We evaluated the therapeutic efficacy of antifibrotics in CPFE patients.</p><p><strong>Methods: </strong>This multicentre study used the Korean IPF Cohort registry, identifying 966 CPFE patients. A 6-month landmark window controlled for immortal time bias. Propensity score matching (PSM) balanced baseline characteristics for age, sex, body mass index (BMI), lung cancer, and Composite Physiologic Index (CPI). Post-matching survival outcomes (N=584) were evaluated via 100-month truncated Restricted Mean Survival Time (RMST) to account for proportional hazards violation. Annual longitudinal lung function trajectories (n=310) were estimated via piecewise linear mixed models (LMM) handling missing data as missing at random.</p><p><strong>Results: </strong>In the matched cohort, conventional Cox analysis exhibited a marginally significant risk in the treatment arm (HR=1.29; 95% CI: 1.01-1.65; p=0.045); however, a distinct trajectory cross-over occurred at ∼100 months. Truncated RMST confirmed that the net survival difference was statistically equivalent (-6.87 months; 95% CI: -14.64 to 0.90; p=0.264). Participants underwent a standardized clinical evaluation, completed sleep questionnaires, performed a home sleep apnea test, and wore a wrist activity monitor for one week. The primary variables used to characterize the circadian rest-activity rhythm were intradaily variability (IV), interdaily stability (IS), relative amplitude (RA), and the circadian function index (CFI).</p><p><strong>Results: </strong>The cohort comprised 1828 participants (56.1% female), with a mean (SD) age of 49.1 (8.2) years. After adjustment for potential confounders, OSA showed no significant associations with any circadian-related variables. However, stratified analyses revealed that higher apnea-hypopnea index (AHI), treated as a continuous variable, was associated with decreased CFI exclusively in females and with decreased daytime activity in the oldest age tertile (-6.41 [-10.64 to -2.17]). Participants were followed through 2023. Incident TB was defined as disease diagnosed at least 30 days after initiation of treatment in the index case. All cases diagnosed among household contacts younger than 5 years were also considered incident cases. Cox proportional hazards models were used to identify risk factors for incident TB.</p><p><strong>Results: </strong>During a median follow-up of 3.6 years, 69 contacts (4.3%) developed TB, corresponding to an incidence rate of 1370 cases per 100000 person-years. Children younger than 5 years had the highest risk (adjusted HR [aHR], 25.0; 95%CI, 11-59). An increased risk was also observed among household contacts (aHR, 3.2; 95%CI, 1.6-6.5), smokers (aHR, 2.8; 95%CI, 1.7-4.6), and contacts who did not initiate treatment for TB infection (TBI) (aHR, 7.0; 95%CI, 3.3-14.6). The etiologic fraction of risk was 81.4%.
