The author reviewed the up-to-date indications of the echographic methods in Pediatrics. The attention is focused on the biologic side-effects of the ultrasounds in children, including the prenatal period. The clinical and paraclinical examinations showed the existence of several compression phenomena at the level of the brain stem. The severe evolution and the death were explained by the discovery, at necropsy, of an astrocytoma, degree I, placed in the brain stem. Very seldom in the pediatric practice, superior respiratory insufficiency of the small child, at this age, is due to the astrocytoma of the brain stem. The paper reviews the etiologic data of the disease in its secondary form, with emphasis on the form of the disease better studied in the last years, i.e. the so-called infantile benign epileptic spasms. {"title":"[The realities and paradoxes in the nephrotic syndrome with minimal glomerular lesions in children].","authors":"I Sabău","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>On the basis of the present knowledge in the nephrotic syndrome with minimum glomerular lesions, the paper reviews the paradoxes of the disease: the glomeruli with normal histologic structure in optic microscopy, but with the highest proteinuria in the whole human pathology; absence of some clear arguments concerning the existence of the immunologic disorders, but the best medication is corticotherapy and immunodepressive drugs; the high incidence of the allergic background against which the nephrotic syndrome appears, but with or without a limited effect of the antiallergic medication. {"title":"[Pneumocystis carinii pneumonia].","authors":"D Dragomir, L Tauberg, V Popescu","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The paper reports on a theme of important pediatric interest, pneumonia with Pneumocystis carinii (PC), a more widespread affection lately, due to the progress in pediatric reanimation, on the one hand (that allowed the survival for a long time of several biologically handicapped infants) and as a consequence of the more and more common use of immunosuppressing therapy, on the other hand (malignant diseases, transplant of organs, etc.); another important role in the increase of the PC pneumonia is played, recently, both in the adult and the child, by the infection with HIV (acquired immunodeficiency syndrome). The authors discuss largely on the pathogenic agent, PC, proved to be a fungus by molecular genetic studies, and on the biological field (small age, dystrophy, prematurity, \"debilitating\" chronic diseases, long diarrheic diseases, congenital malformations, hospitalization, some infections with long evolution: tuberculosis, Cryptococcus, infection with cytomegalic virus). The paper also deals with epidemiology, pathogeny and pathologic anatomy of the disease (characteristic macroscopic aspect: nonaired lung of high consistency, with whitish infiltrations alternating with congestive zones and the microscopic aspect of alveolo-interstitial pneumonia). The clinical picture includes the common form, met in the biologically handicapped infant and hypoergic hypoimmune form, appearing in all ages in immunodeficient subjects. Radiological examination, pulmonary scintigraphy and a series of laboratory data (with evidence of parasitic cysts) complete the clinical examination, making easier the diagnosis. The techniques of seroimmunological diagnosis offers great hope for the future. The positive diagnosis shows the clinical elements that suggest the presence of the disease, the contribution of the radiologic examination and mainly the presence of the parasite in the bronchitic secretions; the therapeutic response is a retroactive element in the positive diagnosis. The differential diagnosis is presented in detail. {"title":"[The incidence of a neonatal bacterial infection syndrome with maternofetal transmission in the Neonatal Department of the Baia Mare District Maternity Hospital in 1983-1987].","authors":"A Pohl, I Radu, M Mesaroş, E Surianu, T Bancoş","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In a retrospective study extending on a 5-year period, authors have analysed the incidence of the bacterial infectious syndrome with maternofetal transmission in the newborns admitted to the above-mentioned department. Among 20,674 live-born infants, 241 cases (1.16%) with diseases of an infectious character with vertical transmission were recorded. The incidence of the neonatal infectious syndrome was marked by a yearly increase, from 0.86% in the first year of the study to 1.24% in the last year, with a peak of 1.43% in 1986. The infection risk by maternofetal transmission was more than 10 times higher in the group of premature children. During the study period, of the total of 128 deaths, 31 (24.22%) were due to the neonatal infectious syndrome; of these 31 deaths, 21 occurred in premature children, hence 2/3 of all the failures were recorded in premature infants.
