Although it does not provide a specific diagnosis or etiology, it can identify abnormalities in lung volumes, airflow, diffusion, respiratory endurance, and respiratory muscle strength. Pulmonary exercise testing can help differentiate pulmonary from cardiac disease with exercise, detect the presence of ventilatory and/or cardiac limitation with exercise, and measure the degree of physical fitness. These studies have a role in the research of lung disorders in children because changes in lung function with age and alterations in lung growth secondary to acute and chronic disease can be identified.","PeriodicalId":88951,"journal":{"name":"Pediatric asthma, allergy & immunology","volume":"2 1","pages":"193-213"},"PeriodicalIF":0.0,"publicationDate":"2003-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"82448563","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, chest physical therapy is not generally recommended for acute asthma. Flutter VRP1 is a hand-held device approved by the Federal Drug Administration in the United States for patients with cystic fibrosis (CF) and chronic obstructive pulmonary disease (COPD) to promote mucus clearance. The purpose of this study is to determine whether the addition of chest physical therapy such as the Flutter VRP1 device has any effect on pediatric patients hospitalized with acute asthma in an open label, prospective, randomized-controlled clinical trial. Forty-five children between 6 and 16 years of age who were admitted to a tertiary care children's hospital in a large metropolitan area with uncomplicated acute asthma were enrolled in the study. Of these, 20 were randomized to receive the Flutter treatment in addition to the standard therapy for acute asthma upon hospitalization and compared with 20 who re...","PeriodicalId":88951,"journal":{"name":"Pediatric asthma, allergy & immunology","volume":"16 1","pages":"295-303"},"PeriodicalIF":0.0,"publicationDate":"2003-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"78011007","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The tuberculin skin test using 5 TU of purified protein derivative (PPD) applied by the Mantoux technique remains the primary method of identifying children infected with Mycobacterium tuberculosis or Mycobacterium bovis. PPD is injected intradermally, and induration is measured 48-72 hours after application. Interpretation of the skin test reaction depends on the age of the child and presence or absence of risk factors for acquisition of tuberculosis, irrespective of previous immunization with bacille Calmette-Guerin (BCG). False-positive and false-negative skin test reactions can occur. Following identification of a positive tuberculin skin test reaction, a complete history, physical examination, chest radiographs, and laboratory studies are necessary to determine the appropriate chemotherapeutic agents to treat tuberculosis infection or disease. Initially it is important to differentiate wheezing from \"noisy breathing\" due to stridor. By definition, wheezing is a prolonged or continuous musical adventitial lung sound of varying intensity due to the obstruction of the intrathoracic airways. In contrast, stridor is a high-pitched, loud respiratory sound caused by oscillations of narrowed large extrathoracic airways. Specific diagnostic criteria for asthma in infancy are not well-defined. Since wheezing is often associated with viral respiratory infections in infants, it is difficult to distinguish an initial episode of asthma triggered by a viral respiratory infection from acute viral bronchiolitis. The history is one of the most important components of the evaluation of the wheezing infant. Birth history, age of onset, history of maternal smoking, atopy, and clinical circumstances associated with the wheezing episodes may suggest a diagnosis other than asthma. Children and adolescents with chronic respiratory symptoms require thoughtful and complete evaluation to avoid misdiagnosis with the potential for harm due to inappropriate medical therapy. This is particularly true of patients with vocal cord dysfunction, which is a common mimic of asthma. In this report, we present the case of an adolescent female athlete with exercise-induced vocal cord dysfunction misdiagnosed as asthma. Significant adrenal suppression resulted due to high doses of inhaled corticosteroids. The diagnosis of exercise-induced vocal cord dysfunction was confirmed by performing a flexible laryngoscopy during a maximal cardiopulmonary exercise test. We review our experience with a technique of combined exercise testing and real-time flexible laryngoscopy in athletes wi...","PeriodicalId":88951,"journal":{"name":"Pediatric asthma, allergy & immunology","volume":"45 1","pages":"215-224"},"PeriodicalIF":0.0,"publicationDate":"2003-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"73721216","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The purpose of this study was to evaluate a model to compare systemic activity of inhaled corticosteroids given in topically equipotent doses in children. Systemic effects were measured by 24-hour urinary free cortisol (UFC) and a.m. and p.m. serum osteocalcin. Efficacy was monitored by a.m. and p.m. exhaled nitric oxide (eNO) and a.m. peak expiratory flow (PEF). Twenty-one children, 6-16 years old on inhaled corticosteroids at ≥400 μg/day beclomethasone dipropionate (BDP) equivalent were placed on BDP for a 2-week, open-label, run-in, baseline period. Patients were then randomized into a double-blind, double-dummy, crossover trial for two 3-week periods of equipotent doses of fluticasone propionate (FP) or triamcinolone acetonide (TAA) as defined by the NHLBI Expert Panel Report 2.
