This study concentrated on PPAR gamma's potential role in the maturation process of laryngeal squamous epithelium, both normal and premalignant, as well as in the differentiation grade of squamous cell carcinomas (SCCs) of the larynx.</p><p><strong>Design: </strong>A retrospective study.</p><p><strong>Setting: </strong>A basic research anatomy laboratory, operating within a tertiary care institution.</p><p><strong>Materials: </strong>Clinical specimens from 89 subjects with normal laryngeal epithelium and hyperplastic, dysplastic, or malignant lesions.</p><p><strong>Methods: </strong>Paraffin-section immunohistochemistry.</p><p><strong>Main outcome measures: </strong>Strength and extent of PPAR gamma presence, specified by stain intensity and ratio of stained cells, respectively.</p><p><strong>Results: </strong>All stratified histologic categories, that is, normal mucosa, hyperplasia, and dysplasia, displayed a significant increase in PPAR gamma expression in suprabasal differentiated layers compared with basal undifferentiated ones (rho < .01). Hyperplasia and dysplasia manifested lower and higher levels of PPAR gamma, respectively, in comparison with normal epithelium. Whereas grade Pi and III SCCs were characterized by equal expression, well-differentiated tumours possessed considerably raised receptor content. If surgery is indicated, controversy exists regarding staging versus nonstaging and whether to perform stapedectomy or mobilize the fixed ossicles by removal of plaques. The aim of this study was to review the previous reports and to analyze the long-term surgical outcome of 30 patients who have been operated on owing to tympanosclerosis.</p><p><strong>Methods: </strong>Nine hundred twelve cases with chronic otitis media with or without cholesteatoma that have undergone either the intact canal wall or canal wall down mastoidectomy technique were analyzed retrospectively. Thirty patients who underwent ossicular chain reconstruction between 1990 and 2005 owing to conductive hearing loss resulting from tympanosclerosis were selected after excluding those patients who had repair of a sclerotic eardrum only. The types of surgery for restoration of hearing loss, postoperative hearing gain, and closure of air-bone gaps were compared. The average follow-up was 4.3 years.</p><p><strong>Results: </strong>There were 19 male and 11 female patients, with ages ranging between 12 and 48 (27.6 +/- 9 years). Fourteen patients (47%) underwent myringoplasty and ossiculoplasty only, 14 patients (47%) had simple mastoidectomy, and 2 patients (6%) had a modified radical mastoidectomy owing to associated cholesteatoma. Seven patients with advanced tympanosclerosis had stapedectomy and a total ossicular replacement prosthesis (TORP), five patients had stapes mobilization and a partial ossicular replacement prosthesis (PORP), two patients had incus transposition, one patient had a tragal cartilage graft over the stapes as ossicular reconstruction, and one patient had a graft over the incus. Fourteen patients with an intact ossicular chain had a mobilization procedure associated with mastoidectomy and atticotomy. Thirty-three percent of the patients had less than 20 dB air-bone gaps in the long term. Patients with stapedectomy with TORP and PORP application presented with better air-bone gap closure (to 20 dB) compared with those with the mobilization procedure only. None of the patients had a dead ear after surgery.</p><p><strong>Conclusion: </strong>The success of the surgery was dictated by the location and the extent of tympanosclerotic involvement. Sinonasal disease was present in 19 patients, of whom 12 had nasal polyposis. All 12 patients had observed absence of nasal polyps at some point during their ibuprofen course. Nasal polyps were present in five patients during ibuprofen therapy, and all resolved with increased ibuprofen doses. Polyps occurred in six of eight patients after ibuprofen therapy ceased. Five of the 12 patients required endoscopic sinus surgery for polyposis.</p><p><strong>Conclusion: </strong>High-dose ibuprofen therapy chronically administered at appropriate weight-based dosing is a possible treatment option for children and young adults with CF polyposis. {"title":"Clinical features of the paranasal sinus fungus ball.","authors":"Kyung Chul Lee","doi":"10.2310/7010.2007.0037","DOIUrl":"https://doi.org/10.2310/7010.2007.0037","url":null,"abstract":"<p><strong>Objective: </strong>The goal of this study was to delineate the clinical features of the paranasal sinus fungus ball and to evaluate the effectiveness of transnasal endoscopic sinus surgery as a primary surgical method for treating the paranasal sinus fungus ball.</p><p><strong>Setting: </strong>A tertiary medical centre.