Therefore, we surveyed to assess the knowledge of radiation protection and dose assessment among clinicians. Methods: A cross-sectional web-based survey where the link was circulated to 87 clinicians who requested diagnostic radiological imaging. A validated questionnaire consisted of 20 multiple-choice questions divided into three sections, viz, demography, awareness of radiation risk, and knowledge of radiation dose, were asked to reply. Results: A total of 87 physicians from 15 different medical specialties were included in the analysis. Interns had the highest knowledge score in radiation protection (59.5%, P=0.198), while the dose assessment score was highest among radiologists (50.8%, P=0.013). The average knowledge of radiation protection was 43% ± 17%, and awareness of radiation dose was 30% ± 18%. Clinicians having experience >20 years were found with the least score (33.3%) of radiation protection knowledge, while those with <3 years of experience showed a better score (48%, P=0.064). Clinicians who claimed to be attending radiation protection courses regularly were found with better scores of knowledge (47.6%, P=0.340) and dose assessment (37%, P=0.161), although the difference was nonsignificant. Conclusion: The clinician’s knowledge of radiation protection and dose assessment is inadequate which could infer a propensity of the inappropriate use of radiation imaging. There is a substantial need for training/refresher courses to improve the knowledge of radiation dose and consequent risk in order to assure patient safety during radiological examinations. It is rarely reported to present because of cervical disc herniation (CDH), especially as a result of 2 consecutive discs prolapse, with only 8 cases reported worldwide, 4 of which have the cervical disc levels of C5-C6, C6-C7 being affected just like the case we are presenting. A 45 years male had complained of left-sided body pain and numbness following a fall, his symptoms progressing over two weeks to left side hemiparesis and impaired sensation; MRI of C spine revealed C5-C6, C6- C7 disc prolapse with cord compression resulting in BSS. The patient was treated surgically by anterior cervical discectomy and fusion (ACDF) followed by physiotherapy. The patient showed complete recovery in terms of sensory and motor deficits in both the right and left upper and lower limbs. We are stressing the role of detailed history, comprehensive neurological examination, and proper imaging in the early diagnosis of BSS. Treatment with anterior surgical intervention is the recommended approach in most reported cases and is associated with an excellent prognosis.","PeriodicalId":85027,"journal":{"name":"Journal of the Bahrain Medical Society = Majallat Jam'iyat al-Atibba' al-Bahrayniyah","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69075538","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The patient had a history of mild developmental delay, failure to gain weight, and frequent vomiting. Anesthetic implications of RSS, including airway management, glucose homeostasis, hypothermia, and pharmacological considerations, were considered during the patient's pre-operative assessment and intraoperative management. The patient was induced with propofol and fentanyl, and maintenance anesthesia was achieved with sevoflurane. The patient's airway was safely secured, and extubation was performed smoothly. The anesthetic management proceeded without complications, ensuring a successful outcome. This case report highlights the importance of considering the specific anesthetic implications of RSS in the perioperative management of patients with this rare genetic syndrome.","PeriodicalId":85027,"journal":{"name":"Journal of the Bahrain Medical Society = Majallat Jam'iyat al-Atibba' al-Bahrayniyah","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69075468","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Recognition of early outcomes associated with the procedure can provide members of the healthcare team with a better awareness of their occurrence, hence can impact on important decisions with respect to provision of monitoring, diagnosis, and further treatment. Objective: The objective of this study is to determine the early outcomes of patients who have undergone on-pump CABG at Mohammed Bin Khalifa Bin Salman Al Khalifa Cardiac Center, Bahrain Defence Force Hospital (BDF) in the Kingdom of Bahrain. Method: This is a retrospective medical record review of patients who have undergone on-pump CABG at BDF Hospital from January 1, 2010 to December 31, 2010. Data extraction was carried out by one investigator and cross checked. Results: 150 patients were included in this study, 85.3% were between 40 and 59 years of age and 70% were male. 43.3% of patients stayed in the Intensive Care Unit (ICU) for 5 days. One patient had congestive heart failure and another one had stroke. Two patients had acute renal failure. Post-op ejection fraction at 30-40% was seen in 35% of patients. Blood loss in 25% of patients was estimated at 150cc. Post-operative wound complication occurred in 3 patients, and in one this necessitated sternal debridement. Conclusion: On-pump coronary artery bypass grafting (CABG) is increasingly being undertaken for Bahraini men aged 40-59 years, with ejection fraction