The first section, titled \"The Ottoman Concept of Waqf\", gives a brief account of waqfs, their roles and purposes. The second section under the title of \"The Translation of the Document into the Turkish\" constitutes our target research subject. The document examined was written by one of the Müteferrikas of the Imperial Court, Zaim Haci Ibrahim Agha, son of Hüseyin. It is registered among the documents of the Directorate of Pious Foundations: 739/259/173, 739/253/157 and dates 1176 (Hijri calendar), 1762-1763 (Gregorian calendar). The document is about a medical dimension of the waqfs, which was not much encountered before. It gives the contents of a medicine in detail, which was stated to be good for eye diseases. The ingredients of the medicine and their proportional amounts are described clearly. Then, how to prepare the medicine is also explained with equal clarity. Referring to the written medical works, the pronunciations of the ingredients in the content of the medicine are given textually. The names of some ingredients, though legible, whose meanings could not be found, are given in Turkish transcription with a question mark. It is a place where different civilizations and cultures met and transformed each other by interaction. History of important immigrations to Anatolia during the reign of the Ottoman Empire goes back to 1683. Also, the period between the end of 19th century and the beginning of 20th century was the time of wars, immigrations, and death and birth of nations. The Ottoman Empire had joined the several wars as well, and the new Republic of Turkey was established after it collapsed. He graduated as a military doctor from Ankara University in 1946, later obtaining a specialty in bacteriology and infectious diseases and working as the hospital director at the Heybeliada Sanatorium during 1953-1958. His first visit to the US was in 1958, when he spent time in at various military hospitals in Washington DC as an observer. He continued his career in the US in various hospitals in Boston, Massachusetts, Pittsburgh, Pennsylvania and Waco, Texas. During the years that he worked in Boston, he also taught courses at the Tufts University Medical School Subsequently Dr. Altuğ worked in hospitals within the U.S. Veterans' Administration from 1980 until 1995 when he retired. And it is found out via our paper about \"neurosurgical operation\" that the above mentioned work can be studied for researches concerning the history of medicine. It is seen that Evliya Celebi reports a neurosurgical operation for curing a gunshot wound, and that he is very well aware of medical paradigm of his era. And he reports the situation without departing from the work and the dominant literary language of the work and without passing over the technical details, and he also does not get lost in the details. He also reports all the general stages of the operation by giving all the techniques and methods of his era. This work can also be considered as an evidence for the origins of western medicine and surgery in the east.
