{"title":"[Nonspecific bronchial reactivity in chronic bronchitis].","authors":"M Pavlović","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In a segment of the population of Zagreb, Crnomerec, which was included in the epidemiological study on chronic bronchitis in Croatia (1969-1972-1982-1989) the values of ventilatory function and non-specific reactivity of airways were examined. The sample consisted of 34 women and 29 men with chronic bronchitis. The control group consisted of 34 men and 33 women--without cough. The average age was 58.6 (M) and 59.4 (F). Investigation of non-specific reactivity was carried out on the dosimeter apparatus APTA-Jaeger, Wurtzburg, W. Germany (by using histamine dehydrochloride (1-30 mg/ml dose) with expressed reactivity PC 40 Raw. PC 40 in subjects with chronic bronchitis amounted to 7.79 mg/ml in men and 8.89 mg/ml in women, which is significantly lower than the control group (14.04 M, 17.8 F). We did not determine significant difference between smokers and non-smokers with regard to PC 40. By factor analysis in an orthogonal projection a link was noted between symptoms of chronic bronchitis, basic values of ventilatory function and PC 40. FEV1, FEF50, FEF25 and total respiratory resistance (Rrs) measured by Astograph in nonspecific metacholine bronchoprovocation test were analyzed in 378 patients suspected of bronchial asthma. The patients were divided into 7 groups based on Astograph and S.P. instances criteria. Of 92 patients with positive test results, 91% met the set Astograph criterium; 72% met all three S.P. criteria, 1% one criterium and 12% failed to meet any of the S.P. criteria. It was noted that the patients met more the FEF50 and FEF25 criteria (87%) than FEV1 (73%). Also, there was no significant increase in respiratory resistance for those with a predominant reaction in small airways (9%). High levels (above 13.0 micrograms/l) of NSE in pleural fluid of 9 patients with malignant disease were observed (sensitivity 57%). However, pleural fluid NSE levels were also increased in 14 with non-malignant diseases (specificity 45%). {"title":"[Legionnaires' disease in Yugoslavia (an epicritical review of patients)].","authors":"I Petricević, V Presecki, I Kuzman, I Soldo, V Drazenović","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A review of patients with Legionnaires' disease hospitalized in our Clinic from the first cases recorded in 1978 up to now has been given. The diagnosis was confirmed by indirect immunofluorescent method in 23 patients with pneumonia, using antigen and method of the Center for Disease Control Biological Products Division, Atlanta, Georgia, USA (Dr Wilkinson). Most of the patients (16) were sporadic cases and twice as less cases were from two epidemics (7). Two patients among sporadic cases developed a very severe form of the disease during the immunosuppressive therapy after kidney transplantation. Clinical, epidemiological and laboratory data of 10 patients hospitalized and treated during the last three years (1986-1988) were analyzed in details. Two thirds of these patients were above 40 years of age and the youngest one was 29 years old. Except the advanced age no other firm disposable factor was found in patients belonging to sporadic cases. In cases from epidemics the source of infection was probably water from water-pipes i.e. hotel devices (bathrooms, douches, drinking water, air-conditioning units, water used in kitchen for cooking etc). All patients recovered after a longer symptomatic and antibiotic treatment. Erythromycin and rifampicin were applied the most frequently. {"title":"[Primary pulmonary cancer in women the the Doboj Region].","authors":"N Karabegović, I Petricević, M Mijatović, B Durić, D Povazan, N Zafran","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The aim of this work is primary lung cancer in female population of the Doboj district in the period 1975-88. incidence research as well as age structure, localisation, histological types, stage and diseases associated with lung cancer. A group of 114 female patients was investigated and as comparative group male patients with lung cancer in the same period was used. The results show that incidence rate increase is significant and in the future that trend will be continued. Especially serious is often microcellular and adenocarcinoma appearance as well as female non-smoking patients (42%) and patients with diabetes mellitus (18%) in the investigated group high percentage. These phenomena lead to conclusion that risk factors are partly different and they should be better investigated. {"title":"[Comparison of in vitro activity of ciprofloxacin and pefloxacin against strains isolated from clinical material].","authors":"M Kumelj, A Rus, E Music","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The susceptibility of 392 strains of pathogenic bacteria isolated from clinical material of hospitalized patients in Institute for Chest Diseases Golnik were studied. 100 strains of Escherichia coli, 56 strains of Enterobacter spp., 40 strains of Proteus mirabilis, 50 strains of Gram negative nonfermentative bacteria, 46 strains of Pseudomonas aeruginosa and 100 strains of Staphylococcus aureus were tested. Minimal inhibitory concentrations of ciprofloxacin and pefloxacin were determined by agar dilution test. The susceptibility of tested strains is greater to ciprofloxacin, except susceptibility of strains of Staphylococcus aureus, which is greater to pefloxacin. {"title":"[The epidemiologic situation of chronic obstructive lung diseases in the population in Croatia].","authors":"Z Radosević, D Knapić","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In the introduction the importance of epidemiologic studies of COPD is indicated, thus stressing the complexity of their realization and high cost of these projects. According to the data that are gathered through every day routine work of health service and health statistics the authors try to find out the place of COPD in the population of Croatia studying the year 1987. In general medicine 24.8% visits due to respiratory diseases have been registered, in occupational medicine 24.8%, school medicine 43.2%, pediatric health care 60.8%. In this number COPD are represented by 8.6% with 111.822 visits, occupational medicine by 3.8% with 18.445 visits, school medicine by 4.2% with 22.629 visits and pediatric health care by 5.5% with 40.961 visits. On the whole in this year in Croatia there were 4.214 visits on 100,000 population due to COPD. According to the data of pneumology service the incidence of COPD is 164 0/0000 and the prevalence 691,6 0/0000. In pneumology stationaries 6.207 patients with COPD or 21.5% were treated. In the proclaimed invalidity the COPD is to be found on the fourth place. In the mortality the respiratory diseases are found on the sixth place out of which half of them are COPD. By the follow-up of these data a lot of time and money is wasted without giving the adequate results. The workers were divided into two groups: smokers (32) and non-smokers (88) and the values obtained from lung function tests were compared with the normal standards.
