The essential fatty acids linoleic and dihommogammalinolenic acid were administered as evening primrose oil capsules (Efamol) for one week prior to the study. Vascular sensitivity was then determined in response to 4, 8 and 16 ng kg-1 min-1 angiotensin II. Vitamin supplements (Efavit) were given with the Efamol capsules. Seven women have been studied, and their vascular sensitivity compared with controls on normal diet. Blood samples were obtained in the lateral and supine positions. The mean supine NE levels were not significantly higher in those patients destined to develop pregnancy-induced hypertension (PIH) than in those who remained normotensive. {"title":"Altered ferrokinetics in toxemia of pregnancy: a possible indicator of decreased red cell survival.","authors":"S S Entman, L D Richardson, A P Killam","doi":"10.3109/10641958309023469","DOIUrl":"https://doi.org/10.3109/10641958309023469","url":null,"abstract":"<p><p>Serum iron concentration was increased in women with toxemia of pregnancy (mean 135 mcg/dl) compared to normotensive parturients (62 mcg/dl) and chronic hypertensive parturients (72 mcg/dl). Mean iron for eclamptics was 203 mcg/dl; for severe preeclamptics, 137 mcg/dl, independent of hepatic or renal function. Recovery to normal postpartum levels occurred in 1-3 days. Concomitant increase in serum ferritin (mean: 59 ng/ml vs. 19 ng/ml for normals) persisted longer. When hepatocellular injury occurred, mean ferritin increased to 421 ng/ml. Increased iron was independent of transferrin concentration. Serum hemoglobin was detectable in 15 of 25 toxemic patients who had elevated serum iron; only 3 of these patients had clinically significant levels. Serum haptoglobin was significantly decreased in 4 of 25 patients. Reticulocyte count averaged 2.5%, with an average hematocrit of 38%. It is hypothesized that serum iron and ferritin increase in response to catabolism of released hemoglobin. There was one maternal mortality and two patients who developed pulmonary edema. The corrected perinatal mortality rate was 143/100. Epidural anesthesia did not adversely affect fetal outcome. Perinatal morbidity and mortality was mainly related to the development of neonatal respiratory distress syndrome (RDS). Twenty-three neonates developed respiratory distress syndrome (RDS). RDS was strongly associated (P less than .001) with gestational age. In this study the effect osmolar and water loading on the renin-angiotensin system and arginine vasopressin (AVP) secretion has been tested in seven women during the third trimester and again 8-10 weeks after delivery. Pregnant women had markedly increased plasma renin substrate (PRS), plasma renin activity (PRA) and plasma renin concentration (PRC) values as well as aldosterone levels when compared to their post-partum values. Osmolar loading with intravenous infusion of hypertonic saline resulted in a decrease in PRA and aldosterone levels both during and after pregnancy but even at the end of the infusion the pregnant women still had values greater than the pre-infusion levels obtained post-partum. Surprisingly, oral water loading also significantly decreased PRA and aldosterone levels in pregnancy, possibly related to the redistribution of extracellular fluid centrally when the pregnant women were in left lateral recumbency. Despite the decreased basal Posm of pregnancy, urinary AVP increased and decreased appropriately during osmolar and water loading. Exact characterisation of the resetting of the threshold for AVP secretion in pregnancy awaits the development of a reliable radioimmunoassay for the determination of AVP in human pregnancy plasma.