Use of cardiovascular drugs was studied with respect to optimal results of therapy in 361 patients with main cardiac pathology (ischemic heart disease, essential hypertension and their combination) who had received 1125 courses of drug treatment. The prescriptions were analysed according to their intensity, regularity and optimal effect. {"title":"[Various immunological aspects of essential and symptomatic hypertension].","authors":"I K Shkhvatsabaia, V I Rudnev, Iu I Suvorov, G Iu Domba, S G Osipov","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>131 patients with essential hypertension (EH) and 30 patients with secondary hypertension (SH) of renal genesis were examined, all of them Russian inhabitants of Moscow, aged 20-56. In patients with EH increased rate of HLA-B13 and B22 antigens was determined. The highest rate of HLA-B13 antigen in this group was registered in patients without IHD, while patients with IHD had the highest rate of HLA-B22 antigen compared to controls. Patients with SH demonstrated no significant difference in HLA antigens distribution from that in controls. Besides, patients with EH had significantly increased serum concentration of circulating immune complexes (CIC), of IgA and beta 2-microglobulins as well as of three complement components (C3c, C4 and B factor). Similar changes were observed in patients with SH, excluding CIC and C3c, concentration of which did not differ from that in the control group. No strict dependence between the level of immunity humoral factors and presence of HLA-B13 and -B22 antigens was observed. {"title":"[Use of tricyclic antidepressants in patients with chronic ischemic heart disease].","authors":"V P Zaĭtsev, T I Shubina","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The results of a course of therapy in 187 men with proved ischemic heart disease (IHD) showed that tricyclic antidepressants in minimal and small doses may be effectively used in patients with IHD for treatment of psychic alterations as well as in chest pain syndrome. Amitriptyline was more effective than azafen or melipramine but significantly more often induced side effects. {"title":"[Determining the linear dimensions of the heart in healthy persons by the method of magnetic resonance tomography].","authors":"V E Sinitsyn, L M Sergakova","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Linear dimensions of hearts in 16 healthy volunteers obtained by magnetic resonance tomography (MRT) and 2-dimensional echocardiography (EchoKG) were compared. MRT was performed with BMT 1100 tomograph (\"Bruker\", FRG) with magnetic field strength of 0.235 T using EKG-triggering. Oblique slices through anatomic heart axes were registered. Echocardiography demonstrated significantly more manifest hypertrophy of interventricular septum and higher coefficient of asymmetry in patients with HCMP compared to those with EH while left ventricular myocardium mass was equal in both groups. Phase-volumetric analysis exhibited in all the patients deterioration of myocardial relaxation processes. EKG-monitoring showed a significant increase in frequency of supraventricular rhythm disturbances in patients with HCMP while no reliable differences in total frequency of rhythm and conduction disturbances were registered. Regular adequate antihypertensive therapy resulted in patients with EH in decrease of ASH degree caused by regression of interventricular septum hypertrophy. In natural course of EH the ASH degree also decreased, but in this case because of increased hypertrophy of left ventricular posterior wall. The patients with HCMP were followed for 1-1.5 years. Selection of the parameters of the cartogram of diagnostic significance for the differential diagnosis of combined myocardial hypertrophy and its isolated forms].","authors":"A V Sobolev, G V Riabykina, Z Z Dorofeeva, T S Markovskaia","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A new approach to the choice of diagnostic criteria of combined ventricular hypertrophy (CVN) is proposed: after determination of EKG parametres differing from the norm are separately compared to the EKG parametres in isolated left ventricular hypertrophy (LVH) and right ventricular hypertrophy (RVH). The diagnostic parametres were chosen on the basis of computer statistical analysis of EKG-35 data of 136 patients with CVH, 39 with LVH, 57 with RVH and 71 normal subjects. For differentiation from LVH the amplitude/area ratio (S10 + S17 + S24)/(S1 + S8 + S15) and R27/R17 was effectively used, for differentiation from RVH parametres R14/R1 or S25/S27 are recommended. The amplitude parameters provide the sensitivity of the method of 58.8% with the specificity of 94.6%; the use of area ratio of the same waves increases sensitivity up to 72.8% with the specificity of 94.4%. All the examined subjects were asked, how often they used alcohol. Only 31.1% of women and 14.3% of men used no alcohol at all. The highest was the percentage of persons using alcohol several times a year: 42.1% of men and 60% of women. Men using alcohol several times a week proved to have arterial hypertension more often than those not using alcohol. Among men aged 55-64 using alcohol several times a week nobody had normal blood pressure. It was suggested that this effect depended on protein DNIC formation which were an NO-store in animal tissues. NO was hypotensive agent. To test this suggestion low molecular weight DNIC with thiosulphate or phosphate were incubated with blood plasma of dogs and rats which evoked protein DNIC formation proved by EPR method. Injections of these plasma samples into narcotized dogs and rats produced hypotensive responses similar to those produced by low molecular weight DNIC. An acute test with 20 mg nifedipine (N) sublingually was performed and the pulmonary artery pressure (PAP) was measured. N had manifest arterolodilating effect in the systemic and especially pulmonary circulation, induced decrease in the total peripheral and total pulmonary vascular resistance (TPerVR and TPulVR) by 21.7 and 25.3%, correspondingly. N had principal effect on pulmonary arterioles (TPerVR/TPulVR ratio increased by 16.9%). Systolic and diastolic PAP decreased in this case by 15.2 and 18.4%, correspondingly.
