According to statistics, DD is among the top five most common colorectal diseases. Complicated course of DD is observed in 12-15% of patients, and in 10.7% of cases can lead to death. Algorithms and recommendations for predicting the complicated course of DD for further prevention have not been developed.</p><p><strong>Objective: </strong>Comparative analysis of clinical data and structural characteristics of the colonic muscular tissue in patients with different course of DD and without colonic diverticula to identify morphological predictors of a complicated DD.</p><p><strong>Material and methods: </strong>A comparative analysis of clinical data, pathomorphological and ultrastructural changes of the colonic musculature in the surgical material from 68 patients undergoing left-sided hemicolectomy was carried out.</p><p><strong>Results: </strong>The operative material of 37 patients with complicated course of DD, 19 with uncomplicated course of DD and 12 without diverticula was analyzed. In men, this disease occurs at a younger age than in women (Median age of men 49 (39.5; 61) years, women 66.5 (58; 81) years; <i>U</i>=178, <i>p</i><0.001). The main triggers for the development of mucormycosis are: immunodeficiency states, use of glucocorticosteroid drugs, decompensation of diabetes mellitus, concomitant diseases, age > 65 years. We present a clinical case of rhinocerebral mucormycosis in a 79-year-old patient against the background of uncontrolled type 2 diabetes mellitus with ketoacidosis, a condition after previous glucocorticosteroid therapy for COVID-19 (according to the severity of the disease). After suffering a new coronavirus infection caused by the SARS-CoV-2 virus, she was admitted to the hospital with complaints characteristic of mucormycosis. On the 5<sup>th</sup> day of hospital stay, the patient's condition worsened significantly, despite the correction of the therapy, and on the 12<sup>th</sup> day the patient died. OSCC arising in the background of OSMF (OSCC with OSMF) has been a topic of interest among researchers recently and a few studies have considered this to be a distinct clinicopathological entity. This systematic review analyses the demographic and clinicopathological variations of OSCC with OSMF from conventional OSCC to evaluate the distinctiveness of OSCC with OSMF. A comprehensive search from PubMed, Google scholar and manual search were carried out and 4 articles were retrieved and analysed systematically. Out of the total 377 OSCC with OSMF cases and 542 conventional OSCC, males were found to be predominantly affected (82.7% and 73.6%). 47% of the OSCC with OSMF cases were well differentiated squamous cell carcinomas as against 33.4% in conventional OSCC. Lymph node metastases were seen predominantly in conventional OSCC (49.1%) than OSCC with OSMF cases (40.7%). OSCC with OSMF were more prevalent in males and showed better tumour differentiation and lesser lymph node metastasis. Despite their origin from active germinal centers, in some cases LP-cells express IgD, which is characteristic of naive B-lymphocytes of the mantle zone. Due to the rarity of NLPHL, assessing the frequency of IgD-positive cases is difficult. This marker can serve not only for differential diagnosis with other diseases, but also indicate the possible heterogeneity of NLPHL, which is currently represented by six immunoarchitectural patterns.</p><p><strong>Objective: </strong>To determine the frequency of IgD-positive cases of NLPHL in children with subsequent assessment of the association with types of immunoarchitectural patterns.</p><p><strong>Material and methods: </strong>The study included 52 cases of NLPHL, which were divided to typical and atypical patterns. Differences between two groups were compared using Fisher's exact tests.</p><p><strong>Results: </strong>IgD expression was found in LP-cells in 26 of 52 cases (50%) and was positively correlated with atypical types (typical - 5/23, 21.7% vs atypical - 21/29, 72.4%, <i>p</i>=0.0003), among which pattern C was most common.</p><p><strong>Conclusion: </strong>Due to the high incidence of IgD-positive cases in NLPHL, this marker may be useful in differential diagnosis with histologic mimics. {"title":"[Exosomes' role in intercellular interactions in different variants of lung injury in fatal cases of COVID-19].","authors":"E A Zarubin, E A Kogan, N V Zharkov, A M Avdalyan, D N Procenko","doi":"10.17116/patol20248602122","DOIUrl":"https://doi.org/10.17116/patol20248602122","url":null,"abstract":"<p><strong>Background: </strong>Extracellular vesicles are surrounded by a phospholipid bilayer, carrying various active biomolecules and participating in many physiological and pathological processes, including infectious ones.</p><p><strong>Objective: </strong>To research the role of exosomes in intercellular interactions in the pathogenesis of various types of lung damage in fatal cases of COVID-19.</p><p><strong>Material and methods: </strong>We conducted a clinical and morphological analysis of 118 fatal cases caused by coronavirus infection in Moscow. We selected 32 cases with morphological signs of various types of lung lesions for immunohistochemical reaction (IHC) with antibodies against tetraspanin proteins (CD63, CD81), which are involved in the assembly of exosomes, as well as with antibodies against viral proteins: nucleocapsid and spike protein. We determined the main producing cells of extracellular vesicles and cells containing viral proteins, carried out their comparison and quantitative analysis.