lower than normal levels who have multi-vessel disease. The relative safety of CABG was illustrated by the comparatively low mortality rate.","PeriodicalId":85027,"journal":{"name":"Journal of the Bahrain Medical Society = Majallat Jam'iyat al-Atibba' al-Bahrayniyah","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69073731","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: This is a retrospective, cross-sectional study including the first 247 confirmed cases in Bahrain. Demographic, clinical, and laboratory data were extracted from electronic medical records. Results: Mean Standard Deviation (SD) age of patients was 44.15 (16.5) years. More males were affected by the disease (61%; 151/274). Mean (SD) of the duration between confirmation and discharge was 9.8 (5.1) days. Of 247 patients, 4 deaths were reported (1.6%); 17.5% (24/137) showed a temperature of >37- on admission, with 4% (6/148) yielding an oxygen saturation of 94% or less. Leukopenia was reported in 36.8% of patients (63/171). One quarter of patients (25.5%) received oseltamivir, 24.7% received hydroxychloroquine sulfate (24.7%), and 1.2% received steroids. Conclusion: In this study, the authors have captured the epidemiological and clinical profiles of the first cases of COVID-19 pertaining to the first wave of the pandemic in Bahrain. The early strict measures may have contributed to the lower incidence as well as lower morbidity and mortality of COVID-19 infection in Bahrain. When superimposed with pregnancy, most physicians fear its health complications and the role of the treatment and its safety for the fetus. This case presents a 37-year-old woman diagnosed with juvenile parkinsonism who was blessed with her second child, eight years after her first. Despite all the odds, concerns, and warnings from family and physicians, the patient was determined to conceive, avoiding all means of contraception. During pregnancy, the patient experienced multiple hypoglycemic attacks and was diagnosed with gestational diabetes mellitus, which was controlled accordingly. The patient also suffered from motor impairments that worsened with the progression of pregnancy. However, the patient regained previous motor function upon delivery. The expectation that pregnancy may permanently worsen the symptoms of Parkinson’s disease is not explicit; some pregnancies are uncomplicated by Parkinsonism yet complicated by pregnancy-induced medical conditions. As demonstrated in this case, family support and care, alongside continuous maternofetal monitoring, aids in the success of pregnancy in patients with juvenile Parkinson’s disease regardless of their risks.","PeriodicalId":85027,"journal":{"name":"Journal of the Bahrain Medical Society = Majallat Jam'iyat al-Atibba' al-Bahrayniyah","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69074661","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

As the pandemic moves towards endemic status, little knowledge exists on how it has affected HCWs in the Kingdom of Bahrain. This study aims to evaluate the psychological impact and related factors of HCWs during the COVID-19 pandemic. Factors were assessed at two time periods: 1- July 2020 (T1: 6 months after the pandemic started in Bahrain) and 2- 6-month follow-up during January 2021 (T2: 12 months). Methods: An online cross-sectional survey using standardized questionnaires; Impact of Event Scale-Revised (IES-R), the Beck Anxiety Inventory (BAI), and factors including lifestyle, exposure to COVID-19, and knowledge of the centre’s psychological support services was circulated during both the time periods. Results: Based on a sample size of 366 (T1) and 537 (T2) HCWs, mild/positive PTSD prevalence increased from 18.9% (T1) to 19.6 % (T2), but anxiety decreased from 31.1% (T1) to 27.4% (T2). Sleep-related disturbances significantly decreased from T1 to T2 (32.5 % vs 24.8%, p = 0.013). Although the frequency of HCWs with psychological distress and anxiety did not change significantly over time, there was a decrease in severity of both psychological distress (p=0.06) and anxiety (p=0.005). HCWs (75%) who were aware of available psychological support services also had lower IES-R and BAI scores. Conclusion: COVID-19 has caused psychological distress and anxiety to HCWs, but rates were lower compared to other studies. Further development of COVID-19 leadership strategies incorporating interventions focused on psychological support for HCWs is needed. The outcome of SCT is dependent on the growth rate, the presence of solid components in cancer, and the presence or absence of high flow cardiac involvement of the fetus. This case report presents the management and outcome of a pregnant woman carrying a fetus with a rapidly growing SCT. Confirmation of diagnosis was based on the universal prenatal ultrasound examination standards. Eventually, the patient delivered a girl weighing 2.9 kg, including the neoplasm, by a planned Cesarean section. The newborn received one dose of surfactant and underwent complete resection of the teratoma on the second day with a favorable outcome. This case report aims to show that rapidly growing SCT requires frequent antenatal follow-up with serial prenatal ultrasound examinations, optimum timing, mode, and appropriate place of delivery for a better fetal outcome.
