Test findings about error type and stimulability often give direction to our decisions about speech therapy. Other kinds of tests are useful for confirming these findings or for other purposes. Trial speech therapy is a very useful diagnostic procedure. Problem patients (those who fail to make suitable progress) should be referred without hesitation to a cleft palate team, or to a colleague who is an SLP on that team. Intervention.","authors":"M A Hardin","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Results of recent investigations (O'Gara and Logemann, 1990; Estrem and Broen, 1989) indicate that the presence of an unrepaired cleft will influence a toddler's early phonologic development and lexical selectivity. Although additional information is needed to characterize the early vocalizations of children with cleft palate, the available findings underscore the need for early, aggressive speech-language intervention. The education and assistance in language stimulation provided to parents during a child's first year of life can minimize the deleterious effects of an unrepaired palate. Treatment for older children and adults with borderline VPI continues to pose management problems for the SLP. {"title":"Cleft palate and velopharyngeal dysfunction.","authors":"J L Marsh","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Surgical and prosthetic interventions are effective means of managing cleft palate and velopharyngeal dysfunction. Most patients are managed surgically because usually there is life-long benefit from that type of intervention and an operation does not require continued cooperation from the patient. In selected cases, prosthetic intervention is preferred. Over the past century, the efficacy of such interventions has increased markedly. Not all of the factors responsible for these improved results are understood. Disagreement among care providers persists regarding the specific criteria for intervention, the use of ancillary tools beyond perceptual speech evaluation for both pre-intervention and postintervention assessments, the technical details of intervention, and the ideal age for intervention. {"title":"A minority perspective in the diagnosis of child language disorders.","authors":"H N Seymour, L Bland","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The effective diagnosis and treatment of persons from diverse minority language backgrounds has become an important issue in the field of speech and language pathology. Yet, many SLPs have had little or no formal training in minority language, there is a paucity of normative data on language acquisition in minority groups, and there are few standardized speech and language tests appropriate for these groups. We described a diagnostic process that addresses these problems. The diagnostic protocol we have proposed for a child from a Black English-speaking background characterizes many of the major issues in treating minority children. {"title":"Parents of children with cleft lip and plate.","authors":"J E Riski","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The preceding literature review and parent interviews are intended to provide novel insight into the stress, anxiety, and fears faced by parents of children born with a cleft lip and palate. Professionals need to be available to assist with the adjustments and transitions that the parents will face and to tailor their interactions to the specific needs of the parents and the child. Several conclusions are apparent. 1. Needs appear to be greatest at times of transition: birth, operation, and school. 2. Feelings of loss and grief may accompany the birth of a child with a cleft. The birth may be marked by the realization that the perfect child is lost and by the grief that this imperfect child must be loved and nurtured. The rearing process is complicated by the parents' own insecurities and can be assisted by positive coping strategies and the support of other parents of children with clefts. 3. Parents may feel uncomfortable discussing aspects of finance and personal fears. The parents interviewed indicated that the fears and concerns expressed here were the very concerns that they do not raise with professionals. 4. Parents are highly variable in their experience and needs. The age of the parents, the extent of the cleft, and familiarity with clefting appear to be major factors in determining how well parents deal with the birth of a child with a cleft. 5.
