frequency of diarrhea, absence of thirst, tachycardia, abdominal distension and severe dehydration. Conclusion: Hyponatremic dehydration is the second most common type of dehydration next to Isonatremic dehydration, but it is more common in children who took diluted ORS. Hyponatremic dehydration may be suspected from the history and clinical features. Increased awareness regarding ORS preparation may help in preventing Hyponatremia in AGE. What is already known: Isonatremic dehydration is the most common electrolyte abnormality found in AGE. Objective: The main objectives of the study was to compare the post-test knowledge of parents on postoperative care of cleft lip and cleft palate in control and experimental group and to assess the post-test practice of parents on post-operative care of cleft lip and cleft palate in control and experimental group. Methodology: Quasi-experimental, two groups pre-test and post-test designs were used in the study. The study was done in selected hospital Dehradun, Uttarakhand. The sample comprised of 60 parents of children with cleft lip and cleft palate who fulfilled inclusion criteria. All the participants who are available at the time of data collection were taken consecutively. Data was collected from participants by conducting interview schedule using structured knowledge questionnaire and observation practice checklist. Result: The result showed that in experimental group the mean post-test knowledge score (18.90 ± 0.95) was higher than the mean post-test knowledge score (10.46 ± 1.52) in control group which was found statistically significant at p<0.05. The mean post-test practice score in experimental group in day 1 was 10.40 ± 1.95 which was higher than the control group 7.00 ± 1.59 and the ‘t’ value was 7.36. Again the mean post-test practice score in experimental group in day 2 was 11.80 ± 1.95 which was higher than the control group 6.66 ± 1.72 and the ‘t’ value was 11.84. On third day the mean post-test practice score in experimental group was 10.76 ± 2.26 which was higher than the control group 6.43 ± 1.45 and the ‘t’ value was 8.80. Hence the score predict that there was significant difference between the mean post-test practice in experimental and control group at p<0.05 level. So there was no evidence to accept the null hypothesis, hence the researcher rejects the null hypothesis and accepted the research hypothesis.
