Its appropriate use requires a solid understanding of respiratory physiology, lung mechanics, and patient-ventilator interaction.</div><div>This article reviews the basic concepts of mechanical ventilation, including respiratory system anatomy and physiology, as well as key mechanical properties (compliance, resistance, and time constant) that determine respiratory dynamics. Ventilatory modes are classified according to control variable, breath sequence, and targeting scheme, facilitating their interpretation and clinical application.</div><div>Initial ventilator settings are described, emphasizing the importance of lung protective strategies. The article also addresses comprehensive monitoring, based on the systematic assessment of the patient, the ventilator, and the clinical response. Enteral nutrition supports the gut barrier, but its effect on intestinal permeability in pediatric AP has not yet been evaluated. The aim of this study was to assess how very early enteral nutrition (within 24hours of admission) and early enteral nutrition (after the first 24hours) affect plasma zonulin levels in children with mild-to-moderate AP.</div></div><div><h3>Methods</h3><div>We conducted a prospective randomized study between January 2019 and September 2022 in three university hospitals. It included children aged 1 to 18 years with AP. Patients were randomly assigned to one of two groups: (A) refeeding within 24hours of admission (very early nutrition) or (B) refeeding at least 24hours after admission (early nutrition). Blood samples and clinical data were collected during the first 3 days of the hospital stay.</div></div><div><h3>Results</h3><div>Zonulin concentrations were measured in 61 children (37 in group A and 24 in group B). The groups did not differ significantly in age, weight, height, body mass index, AP diagnostics, or laboratory results. However, there is limited scientific evidence supporting specific pedagogical strategies for this purpose. This study evaluated the impact of interactive teaching resources compared with traditional instruction on the acquisition and retention of skills related to activating the chain of survival in schoolchildren aged 5 to 8 years.</div></div><div><h3>Methods</h3><div>We conducted a quasi-experimental study was conducted in five schools. A total of 298 schoolchildren were included: 111 in the control group (traditional instruction) and 187 in the intervention group (learning with a “never-ending book” and a telephone simulator). Assessments were carried out immediately after training (E0) and at 12 months (E1) using a simulation scenario and a checklist.</div></div><div><h3>Results</h3><div>In the initial assessment (E0), the intervention group achieved superior results in four key variables. At the 12-month evaluation (E1), the intervention group maintained significant advantages in competencies related to activating emergency medical services and effective communication with the dispatcher. The ability to use the phone in hands-free mode declined over time in both groups, while the ability to recall a parental phone number improved.</div></div><div><h3>Conclusions</h3><div>Simple hands-on and interactive resources can improve both the acquisition and retention of basic skills required to activate the chain of survival. They share common features, including delayed or absent diagnosis; being severe, chronic, and disabling; lack of effective treatments; and insufficient knowledge or awareness among health care professionals and society at large. Their management frequently requires specific treatment with orphan drugs. However, the development of pediatric orphan drugs and their accessibility continue to be limited. The aim of this study was to describe the situation of pediatric orphan drugs in Spain as of December 31, 2025.</div></div><div><h3>Methods</h3><div>Cross-sectional descriptive study based on the review of the scientific literature and regulatory databases between 2013 and 2025. We identified all the orphan drugs included in the CIMA database of the Spanish Agency of Medicines and Medical Devices (AEMPS) and reviewed their summaries of product characteristics to determine their pediatric indications. We analyzed variables related to the authorization, commercialization, funding, and clinical characteristics of the drugs.</div></div><div><h3>Results</h3><div>We identified a total of 67 orphan drugs with pediatric indications authorized in Spain, which amounted to 50% of all orphan drugs. Of these, 22.4% were only indicated for pediatric use. Forty-nine (73.1%) were marketed and 46 were funded by the Spanish national health system, with restrictions in 63% of the funded cases.</div></div><div><h3>Conclusions</h3><div>Orphan drugs with pediatric indications remain a minority among orphan drugs. Pediatric clinical research needs to be reinforced, and strategies implemented to ensure more agile, equitable, and coordinated access.