</p>","PeriodicalId":79209,"journal":{"name":"Clinical and experimental hypertension. {"title":"Pregnancy-associated hypertension: interrelationships of volume and blood pressure changes.","authors":"E D Gallery","doi":"10.3109/10641958209037179","DOIUrl":"https://doi.org/10.3109/10641958209037179","url":null,"abstract":"<p><p>In normal human pregnancy there is considerable expansion of both plasma volume and total extracellular fluid volume, together with marked diminution of vascular reactivity in response to intravenous infusion of vasopressor agents. Pregnancy-associated hypertension (pre-eclampsia) is characterised by increased vascular reactivity, vasoconstriction and a fall in plasma volume associated with a shift of fluid from intravascular to interstitial fluid space. Direct expansion of plasma volume in patients with pregnancy-associated hypertension leads to partial correction of the abnormalities of volume homeostasis together with a significant fall in blood pressure. Certain forms of antihypertensive therapy also lead to reversal of volume contraction and improvement in cardiac function. This is associated with lowered vascular resistance and improved placental function, leading to improved pregnancy outcome. Although these observations do not elucidate the ultimate cause/causes of pregnancy-associated hypertension, they help to unravel the pathologic mechanisms by which the syndrome causes its effects and to suggest rational paths by which therapeutic endeavours may be expected to benefit mother and fetus.</p>","PeriodicalId":79209,"journal":{"name":"Clinical and experimental hypertension. To validate this observation in vivo PGE were measured weekly in 24h urine collections, in a prospective cross-sectional study in 9 women from 7 to 40 weeks. In addition, urinary PGE levels were also measured in 28 hospitalized pregnant women with either chronic hypertension or toxemia. Prostaglandins E were measured by radioimmunoassay after organic extraction and silicic acid separation. Urinary PGE levels during pregnancy (normotensive and chronic hypertensive women) were significantly elevated than those of the non-pregnant state. Mean urinary PGE levels in toxemic patients were significantly decreased compared to those of normal pregnancy and patients with chronic hypertension but they were similar to the mean levels of the non-pregnant state. One fetal death attributed to aggravation of chronic hypertension and one eclampsia were associated with undetectable levels of maternal urinary PGE. In conclusion, normal pregnancy is associated with a significant increase of urinary PGE; chronic hypertension and occurrence of toxemia are associated with significant diminution of urinary PGE excretion.</p>","PeriodicalId":79209,"journal":{"name":"Clinical and experimental hypertension. All the patients were ambulatory on a normal salt diet. PRA was significantly higher in the RRH than in the control and PIH groups (15,8 +/- 2,3ng/ml/l versus 6,7 +/- 0,5 and 8,9 +/- 0,9 respectively). Plasma epinephrine (PE) and norepinephrine (PNE) were significantly higher in the PIH than in the control and RRH groups (respectively 135 +/- 28 pg/ml versus 56 +/- 13 and 63 +/- 17 for PE and 387 +/- 91 versus 206 +/- 32 and 200 +/- 47 pg/ml for PNE). In the PIH group there was a negative correlation between PRA and blood uric acid. In order to identify some factors that may be associated with maternal deaths in eclamnsia in this region, nine deaths that occurcd during a qrospective study of 100 eclamptics in 1979 were reviewed. It was discovered that maternal death occured twice more commonly in the under 16 and rarely in mothers over 20 years of age. There was an almost two fold mortality rate among multinarous patients implying that eclampsia occurs in this grouo in the nresence oi some other underlying medical condition. Intrapartum eclamnsia (mortality rate of 11. 1%) was more lethal than antepartum (6.8%) and nost nartum (5.8%) ones. There was no correlation between degree of hypertension or number of fits and maternal mortality, though there was slight rise when the diastolic pressure was higher than llOmmHg at the time of fits. PGE1 15 ng kg−1 min−1 had no effect on basal blood pressures in the pregnant patients, but increased heart rate by 9.1 ± 1.6 bpm (P<0.001). Six of these eight wanen had scanewhat diminished pressor responses to AII (4, 8 and 16 ng kg−1 min−1) during PGE1 infusion; this difference reached statistical significance at the lowest dose of AII (P<0.05). PGE1 10 ng kg−1 min−1 slightly increased basal diastolic pressure in the non-pregnant subjects (P<0.05) and increased heart rate by a similar amount (8.4 ± 1.8 bpm, P<0.01). Again, the overall pressor response to AII was sanewhat diminished in the majority of these subjects.
