{"title":"Residual Excessive Daytime Sleepiness in Obstructive Sleep Apnoea and New Pharmacological Possibilities for Treatment.","authors":"Grace Oscullo, Amina Bekki, Jose Daniel Gómez-Olivas, Miguel Ángel Martínez-García","doi":"10.1016/j.arbres.2025.07.024","DOIUrl":"https://doi.org/10.1016/j.arbres.2025.07.024","url":null,"abstract":"<p><p>Excessive daytime sleepiness is one of the most important symptoms in obstructive sleep apnoea and it is often used to decide whether the latter should be diagnosed as a syndrome or considered a priority in diagnosis and treatment, and whether the treatment is effective. Beyond this, one concept of enormous clinical significance has emerged: \"residual excessive daytime sleepiness\", defined as excessive daytime sleepiness in the context of a diagnosed obstructive sleep apnea that is treatable with continuous positive airway pressure (CPAP) or alternatives to it; it appears as a consequence of poor adaptation to, or refusal of treatment, or even sometimes in situations of good tolerance. Given the direct relationship between excessive daytime sleepiness (usually defined by an Epworth Sleepiness Scale [ESS] value of more than 10 points) and cardiovascular, neurocognitive and quality-of-life disorders, attempts have been made for years to alleviate it with drugs (modafinil, armodafinil, sodium oxybate or amphetamines), to little effect and with a large number of adverse effects. However, unlike their predecessors, two products that have recently appeared on the market - solriamfetol and pitolisant - have achieved clinically significant reductions without any major adverse effects, in most cases. In fact, a Task Force from the European Respiratory Society has even proposed an algorithm for residual excessive daytime sleepiness and its pharmacological treatment in the context of obstructive sleep apnoea. Linear regression and fractional polynomial modelling were used to develop the equations. Second, the developed equations were applied to 715 COPD patients from Spanish primary care centres and hospitals, and the % predicted 6MWD obtained was compared with previously published equations using Dunnett's multiple comparisons test.</div></div><div><h3>Results</h3><div>568 healthy subjects were included (51% females, mean (SD) age 62 (11) years), walked a 6MWD of 615 (113) and 557 (93)<!--> <!-->m in males and females, respectively. The developed equations included age, weight and height, and explained 43% and 51% of the 6MWD variance for males and females, respectively. In the COPD sample (<em>n</em> <!-->=<!--> <!-->715, 14% females, 68 (9) years, FEV<sub>1</sub> 61 (18) % predicted, 6MWD 464 (97)<!--> <!-->m), only 1 out of 9 previously published equations for males, and 6 out of 9 for females predicted 6MWD values similar to those of the newly developed Spanish reference equations.</div></div><div><h3>Conclusions</h3><div>The newly developed reference equations provide a more valid prediction of 6MWD in Spanish adults with COPD compared to previously published equations. However, only the baseline BEC has been used to assess this relationship and it is known that BEC could change over time. The objective of this study is to analyse the association of persistent eosinophilia or eosinopenia with outcomes in bronchiectasis.</div></div><div><h3>Methods</h3><div>Multicentre, prospective and observational study from 43 centres in Spain derived from the Spanish Bronchiectasis Registry (RIBRON). Asthma and anti-eosinophil treatments were excluded. Patients with at least two yearly BEC measures (including the baseline measure) were included. Persistent eosinophilia (at least 300<!--> <!-->cells/μL) or persistent eosinopenia (less than 100<!--> <!-->cells/μL) were defined as the persistence in the same eosinophil group after three yearly measures (being the baseline the first measure).</div></div><div><h3>Results</h3><div>Five hundred two patients with at least three BEC measures were included; 24.5% and 16.6% presented baseline eosinophilia or eosinopenia, respectively. Of these, 57.7% and 56.6% presented persistent eosinophilia and eosinopenia, respectively. Patients with persistent eosinophilia presented greater severity and a higher number/greater severity of exacerbations than those with non-persistent eosinophilia and those with persistent or non-persistent eosinopenia. Finally, patients with non-persistent eosinopenia presented more severity and a higher number/greater severity of exacerbations than those with non-persistent eosinophilia.</div></div><div><h3>Conclusion</h3><div>When only the baseline BEC was taken into account, patients with eosinopenia presented greater severity than those with eosinophilia. However, patients with persistent eosinophilia presented greater severity than those with persistent eosinopenia.
