{"title":"In vitro fertilization outcomes In infertile women with adenomyosis","authors":"Giang Tran Ngoc Ha, Tam Le Minh","doi":"10.34071/jmp.2023.4.8","DOIUrl":"https://doi.org/10.34071/jmp.2023.4.8","url":null,"abstract":"Objectives: This study aimed to investigate the efficacy of in vitro fertilization (IVF) in infertile patients with adenomyosis and to identify relevant factors.\u0000\u0000Subjects and methods: Retrospective descriptive study of infertile cases with adenomyosis who received IVF therapy and embryo transfer from November 2013 to October 2022 at the Center for Reproductive Endocrinology and Infertility, Hue University of Medicine and Pharmacy Hospital, excluding cases of oocyte donation or surrogacy. The β-hCG test was examined two weeks following embryo transfer. Then, women with hCG positive test were followed the pregnancy at 6 weeks, 8 weeks, and 12 weeks, and examined some factors that influence the clinical outcome of pregnancy.\u0000\u0000Results: Among 61 cycles of IVF treatment for infertile patients with adenomyosis, the average number of retrieved oocytes was 10.9±6.6 oocytes. The percentage of mature oocytes was 82.7%, the fertilization rate was 79.5%, the implantation rate was 16.7%, the clinical pregnancy rate was 19.7%, the miscarriage rate was 6.6%, and the ongoing pregnancy rate was 13.1%. In the group with GnRH agonist administration before embryo transfer, the pregnancy rate was greater than in the group without therapy (29.2% vs. 13.5%), and th e pregnancy rate in the group with > 10 oocytes was higher than in the group with ≤ 10 oocytes (28.0% vs 13.5%). However, these differences were not statistically significant (with p > 0.05). In the periodontal disease treatment strategy, nonsurgical treatment is considered as the initial phase for anti-infection and soft-tissue management.\u0000\u0000Objective: This study aims to compare the effect of two nonsurgical periodontal modalities: one-stage full-mouth and partial-mouth protocols for periodontitis. Materials and Methods: 60 patients with chronic periodontitis were randomly allocated to 2 groups. Group I (n = 30) was treated according to partial-mouth therapy. Group II (n = 30) was treated according to full-mouth disinfection therapy. Periodontal parameters were assessed at baseline and 1, 3, and 6 months, including plaque index, gingival index, periodontal probing depth, clinical attachment loss, and bleeding on probing.\u0000\u0000Results: Both two treatment modalities resulted in significant improvements in all clinical parameters over the entire duration of the study (p < 0.05). Full-mouth disinfection therapy showed significantly better improvements than the partial-mouth one during follow-up times (p < 0.05).\u0000\u0000Conclusion: Nonsurgical periodontal treatment has positive effects on controlling periodontitis. Developing and applying the most appropriate and effective methods for diagnosing DR-TB from clinical samples is necessary, allowing a more rapid detection method for large-scale screening.\u0000\u0000Methods: Applying real-time PCR Taqman allelic discrimination with a PCR Taqman probes panel to identifying the DR-TB associated mutations in rpoB and katG of Mycobacterium Tuberculosis from isolates and clinical samples.\u0000\u0000Results: Comparing results of the real-time PCR allelic and DNA sequencing results, the sensitivity and specificity for Isoniazid resistance detection by analysing katG were found 95%(75.1 - 99.8) and 100%, Rifampicin resistance determining region (RRDR) of rpoB were found 95.5(77.16 - 99.88) and 100%, respectively. To date, orlistat is the only pancreatic lipase inhibitor approved for the long-term treatment of obesity. Therefore, there is an elevated need to find new drugs for this disease. Determining the factors affecting the test to evaluate pancreatic lipase inhibitory activity in order to build a standard assay procedure is necessary. This will make it much easier for researchers to find novel compounds that inhibit the enzyme. Materials and method: The current study investigated the factors influencing pancreatic lipase activity and evaluated the enzyme inhibition of orlistat by spectrophotometric method at 405 nm using p-nitrophenyl palmitate as a substrate. Results: With the optimized conditions, the test to evaluate pancreatic lipase inhibitory activity of orlistat gave results similar to those published by other authors. Surgical treatment must ensure safe surgical margins and effects in functional and cosmetic aspects. Therefore, the selection of surgical excision and postoperative reconstruction methods remains a challenge in treatment.\u0000\u0000Methods: We conducted a prospective descriptive clinical intervention study with 34 patients with basal cell carcinoma of the face (35 lesions) who met the inclusion and exclusion criteria of local flaps of the face for reconstruction after wide local excision in facial basal cell carcinoma at Hue University of Medicine and Pharmacy Hospital from January 2017 to April 2021.