{"title":"[Short term efficacy of inhaler beta2 agonists in childhood asthma (author's transl)].","authors":"R Kraemer, D Duquennec, C Mossay, F Geubelle","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>An attempt was made to assess the relationship between the bronchodilator response of three beta2-agonists (fenoterol salbutamol and terbutaline) administered by a metered aerosol inhaler during 43 lung function measurements on 25 asthmatic children. At constant one minute intervals the changes in total pulmonary flow resistance (R1) and dynamic lung compliance (C1 dyn) were measured. The effect of the three bronchodilators on the mean decrease of R1 was similar and not significantly different. Since R1 may fail to detect peripheral airway obstruction, the efficiency of the three drugs was evaluated on the basis of changes on C1 dyn in two subpopulations, one without signs of overinflation and one with evident overinflation, as judged from the initial values of the functional residual capacities and the C1 dyn's. On the basis of this distinction it is suggested that fenoterol has its predominant effect on patients with overinflation of the lungs and increased C1 dyn probably due to a loss of elastic recoil. {"title":"[Effect of sleep on respiration].","authors":"S Bussi, Y Lecocguic, D Murciano, M Aubier, R Pariente","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Physiologists as well as chest physicians are both interested in respiratory control and disturbances during sleep. Many studies have demonstrated episodic falls in oxygen saturation in lung disease during sleep related to episodic alveolar hypoventilation and/or apneas. A better understanding of these sleep-induced disorders has been possible since the use of electroencephalo-and oculogram (EEG-EOG) to separate the two stages of sleep i.e rapid eye movement sleep (REM sleep) and slow wave sleep (non-REM sleep), and of non invasive methods in assessment of respiratory parameters. Several mechanisms may explain respiratory disturbances during sleep: --a failure of respiratory control systems which differ between the two stages of sleep (central apnea); --a decrease in activity of all upper airway muscles, mainly during REM sleep (occlusive apnea); --a worsening in ventilation-perfusion inhomogeneities which may occur independently of the apnea periods. The detection of such falls in oxygen saturation in pulmonary patients, especially in hypoxemic ones appear to be very important to avoid the adverse effects of hypoxemia on pulmonary circulation. This could be realized by: --an evaluation of respiratory disorders during sleep; --a better assessment of the respiratory control systems in cold patients; --suppression of predisposing factors of occlusive apnea. {"title":"[Massive haemothorax during haemodialysis in a child].","authors":"A Bensman, A Grimfeld, D Vasmant, J P Montagne","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A massive haemothorax occurring acutely during haemodialysis in a child of 8 1/2 years is reported. The pleural effusion occurred synchronously with a round, dense shadow in the parenchyma similar to an intra-pulmonary haematoma. The density became bullous in six weeks and disappeared in three months. The suggested cause of this haemothorax is a pulmonary embolism whose site of origin was the arteriovenous fistula. This would have been responsible for the adjacent pleural and parenchymal haemorrhage in the lung. A low grade resistance to penicillin G is observed in two strains (MIC 0,12 and 0,3 mg/l). Clinical correlations indicate that only 48% of the S. pneumoniae isolated are undoubtedlzy responsible for bacterial infection. The serotypes isolated from blood cultures and from other specimens are not statistically different. Immuno-allergic asthma, mediated by specific IgE of an allergen only represents one aetiological form, among many others, that make up the clinical spectrum of asthma in the human species. It is, however, the type of asthma whose experimental reproduction, a priori, is the most easily reproduced and has been the most tested. A comparative study in 22 subjects with acute (IRA) or chronic respiratory failure (author's transl)].","authors":"J F Muir, P Aubry, P Levi-Valensi","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>22 patients (4 female, 18 male) suffering from severe chronic obstructive respiratory failure (IRCO), were given artificial ventilation because of a super-imposed acute respiratory failure (IRA). The object of this study was to compare different methods of weaning patients from the ventilators using either intermittent mandatory ventilation (IMV) or an automatically triggered ventilation (VAD). Both blood gases in the acute phase and the functional data collected before the acute exacerbation failed to show any statistical difference between the two groups. The reduction of ventilation in the group of patients weaned by the technique of IMV did not appear statistically significant as shown by the following facts : controlled ventilation (IMV : 2.45 +/- 2.06 days; VAD : 4.72 +/- 3.89 days) : total duration of ventilation (IMV : 7.35 +/- 2.15; VAD : 8.54 +/- 6.56 days). The PaO2 of the subjects weaned by the technique IMV appeared higher before extubation (P less than 0.02) than the PaO2 obtained after weaning on VAD. {"title":"[An inquiry into the use of antibiotics in acute respiratory infections in primary health care centres in Algeria during 1980 (author's transl)].","authors":"P Chaulet, N Ait Khaled","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In an acute respiratory infection the prescription of antibiotics is often still routine in Algeria. Antibiotic consumption has accounted for 20 to 28% of the drug bill in recent years. A prospective pilot study was carried out in 1980 in 26 health centres in different parts of the country. This has allowed an evaluation to be made of the importance of acute respiratory infections in the demand for primary health care and the frequency of antibiotic prescriptions. In 1980 acute respiratory infections accounted for 23% of consultations in primary medical care. In 69% of cases they were for upper respiratory tract above the glottis sometimes extending to the bronchi; in 28% the infections were isolated to the trachea and bronchi; in 3% the infections were bronchiolar, alveolar or pleural. Although the localisation of acute respiratory infections suggests a predominantly viral origin, antibiotics were prescribed in 76% of cases; these prescriptions depend more on the habit of the prescriber than the observed pathology. An agreed strategy is required to deal with acute respiratory infections at the primary care level and is a priority for health care. This is illustrated by case reports on two boys aged 10 and 12. The clinical, pulmonary function and radiological details were very similar in the two cases, suggesting pulmonary hypoplasia. However, the histology seen in the second case, a type of bronchiolitis obliterans and follicular bronchiectasis were among several arguments in favour of acquired pulmonary disease of infective origin.