</p><p><strong>Results: </strong>IHC reaction with antibodies against CD63 showed cytoplasmic granular uniform and subapical staining of cells, as well as granular extracellular staining. We determined similar staining using antibodies against viral proteins. Extracellular vesicles were found in the same cells as viral proteins. All cases were verified according to the WHO Classification of adrenal gland tumors (5<sup>th</sup> ed., 2022), the tumor's histogenesis was confirmed by immunohistochemical examination. Statistical analysis of the histological and immunohistochemical factors in terms of their value in relation to the diagnosis of ACC was carried out on Python 3.1 in the Google Colab environment. ROC analysis was used to identify critical values of predictors. The cut-off point was selected according to the Youden`s index. Logistic regression analysis using l1-regularisation was performed. To validate the model, the initial sample was divided into training and test groups in the ratio of 9:1, respectively.</p><p><strong>Results: </strong>The study included 143 patients divided into training (128 patients) and test (15 patients) samples. A prognostic algorithm was developed, which represent a diagnostically significant set of indicators of the currently used Weiss scale. The diagnosis is carried out in 3 stages. This mathematical model showed 100% accuracy (95% CI: 96-100%) on the training and test samples.</p><p><strong>Conclusion: </strong>The developed algorithm could solve the problem of subjectivity and complexity in the interpretation of some of the criteria of current diagnostic algorithms. Morphologic diagnosis is sometimes difficult, in which case it is supplemented by immunohistochemical (IHC) examination.</p><p><strong>Objective: </strong>Studying the role of IHC analysis in the differential diagnosis of APT and PC.</p><p><strong>Material and methods: </strong>The study included 44 patients with morphologic diagnosis of the APT established after surgical treatment for primary hyperparathyroidism on the basis of Endocrinology Research Centre during 2018-2023. All cases underwent IHC examination with evaluation of CD31/CD34 and parathormone (PTH) expression for identification of vascular invasion, Ki-67, parafibromin.</p><p><strong>Results: </strong>According to the results of IHC analysis in 8/44 patients (18.2%) the diagnosis of APT was revised in favor of the PC: in 7 of them vascular invasion was detected; in 1 patient the additional series of slices in the surrounding fatty tissue revealed foci of tumor growth, confirmed by positive reaction with antibodies to PTH. According to IHC results, the material was divided into 2 groups: APT and PC. There were no differences in clinical and morphological characteristics, Ki-67% level and parafibromin expression between the groups.</p><p><strong>Conclusion: </strong>Assessment of clinical and laboratory-instrumental data at the preoperative stage does not allow differentiating APT from PC. However, there is little information in the literature about the mechanisms of thanatogenesis in patients who have had COVID-19. An analysis of clinical and laboratory parameters and pathomorphological changes was carried out in 9 autopsy cases of patients who had previously suffered a new coronavirus infection (COVID-19). The age of the deceased ranged from 80 to 96 years. At the time of hospitalization, the concentration of IgG varied from 32.61 to 1013.5 RLU, IgM - from 0.29 to 16.98 U/ml. The period from clinical diagnosis to death ranged from 12 to 46 days, and the time from clinical recovery (negative polymerase chain reaction) to death ranged from 2 to 30 days. The article presents literature data and describes our own observation of a papillary kidney tumor with reverse nuclear polarity in a 73-year-old patient. The difficulties of preoperative diagnosis of a tumor are shown, histological and immunohistochemical criteria for diagnosis and differential diagnosis of this tumor with other kidney tumors are presented. There is increasing evidence of CAF heterogeneity and their different roles in tumor progression. However, there is no single marker identifying CAF, and there is also no understanding of the similarity of CAF markers in various tumors of the genitourinary system.</p><p><strong>Objective: </strong>To evaluate the response of CAF markers - FAP and PDGFR<sub>α+β</sub> in bladder cancer and prostate cancer.</p><p><strong>Material and methods: </strong>An immunohistochemical study with antibodies to FAP and PDGFRα+β was conducted on material from 34 patients with prostate cancer and 44 patients with bladder cancer in order to identify statistical relationships with stage, morphological characteristics and survival, as well as to understand the applicability of the same markers for cancer of different localizations.</p><p><strong>Results: </strong>An increased FAP reaction was shown in prostate cancer with metastases (<i>p</i>=0.0239), a significantly higher number of recurrences in the group with a high PDGFR<sub>a+β</sub> (3+) reaction (<i>p</i>=0.0018); the presence of FAP reaction only in invasive bladder cancers (<i>p</i>=0.0094) and bladder cancers with high grade dysplasia (<i>p</i>=0.0000234), the absence of a relationship between the level of PDGFR<sub>a+β</sub> and clinical and morphological characteristics in bladder cancer.</p><p><strong>Conclusion: </strong>Our results confirm the heterogeneity of CAF in both bladder and prostate tumors and indicate that FAP is one of CAF subpopulation marker.</p><p><p>Considering that FAP is expressed in a population of CAFs that lead to resistance to treatment with immune checkpoint inhibitors in melanoma, the FAP marker may also be diagnostically significant for tumors of the genitourinary system, especially given the fact that bladder tumors are treated with the BCG vaccine.</p>","PeriodicalId":8548,"journal":{"name":"Arkhiv patologii","volume":"86 6","pages":"28-35"},"PeriodicalIF":0.0,"publicationDate":"2024-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"142833352","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