\u0000\u0000Results: The most common sites of tumor were nose and cheek (68.5%); with clinical features mostly non-healing ulcers (54.3%). The safe surgical margins were 2-10mm in 94.3% of cases. Kidney Yin deficiency syndrome accounted for 49.1%, kidney Yang deficiency syndrome 36.4% and kidney Yin Yang deficiency syndrome 20.9%, the average score of kidney Yin was higher than that of kidney Yang. There was difference in kidney Yang score between the body types, in kidney Yin score, kidney Yang and the presence of kidney Yin Yang deficiency syndrome between infertility types.\u0000\u0000Conclusion: Symptoms in the kidney Yin deficiency syndrome and kidney Yang deficiency syndrome appeared at a rather high rate. Kidney Yin deficiency syndrome accounted for the highest rate, followed by kidney Yang deficiency syndrome and kidney Yin Yang deficiency syndrome. Kidney Yang score in overweight and obese groups were higher than in lean and normal groups. Many studies emphasized the importance of ferrule effect and post length in strengthening endodontically treated teeth. However, the simultaneous influence of 2 factors on increasing fracture resistance is still controversial.\u0000\u0000Objectives: To evaluate the influence of ferrule effect and depth of glass fiber post insertion on the fracture resistance and the failure mode of endodontically treated teeth.\u0000\u0000Material and methods: In this in vitro study, sixty sound mandibular first premolars were performed endodontic treatment, and the crown was removed at 2 mm above the cementoenamel junction. Combinations of post lengths of 7 mm, 9 mm, and ferrule lengths of 0 mm and 2 mm formed four different groups consisting of 15 teeth each. The coronal sections were restored by composite build-up technique and received a full metal crown. The fracture load and failure pattern of each group were evaluated by applying a compressive load to the buccal surface of the tooth until the failure occurred. The data were analyzed with SPSS version 20.0.\u0000\u0000Results: The mean fracture load of 4 groups is 459.97 ± 115.92N, 495.96 ± 97.92N, 298.54 ± 82.46 N, 394.40 ± 88.67N, respectively. The difference in resistance to fracture between groups with ferrule and without ferrule or between groups with different post length insertions were statistically significant (p < 0.05). The number of favorable fractured teeth was highest in the group with ferrule effect and post length at 9 mm, whereas the lowest in the group without ferrule effect and with shorter post length.\u0000\u0000Conclusion: Both ferrule effect and post length play an important role in improving the fracture resistance to compressive load in endodontically treated teeth. There is a lot of evidence that global longitudinal strain is more sensitive than the left ventricular ejection fraction for the assessment of left ventricular dysfunction, is used to follow-up cancer therapeutic-related cardiac dysfunction, stratify risks, classify and determine treatment in the asymptomatic left ventricular dysfunction. Thus, this technique can help identify ventricular function and subclinical cardiac dysfunction, even when the ejection fraction is normal. Currently, speckle tracking echocardiography has been standardized and applied to evaluate left ventricular function, right ventricular function and left atrial function according to the consensus of American Society Echocardiography and European Association of Cardiovascular Imaging. White matter integrity analysis has been performed on many different groups of cranial neuropathies. Migraine is a typical unilateral pulsating headache disorder. Although the prevalence of migraine is very high, the pathophysiology is still unclear. From the hypothesis, migraine and persistent migraine may affect the white matter microstructure image. Currently, self-microemulsifying drug delivery systems (SMEDDS) are used to stabilize drug compounds and enhance solubility and absorption of poorly soluble and poorly absorbable drugs. Therefore, the objective of this study was to develop and characterize a SMEDDS formulation containing IBU for oral administration.\u0000\u0000Materials and methods: The study utilized IBU (from China), various oils (such as oleic acid, Peceol, etc.), surfactants (such as Acrysol EL-135, Acrysol K-140, etc.), and co-surfactants (such as propylene glycol, Lauroglycol 90). The study investigated and selected the appropriate excipients and their ratios to optimize the formulation, and evaluated various aspects such as appearance, emulsification capacity, droplet size, size distribution, and drug content.\u0000\u0000Results: The results showed that the IBU-containing SMEDDS consisted of IBU 8.7%, Capryol 8.7%, Acrysol EL-135 66.08%, and propylene glycol 16.52%, with a droplet size of 16.33 ± 0.124 nm and a PDI of 0.185 ± 0.003.
