A quantitative study.","authors":"M Danilewicz, M Wagrowska-Danilewicz","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>15 renal biopsy specimens from patients with mesangial proliferative glomerulopathy (MPGN), 15 from patients with IgA nephropathy (IgAN), and 15 from patients with mesangiocapillary glomerulopathy type I (MCGN-I), for whom both light and electron microscopy as well as immunofluorescence microscopy and full clinical data were available, were examined and compared with 15 normal controls. Morphometric investigations were performed by means of a computer image analysis system to evaluate quantitatively the glomerular deposition of PAS positive material in these glomerulopathies, as well as to assay whether this parameter could correlate with the renal function. The morphometric study demonstrated that the mean values of glomerular deposition of PAS positive material (% of total glomerular area) were similar in MPGN, IgAN, and MCGN-I. Although correlations between glomerular deposition of PAS positive material and serum creatinine tended to be positive in MPGN, IgAN and MCGN-I, they were weak and not significant. {"title":"Histiocytosis of regional lymph nodes associated with hip replacement.","authors":"D Dudorkinová, C Povýsil, R Tomanová","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Five cases of pronounced histiocytic reaction in pelvic lymph nodes after hip replacement are demonstrated. Two patients subsequently underwent radical prostatectomies with bilateral lymph node dissections for adenocarcinoma. In three patients, the change was found during autopsy. The sinuses and interfollicular spaces were distended by numerous large macrophages that had bulky eosinophilic cytoplasm. The cells displayed immunoreactivity to KP1 antigen, alpha-1-antitrypsin and lysozyme, providing support for their histiocytic derivation. Polarization microscopy revealed birefringent needle-like particles in their cytoplasm. We think that the histologic appearance of lymph nodes represents a foreign body reaction to fragments of polyester or polyethylene derived from joint prostheses. {"title":"Mammary hamartoma with brown adipose tissue.","authors":"M F Garijo, B Torío, J F Val-Bernal","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>This report documents a rare variant of mammary hamartoma with presence of lobules of brown adipose tissue. The lesion was discovered in a control mammogram performed in a 42-year-old woman 15 months after hormonal substitute therapy for radical hysterectomy with bilateral salpingo-oophorectomy. Complete excision is the adequate therapy. Both Proliferating Cell Nuclear Antigen (PCNA) and silver binding Argyrophilic Nucleolar Organizer Regions (AgNORs) have been established as a valuable reflection of the tissue proliferative compartment and hence could be of value in studying the biologic behavior of malignant cells. In the present study, we evaluated the proliferative activity of 120 infiltrating duct carcinomas by analysing for PCNA and AgNOR expression in relation to pathologic variables. The AgNOR scores were significantly greater in the malignant tissue than in benign lesions. Expression of PCNA was also found to be increased in malignant breast tumors, when compared to controls. A strong positive correlation between PCNA immunoreactivity and AgNORs was evident. There was also a positive correlation between histologic grading and AgNOR score in breast cancer (r = 0.92, p = 0.000). Tumor with elevated AgNOR counts were often poorly differentiated. {"title":"Bellini duct carcinoma of the kidney--a case report.","authors":"H U Kasper, P Buhtz, G Krüger, D Samland, A Roessner","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Bellini duct carcinoma (BDC), originating from the principal cells of the collecting duct of the kidney, accounts for only 1% of renal carcinomas. This lesion is a histogenetically, morphologically and cytogenetically defined entity. We report an eosinophilic variant of BDC. A 71-year old male presented with a tumor in the right kidney. Nephrectomy was performed. BDC (stage pT3a N0 M0 G2, based on UICC classification) was diagnosed. Electron microscopy revealed an abundance of mitochondria. Intranuclear rodlets were seen. Neuroendocrine differentiation was not demonstrated. The presence of a mixture of smooth muscle cells, blood vessels and adipose tissue, haphazardly arranged within a fibrous background, extending from the hilar region to the lymph node cortex, provides morphologic evidence of the hamartomatous nature of the lesion. CHM and PHM usually present little diagnostic difficulty. However, some may be extremely difficult to distinguish by morphologic features alone. Therefore, we investigated the diagnostic value of silver-stained nucleolar organizer region (AgNOR)-associated proteins in cytotrophblasts as compared to cytogenetic features of nine CHM, nine PHM and six non-molar spontaneous embryonic abortions (controls), as well as of two suspected CHM and two histologically suspected PHM. Tissue sections were submitted to autoclave pretreatment and to silver colloid solution. The proliferating potential of cytotrophoblasts was determined by the analysis of mean number and mean area of AgNORs per nucleus using a PC-based image analysis system. Mean values of AgNOR parameters were significantly different from each other (p < 0.001). Each of the four cases of tentative diagnosis could be assigned to the corresponding group of examined trophoblastic lesions. The persistent cases were histopathologically classified as 5 complete hydatidiform moles, 3 invasive moles and 1 choriocarcinoma. In addition, we determined the DNA content and the karyotype of the sex chromosomes. The data were correlated with the histopathologic characteristics of gestational trophoblastic diseases. Cases with c-erbB-2 amplification and expression in combination with DNA hyperploidy showed higher proliferation and a more aggressive behavior (2 complete hydatidiform moles with lung and liver metastases, 2 invasive moles and 1 choriocarcinoma). {"title":"Postpartal gestational choriocarcinoma fatally misdiagnosed as squamous cell cancer of the uterine cervix.","authors":"L C Horn, K Bilek, H Nenning","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Gestational choriocarcinoma (CCA) is a well-defined tumor, but there may be a surprising variation in its morphologic appearance. A 33-year-old woman with term-pregnancy six months before presented with dysfunctional bleedings that lasted about five weeks. Cervical punch biopsy and currettage revealed a polymorphic tumor which was initially diagnosed as poorly differentiated squamous cell cancer of the cervix. Methotrexat monochemotherapy was performed after histology definitely revealed CCA on the radical abdominal hysterectomy-specimen (Wertheim-Meigs). Immediately after HE, the patient developed diffuse pulmonary metastases and died of respiratory insufficiency after two courses of MTX-therapy. The autopsy confirmed metastatic CCA. The second patient, a 48-year-old women with pregnancy 24 years before, was initially treated with radical HE after misdiagnosis of cervical currettage as squamous cell cancer of the uterine cervix. The patient showed complete remission after two courses of medium risk-protocol and seven courses of high risk-protocol (CHAMOCA). All cervical curettage specimens which did not show typical squamous cell cancer. even in older women, were suspicious of CCA. CCA often shows degenerative changes or predominantly intermediate or cytotrophoblastic cellular elements.
