Questions concerning an increased cancer risk to these women have been raised. A review of the medical literature, including case reports, case series, physician surveys, case-control studies, and cohort studies, failed to turn up any evidence which associated the use of silicone breast implants with either an increased risk of breast cancer or an increased risk of a more advanced stage of cancer at the time of cancer diagnosis. {"title":"Menstrual characteristics of women suffering from insulin-dependent diabetes mellitus.","authors":"A Yeshaya,&nbsp;R Orvieto,&nbsp;D Dicker,&nbsp;M Karp,&nbsp;Z Ben-Rafael","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To assess the frequency of menarche and menstrual disturbances among women suffering from insulin-dependent diabetes mellitus, and to evaluate their correlation to the onset of diabetes, its duration, and complications.</p><p><strong>Research design and methods: </strong>Retrospective analysis of the menstrual regularity and gynecologic and fertility history of 100 women suffering from insulin-dependent diabetes mellitus. These data were compared with information obtained from the patients' medical records regarding the age at onset of diabetes, its management, control, and complications.</p><p><strong>Results: </strong>The average age at menarche was 13.5 years. Delayed menarche tended to appear in girls whose diabetes was diagnosed before the age of menarche, and before the age of 10 years. Menstrual disturbances, such as oligomenorrhea, amenorrhea, and polymenorrhea, occurred in 32% of the women. A significant correlation was found between late menarche and menstrual disturbances. In addition, a higher percentage of morphologically superior embryos was observed in the co-culture group than in the control group, although no significant difference was found between the two groups in blastocyst development or clinical outcome.</p><p><strong>Conclusions: </strong>When taking final clinical results into consideration in a routine IVF program, the conventional method provides more practicality and efficiency if transfers are performed on either day 2 or 3. {"title":"Hemostatic effects of two oral contraceptives containing low doses of ethinyl estradiol and either gestodene or norgestimate: an open, randomized, parallel-group study.","authors":"U H Winkler,&nbsp;C Oberhoff,&nbsp;U Bier,&nbsp;A E Schindler,&nbsp;D Gillain","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To determine effects on blood clotting of two modern low-dose monophasic oral contraceptives.</p><p><strong>Subjects and methods: </strong>We measured in vivo markers of intravascular coagulatory and fibrinolytic activity in 40 volunteers randomly assigned to one of two low-dose oral contraceptives (OCs) for 6 months; one contained 35 micrograms of ethinyl estradiol (EE) plus 250 micrograms of norgestimate and the other, 30 micrograms of EE plus 75 micrograms of gestodene.</p><p><strong>Results: </strong>Both formulations increased coagulatory as well as fibrinolytic activity over baseline: circulating reactive products of thrombin increased by 40%, and plasmin activity by 60%, after 3 months of treatment. Six months of OC use increased hemostatic activity substantially over that with 3 months of use. Differences between both OC formulations were marginal and clinically insignificant.</p><p><strong>Conclusion: </strong>The data suggest an EE-dose-dependent, balanced activation of in vivo coagulation and fibrinolysis in users of currently available, combined OCs. {"title":"Nonbacterial pyospermia: a consequence of clomiphene citrate therapy.","authors":"G J Matthews,&nbsp;M Goldstein,&nbsp;J M Henry,&nbsp;P N Schlegel","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>Since the development of nonbacterial pyospermia in previously nonpyospermic men treated with clomiphene citrate (CC) has been observed, and nonbacterial prostatitis has been after antiestrogen treatment in an animal model, we sought characterize the occurrence of nonbacterial pyospermia in men treated with CC.</p><p><strong>Patients and methods: </strong>Forty-two nonpyospermic men with low serum testosterone levels treated with 25 mg CC/day were retrospectively compared to 27 untreated nonpyospermic men referred for infertility evaluation.</p><p><strong>Results: </strong>Spontaneous nonbacterial pyospermia developed in CC-treated men [14.3%] at rate nearly twice that observed in controls [7.4%]. Serum testosterone increased in CC-treated men, both pyospermic and nonpyospermic. However, only CC-treated, nonpyospermic men demonstrated improvement in semen characteristics. CC-treated men who developed pyospermia were older than nonpyospermic men [pyospermic, 41.7 +/- 8.1 years; nonpyospermic, 35.6 +/- 4.9 years-P < .01). Men over 35 years of age were over six times as likely to develop pyospermia as men under 35 years of age (P < .05). Eight nonpyospermic, CC-treated men (8/36, 22.2%) have contributed to pregnancies leading to live births, whereas no pyospermic man has done so.