Results of the measurements from 32 pre-eclamptic pregnancies and 37 normal controls are compared. The uteroplacental blood flow was measured as an index calculated from the rise time and maximum activity of the isotope accumulation curve. The uteroplacental blood flow was reduced with 50% in pre-eclampsia. In severe pre-eclampsia it was more compromised than in mild pre-eclampsia. A diminished uteroplacental blood flow was found in pre-eclampsia even in the absence of intrauterine growth retardation. The maternal placental circulation in the supine position was reduced with one third compared to that in the left lateral recumbent position.</p>","PeriodicalId":79209,"journal":{"name":"Clinical and experimental hypertension. The effects on the mother and the fetus are compared with those of 97 hypertensive gravidae treated with a combination of hydralazine and a thiazide. The combination of metoprolol and hydralazine seems to be the most favourable one judged by both maternal well-being, fetal intrauterine growth, ten-minute Apgar score and perinatal mortality. At birth the concentration of metoprolol shows a ratio of 1:1 between maternal plasma and umbilical plasma and furthermore the ratio between maternal plasma and breast milk is 1:4. The newborns of mothers on beta-blocking therapy did not differ from those of mothers on hydralazine regarding heart rate, plasma glucose or plasma bilirubin. Both normal and hypertensive gravidae in the third trimester at bed rest and those treated for hypertension have low COP. Likewise, postpartum women may have low serum COP values apparently unrelated to any disease process. Amniotic fluid COP is low in both normal and hypertensive gravidae. COP Measurements therefore, seemingly offers little information regarding the health of pregnant women. Serum COP may increase in some hypertensive gravidae receiving saline infusion, probably reflecting abnormal renal function and hypovolemia. High serum triglyceride values coexisting with low COP may represent hepatic efforts to produce other colloid osmotic-active substances in the semi-chronic situation, although our findings in the acute situation did not support this hypothesis.</p>","PeriodicalId":79209,"journal":{"name":"Clinical and experimental hypertension. These alterations in vascular responsiveness have not been thoroughly investigated and are poorly understood. Suitable models manifesting physiologic conditions known to exist in human pregnancy have not been satisfactorily demonstrated. We investigated the chronically catheterized and unmedicated rabbit for similarities of four conditions known to occur during human pregnancy. We compared mean arterial pressure and vascular response to angiotensin II and to norepinephrine in 19 non-pregnant and 16 pregnant animals. Plasma levels of angiotensin II, norepinephrine, epinephrine, and dopamine levels were measured and compared. We found striking similarities for conditions investigated in our rabbit model when compared with data reported in the literature for human pregnancy. In this study, we analyzed the clinical background of 70(1.5%) cases of severe preeclampsia in 4,633 deliveries in our clinic. The main family history was hypertension (34.2%). The medical complication of this pregnancy were nephritis (11.4%), hypertension (5.7%) and diabetes mellitus (4.3%). In previous obstetrical complication, preeclampsia was found very high frequency (44.4%). In this pregnancy, the cesarean section was done for 15 (21.4%) cases. There was a significant high frequency of low birth weight infant in preeclampsia (2,400 +/- 925 gr, mean +/- S.D.). The stillbirth was found in 12 (17.1%) cases. The perinatal mortality rate was 169/1,000 deliveries, this was a significantly higher than total rate (15/1,000 deliveries). These data suggest that it is important to control medical complications before to be pregnant to prevent preeclampsia.</p>","PeriodicalId":79209,"journal":{"name":"Clinical and experimental hypertension. Daily blood samples were assayed for progesterone, dihydroprogesterone, estriol, estradiol, placental lactogen and prolactin. Under progesterone therapy there was a significant decrease in both systolic and diastolic blood pressure, significant increase in urinary output, amelioration of the edema, slight reduction in weight gain, but no change in the proteinuria. Clinically normal patients had peripheral venous blood sampled from the lateral and recumbent positions monthly in the morning during their last two trimesters in labor and six weeks postpartum. One-third of the patients had 24 hour urine collections. Homologous double antibody radioimmunoassays were performed to determine prolactin levels. The data were analyzed according to pregnancy outcome: pregnancy-induced hypertension or normotensive throughout pregnancy. Acute positional change did not influence prolactin level. Prolactin levels were significantly elevated in the hypertensive outcome group only at 37-39 weeks and were not correlated with sodium excretion. We conclude that circulating prolactin does not play a significant role in pathogenesis of pregnancy-induced hypertension, but perhaps the elevated levels may be reflecting pathophysiologic changes.</p>","PeriodicalId":79209,"journal":{"name":"Clinical and experimental hypertension.
