Reduction of the CH50 occurs when individual complement component(s) are deficient or consumed. The CH50 may be useful for a variety of medical specialties and is most helpful diagnostically when laboratory results are interpreted in the context of the history and physical exam. The primary utility of the CH50 in the practice of an allergist-immunologist is to screen for complement-deficiency associated immunodeficiency (primarily classic or terminal complement component deficiencies) and some vasculitides (usually immune-complex induced and/or small vessel vasculitis). Absent or significantly reduced individual complement components may result in recurrent sinopulmonary bacterial infections, Neisserial meningitis, or sepsis. A reduced CH50 in this situation warrants quantification and functional assays of individual complem...","PeriodicalId":88951,"journal":{"name":"Pediatric asthma, allergy & immunology","volume":"40 1","pages":"219-225"},"PeriodicalIF":0.0,"publicationDate":"2002-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"85077055","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Intravenous immunoglobulin (IVIG) is known to have immunomodulatory effects, and in a few clinical trials has been shown to have potential benefit as a steroid-sparing agent in asthma patients requiring high doses of corticosteroids. This study assessed the efficacy of high-dose IVIG in reducing the steroid requirement and subsequently improving the pulmonary function, symptomatology, and quality of life in patients with severe steroid-dependent asthma. Six immunocompetent, steroid-dependent patients were enrolled in an open-label trial of IVIG given as six monthly infusions of 2 g/kg per dose. Five patients completed the study. Of these, four showed a significant reduction in oral corticosteroid requirement while being treated with IVIG. The mean reduction was 10.1 mg/d (p = 0.02), which was an almost 50% decrease in corticosteroid requirement. The purpose of the present study was to evaluate mold-specific IgG antibodies measured 3 years apart in children exposed to moisture and molds at school. In 1996, 212 primary school students with asthma, wheezing, or prolonged cough participated in a study, including clinical examination and mold-specific IgG determinations to 24 molds. In 1999, 144 students of the same cohort participated in an identical follow-up study; among them, mold-specific IgG was measured in 121 students. The schools they attended were investigated both technically and microbiologically at both phases of the study. No association was found between IgG antibodies to molds and exposure to moisture or molds in school in either year. Genetic predisposition, a decline in infections in early childhood, and exposure and sensitization to environmental allergens all appear to play a role in the pathogenesis of asthma. Elevated immunoglobulin E (IgE) in infancy is associated with onset of the disease later in childhood. Current management strategies for persistent childhood asthma recommend the use of antiinflammatory medications, particularly inhaled corticosteroids. A promising new treatment is based on the neutralization of free IgE by humanized monoclonal anti-IgE antibody (omalizumab).","PeriodicalId":88951,"journal":{"name":"Pediatric asthma, allergy & immunology","volume":"03 1","pages":"153-162"},"PeriodicalIF":0.0,"publicationDate":"2002-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"86266637","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Its function in the allergist/clinical immunologist's practice is to assess patients with an unusual presentation of a normally encountered condition for an underlying disorder, e.g., a severe asthmatic with Churg Strauss syndrome. It is also used to identify potential etiologies of some disorders, such as vasculitis causing chronic urticaria. It is neither completely sensitive nor specific for diagnosing any disorder; thus, it is adjunctive to the history, physical exam, and other laboratory and radiological studies. The ESR does not have a role in assessing uncomplicated atopic conditions.","PeriodicalId":88951,"journal":{"name":"Pediatric asthma, allergy & immunology","volume":"1 1","pages":"167-171"},"PeriodicalIF":0.0,"publicationDate":"2002-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"79619984","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The purpose of this study was to examine the possible impact of parental anxiety and depression on the course of asthma, as well as to examine the relationship between left-handedness and asthma in children. The International Study of Asthma and Allergy in Childhood (ISAAC), the Foulds and Bedford Inventory for Anxiety and Depression (DSSI/sAD), and the Edinburgh Left-handedness Inventory were administered to 70 families with asthmatic children 4-8 years old. One year later, the children's asthma was reevaluated using a brief questionnaire based on ISAAC. A sample of 70 families with nonasthmatic healthy children were used as controls. The majority of children in the study group had mild asthma. Allergic rhinitis was found in 31.4% and allergic dermatitis in 20% of the sample. The parents of the asthmatic children scored significantly higher in DSSI/sAD compared to parents of the controls.