</p><p><strong>Design and methods: </strong>Eighty-five patients (86 cases) who were treated for paranasal sinus fungus ball with transnasal endoscopic sinus surgery between 1995 and 2004 in our Department of Otorhinolaryngology-Head and Neck Surgery.</p><p><strong>Outcome measures: </strong>Clinical features, surgical techniques, operative findings, postoperative results, and complications were analyzed retrospectively.</p><p><strong>Results: </strong>All cases were treated successfully by transnasal endoscopic sinus surgery. No recurrences or postoperative complications were observed. However, in 18 cases, fungal debris was observed when sinus irrigation was carried out at the first follow-up visit. Hearing test records were investigated, and patients with unilateral or bilateral sensorineural hearing loss were included. Forty-five age- and sex-matched persons served as controls. The ipsilateral and contralateral acoustic reflex patterns of these patients were investigated. Reflex-forming acoustic thresholds and latencies of ipsilateral reversed (positive) reflexes were compared with ipsilateral negative (upward) reflexes.</p><p><strong>Results: </strong>The ipsilateral acoustic reflexes of 18 of 63 ears with sensorineural hearing loss were absent, and the remaining 45 were reversed (positive). A significant difference was found between ipsilateral reversed and ipsilateral negative acoustic reflex thresholds (p < .001) and latencies (p < .001). No statistically significant differences were found between ipsilateral and contralateral negative reflex latencies.</p><p><strong>Conclusions: </strong>The reversed acoustic reflex pattern is not an artifact but a physiologic event. This reflex does not appear to be related to stapedius or tensor tympani muscle contraction. We have measured incidence, intensity, frequency, number of peaks and amplitude of emission and their variability across frequency range from 500 to 5000 Hz. A correlation was determined between the OAEs results and the results obtained using hearing thresholds.</p><p><strong>Results: </strong>We have not found statistically significant differences at 500, 1000, 2000, 4000 and 8000 Hz frequencies neither at mean hearing thresholds between the sample of ears with tinnitus and the sample of ears without tinnitus. SOAEs were only present in 1 of the 44 ears tested (2.27%) and it was a 17 dB SPL amplitude peak at 2770 Hz frequency. TEOAEs, however, were displayed in some frequency in all the ears. We have compared TEOAEs parameters between the sample of ears with tinnitus and the sample of ears without tinnitus in 500, 1000, 2000, 4000 and 5000 Hz frequencies, and we have only found statistically significant differences at 4000 Hz, p = 0.02. Comparison of TEOAEs parameters between ears with tinnitus and ears without tinnitus in the same patient have only found statistically significant differences at 4000 Hz frequency, p = 0.011. Nonsteroidal anti-inflammatory drugs could offer significant relief. Unfortunately, concerns about increased risk of bleeding limit their use. Selective inhibitors of cyclooxygenase type 2 (COX-2), such as rofecoxib, which do not affect bleeding time, could be an interesting option.</p><p><strong>Objective: </strong>To evaluate the efficiency and safety of \"rofecoxib-morphine\" versus \"acetaminophen-morphine\" for post-tonsillectomy pain control.</p><p><strong>Method: </strong>Eighty children (5-17 years old) took part in this prospective comparative study. Forty children were in the acetaminophen-morphine group (group A), and the remaining 40 children were in the rofecoxib-morphine group (group R). Pain scores, administered doses of medication, side effects, and the general health status of the child were collected by telephone calls made on the first, third, and seventh days after surgery.</p><p><strong>Results: </strong>Pain scores for group A compared with group R are as follows: day 0 at home arrival, 4.6 to 2.5; day 0 average, 4.7 to 3.3; day 1, 3.6 to 2.6; day 3, 2.3 to 1.4; and day 7, 1.4 to 2.0. Moreover, children who received rofecoxib generally drank, ate, and returned to their regular activities more quickly than the children of group A. No side effects were reported with rofecoxib.</p><p><strong>Conclusion: </strong>Rofecoxib (COX-2 inhibitor) significantly reduces pain scores after pediatric tonsillectomy. Most reports suggesting a link between sensorineural hearing loss (SNHL) and this drug have been in association with prolonged doses for treatment of Mycobacterium lung disease. Mild-moderate, gradual, and reversible SNHL in the speech frequencies has been most often reported.</p><p><strong>Materials and methods: </strong>We describe irreversible SNHL in a patient in association with low-dose oral azithromycin prescribed for acute otitis media.
