By performing only a partial superficial parotidectomy, we probably preserve the connection between the auriculotemporal nerve and the facial nerve. By suturing the remainder of the parotid gland tissue and the superficial musculoaponeurotic system (SMAS) back to the sternocleidomastoid muscle, an interposition is made. Furthermore, by extending the incision not too far cranially, the auriculotemporal nerve cutaneous branch is spared. We also tried to preserve part of the great auricular nerve (GAN).</p><p><strong>Design: </strong>Descriptive study.</p><p><strong>Setting: </strong>One main institute (a tertiary referral centre) and one affiliated secondary centre.</p><p><strong>Methods: </strong>Patients operated on for parotid gland tumours in our hospitals during the last 3 years underwent starch-iodine testing after at least 1 year of follow-up.</p><p><strong>Main outcome measures: </strong>The number of patients developing Frey syndrome. We hypothetically explored the importance of preserving both the auriculotemporal and the great auricular nerve.</p><p><strong>Results: </strong>Eighty-one patients were operated on for a parotid gland tumour. Forty-five (56%) of these patients could be followed up for more than 1 year. Four patients had a positive starch-iodine test. Two of these underwent revision surgery and had symptoms already before our procedure. About half of the patients had a SMAS interposition performed. All four patients with Frey syndrome did not have a SMAS interposition.</p><p><strong>Conclusion: </strong>We advocate our technique in primary benign tumours. The control group consisted of 15 healthy nondiver patients (9 males, 6 females) and 30 ears. For the study group, total diving duration (TDD), total diving score (TDS), duration after the last dive (DALD), and maximum diving depth (MDD) were obtained by questionnaire. All subjects were evaluated by an otolaryngologic examination, pure-tone and high-frequency audiometry, and transient evoked otoacoustic emissions (TEOAEs).</p><p><strong>Results: </strong>There was no significant difference between the hearing thresholds and TEOAEs of the study and control groups. For divers, age, higher TDDs and TDSs, and a higher MDD caused an increase in hearing thresholds and a decrease in TEOAEs. When the DALD increased, cochlear functions again achieved nearly normal levels.</p><p><strong>Conclusion: </strong>We recommend that divers' hearing levels and cochlear functions be evaluated before diving. The subjects completed questionnaires related to the voice disorder and the Symptom Checklist-90-Revision.</p><p><strong>Outcome measures: </strong>The scores of the patient group with less than 1 year of symptom duration (acute) and more than 1 year of symptom duration (chronic) were also compared with those of the controls.</p><p><strong>Results and conclusion: </strong>The total patient group differed statistically from the control group on seven neurotic dimensions (p < .001) and one psychotic dimension (p < .05). The acute group differed on two neurotic dimensions (p < .05). We suggest that the neurotic traits found in the acute group may play a role in the development of vocal nodules. The dimensions in which the total patient group differed significantly from the control group may indicate the changes that occur in the psychological characteristics following voice change. The collective results indicate that psychological characteristics play an important role in the pathogenesis of vocal nodules. {"title":"Accuracy of the Weber and Rinne tuning fork tests in evaluation of children with otitis media with effusion.","authors":"Agnieszka Behn,&nbsp;Brian D Westerberg,&nbsp;Hongbin Zhang,&nbsp;Keith H Riding,&nbsp;Jeffrey P Ludemann,&nbsp;Frederick K Kozak","doi":"10.2310/7070.2007.0025","DOIUrl":"https://doi.org/10.2310/7070.2007.0025","url":null,"abstract":"<p><strong>Objective: </strong>The purpose of this study was to determine the accuracy of the Rinne and Weber tuning fork tests in predicting the degree of hearing loss secondary to otitis media with effusion (OME) in children compared with pure-tone audiometry.</p><p><strong>Design: </strong>A blinded prospective study was performed on cases of unilateral and bilateral OME in children between ages 2 and 11 years.</p><p><strong>Setting: </strong>Patients were recruited from the tertiary Pediatric Otolaryngology Clinic at British Columbia's Children's Hospital in Vancouver.</p><p><strong>Methods: </strong>The subjects consisted of 58 children with unilateral or bilateral OME. The Rinne and Weber tests were performed using a 256 Hz and a 512 Hz tuning fork. Pure-tone audiometry was performed to establish the presence and degree of an air-bone gap (ABG).</p><p><strong>Main outcome measures: </strong>The tuning fork responses were correlated with the ABG to determine their accuracy.</p><p><strong>Results: </strong>The Weber test did not show an association between the ABG and the lateralization response using either the 256 Hz (p = .484) or the 512 Hz tuning fork (p = .120). For the Rinne test, an increase in ABG was significantly associated with a higher rate of response air conduction < bone conduction using either the 256 Hz or the 512 Hz fork (p < .04). Through a retrospective study spread out over 4 years, we brought back six cases of nasopharyngeal tuberculosis, including two men and four women of an average age of 41 years. The diagnosis was made, for all of our patients, after histologic study when we found typical tuberculosis lesions on biopsy specimens. The diagnosis of tuberculosis was established; all patients had an antituberculosis medical treatment in a department of infectious diseases. The goal was to evaluate the oncologic need for dissection in this area owing to the increased morbidity from devascularization of the spinal accessory nerve.</p><p><strong>Methods: </strong>A prospective analysis of 74 patients who underwent 80 neck dissections from July 2002 to March 6, 2004, was undertaken. Seventy-six of the 80 neck dissections were performed in patients with squamous cell carcinoma, whereas the remainder were done for papillary carcinoma of the thyroid. All submuscular recess specimens were sent separately from the remainder of the neck dissection. The location of the primary tumour, staging, presurgical radiation status, and the number of nodes in both specimens were recorded.</p><p><strong>Results: </strong>Forty-six percent (37 of 80) of the neck dissections were positive for malignancy. Of the 80 neck dissections, 8.75% (7) had positive nodes in the submuscular recess.