</p><p><strong>Conclusion: </strong>These findings support an association between a nonbacterial inflammatory response of the human male reproductive tract and CC treatment. This pyospermia may occur without significant deterioration of semen characteristics and with an appropriate response to treatment in terms of serum testosterone level. Men over the age of 35 are statistically more likely to develop pyospermia with this therapy. The acrosome loss, either occurring spontaneously or induced by 10-microM A23181, were examined by staining sperm with Pisum sativum agglutinin labeled with fluorescein isothiocyanate.</p><p><strong>Results: </strong>The incidence of the spontaneous acrosome reaction of spermatozoa incubated with anti-sperm antibodies (6.25 +/- 1.4%) was significantly (P < .001) lower than that of the spermatozoa incubated with control sera (12 +/- 0.8%). {"title":"Conservative management options for cervical pregnancy; case reports and literature review.","authors":"M D Creinin,&nbsp;V A Feldstein","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To describe three cases of cervical pregnancy treated successfully with different conservative methods and review the literature on conservative treatment of cervical pregnancy over the last 30 years.</p><p><strong>Methods: </strong>Three patients treated conservatively for cervical pregnancy are reported. The literature regarding the conservative management of cervical pregnancy since 1965 was compiled using a MEDLINE search for articles published since 1980 and by reviewing the reference lists of published manuscripts.</p><p><strong>Results: </strong>Three cases of cervical pregnancy were successfully treated using methotrexate with leucovorin rescue, methotrexate without leucovorin rescue, and curettage after angiographic embolization of the right hypogastric and left uterine arteries. Review of conservatively managed cases of cervical pregnancy since 1965 indicates that medical management with methotrexate and preoperative angiographic embolization of the uterine blood supply, when appropriate, results in successful treatment without the need for transfusion.</p><p><strong>Conclusions: </strong>Clinical suspicion of cervical pregnancy early in gestation with preoperative diagnosis may safely allow conservative treatment. In view of this, we studied the effect of short-term estradiol treatment, as used in preparing the uterus for embryo implantation, on the serum concentrations of IGFs and their binding proteins (IGFBP) in patients with premature ovarian failure (POF).</p><p><strong>Patients and methods: </strong>Twenty-four patients with POF, enrolled in an assisted reproduction program, were treated with increasing doses of estradiol up to 8 mg daily for 6 weeks. Blood was sampled for measurement of serum estradiol, IGF-I, IGF-II, and IGFBP 1, 2 and 3 at various times during estradiol treatment.</p><p><strong>Results: </strong>There was no significant correlation between serum estradiol concentrations and the serum concentrations of IGF-I and IGF-II. {"title":"A system for simultaneous bilateral tubal cannulation.","authors":"E Confino","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>The efficacy of a new system for simultaneous bilateral tubal cannulation under tactile impression was evaluated.</p><p><strong>Methods: </strong>Two radiologically detectable selective salpingography catheters (Bard Gynecology and Urology, Covington, GA) were glued back to back in a \"T\" configuration similar to an intrauterine device. A sliding plastic sleeve contained the straightened catheters and the uterus was cannulated under tactile impression. The sleeve was then withdrawn and the catheters opened within the uterine cavity and wedged into the right and left tubal ostia simultaneously. Selective injection of water-soluble radiologic contrast material into the tubes resulted in selective salpingography and determined successful tubal cannulation. Withdrawal of the catheters back into the sleeve allowed injection of contrast material into the uterine cavity.</p><p><strong>Patients: </strong>The system was evaluated in 14 consenting patients scheduled for hysterosalpingogram during infertility evaluation.</p><p><strong>Results: </strong>Bilateral selective salpingography was achieved successfully under tactile impression in 12/14 (86%) patients. In two patients with either distorted or very small uterine cavity, the catheter tips embedded into the lateral uterine walls and did not selectively cannulate the tubal ostia. Reapplication of the system under fluoroscopy allowed bilateral selective salpingography of these two patients.</p><p><strong>Conclusion: </strong>Simultaneous bilateral selective salpingography eliminated the need for cervical cannula, significantly reduced fluoroscopy time to seconds, and demonstrated that tactile cannulation is successful in normal uteri. Application of this system for simultaneous tubal cannulation is identical to intratubal uterine device insertion, a procedure familiar to all gynecologists.
