Therapeutic education program must be tailored to the characteristics, needs and expectations of the population. In the ENT Department of Head and Neck Surgery, Larrey Hospital in Toulouse, a therapeutic education program for patient with total laryngectomy has been experienced since 2011. But its propagation remains difficult. The aim of this study is to determine if social factors are nfluencing the participation of the laryngectomized population in the program. The brochure explaining this program and a registration form coupled with a survey questionnaire were distributed to the regional population of patient with total laryngectomy. After two months of investigation we collected 42 responses. {"title":"[Dynamic palatography: Diagnostic tool for dysfunctional swallowing? The speech re-education of dysfunctional swallowing aims to correct or prevent the recurrence of occlusal disorders. The main objective was to test the dynamic palatography as a diagnosis and quantification tool of the dysfunctional swallowing.</p><p><strong>Material and methods: </strong>The study was prospective and descriptive. Twelve average 23.5 years old women with a clinical dysfunctional swallowing have been included between January and May 2014. None was aware of presenting an atypical swallowing or dento-facial dysmorphism of class II. The dynamic palatography device measured the pressure force of the language on the palate during the lingual rest, swallowing saliva and water. Parameters measured were the duration and magnitude of support of the tongue on the palate.</p><p><strong>Results: </strong>Dynamic palatography showed a trend to predominant anterior contact during rest position (25%), and lower position of the language with little contact during swallowing of saliva and water.</p><p><strong>Discussion: </strong>Palatography results are consistent with the clinical diagnostic criteria of atypical swallowing. Our palatography tool has the advantage of being unobtrusive in the mouth compared to other pre existing systems. This device should be tested on larger patient populations and could enable monitore atypical swallowing rehabilitation efficiency. It is characterized by \u0000aspecific clinical presentation, causing a diagnostic problem. The diagnosis is based on a combination of imaging techniques as well as histological examination.</p><p><strong>Case presentation: </strong>We are presenting a case of a patient with thyroid basi-lingual treated surgically.</p><p><strong>Discussion: </strong>The low incidence of ectopic lingual thyroid , and their clinical variability requires radiological and isotopic investigations.</p><p><strong>Conclusion: </strong>The diagnosis of this disease is primarily histological. {"title":"Chondrosarcoma of the hyoid bone: Case report and review of the literature.","authors":"D Amaral, C Zagalo, C Cardina, P Vera-Cruz","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Introduction: </strong>Chondrosarcomas are cartilaginous tumors that range from low-grade tumors with low metastatic potential to high-grade aggressive tumors with premature and high metastatic rate. Low-grade types have few karyotype abnormalities and are near-diploid, while high-grade chondrosarcomas originate from complex karyotypes and are aneuploid. The most common karyotype aberrations found include 12q13-15 and 9p21 rearrangements. Here, it is presented a case of chondrosarcoma of the hyoid bone with its image, surgical procedure and pathological correlation as well as a review of the literature.</p><p><strong>Case presentation: </strong>A sixty-three year old man presented with a large cervical mass located at submental level. The CT scan revealed a voluminous and well-limited mass of 5 x 4 x 3 cm located in the suprahyoid region. The operative finding revealed a grade 2 chondrosarcoma originating from the hyoid bone.</p><p><strong>Discussion: </strong>Chondrosarcoma of the. hyoid bone is a rare pathology, thus making it difficult to diagnose. Fine needle biopsies have high diagnostic accuracy, although correct grading is only obtained 46% of the time. CT scan is the golden standard to characterize tumor extension and origin. Published studies have low statistical power. However it seems that ENT localization have favorable prognosis. Management and check up are not well codified.</p><p><strong>Methods: </strong>Bicentric retrospective study conducted between 1987 and 2015, from patient diagnosed with ENT amyloidosis. The study was performed to the database of the pathology department. People concerned, history, symptoms and diagnostic features were analysed. The immunologic and clinical status, locations, extension check, treatment and prognosis have been evaluated.</p><p><strong>Results: </strong>Twenty patients were evaluated, ten men and ten women, average age was 55.5 year of age. Three patients were afflicted with familial amyloidosis. Main localisation was larynx (80%), main type was immunoglobulinic (AL) (80%). Amyloidosis was mostly localised (90%) and primary form (80%). Dysphonia was the most frequently encountered symptom. Most performed examination were local biopsy and creatinine clearance (100%), serum protein electrophoresis (SEP) (89%), myelogram and/or bone marrow aspiration (75%), and trans thoracic echography (TTE) (75%). Surgical removal was performed for 75% of the patients. Global rate of recurrence was 70%, about 4.6 years after diagnosis. In familial forms, overall survival was 66% at ten years. In non-familial forms, overall survival was 100%.</p><p><strong>Conclusion: </strong>ENT amyloidosis are mostly AL, laryngeal, primary and localised. Distant extension check should be managed by internal medicine specialist and associate creatinine clearance, local biopsy, TTE, SEP and myelogram. We describe the case of a 55 years-old woman, who came to our consultation for atypical trigeminal neuralgia of left V1 and V2 nerves. Brain MRI found two tumors: T1W hypointense with no appreciable enhancement after gadolinium injection and T2W and diffusion hyperintense. This last feature was in favour of an epidermoid cyst, but the multiplicity of cerebral lesions was definitely not in favor of such a diagnostic. They were located behind the right eye and in the left Meckel’s cave (trigeminal cave). The surgical strategy consisted in removal the retro orbital tumor witch was the most accessible of both the diagnostic of epidermoid cyst was retaned thanks to the anatomopathology report. As these lesions had the exact same characteristics, we concluded that they were similar. The aim of our study is to identify the role of some epidemiological, clinical and therapeutic factors in recurrence after surgery of nasal polyposis.</p><p><strong>Materials and methods: </strong>We conducted a retrospective study over a period of 11 years (between 2000 and 2010) including 184 patients operated for nasal polyposis after failure of prolonged medical treatment. We evaluated the impact of epidemiological and clinical factors (age, sex, asthma, Widal disease, allergy and stage of nasal polyposis at the time of surgery) and treatment (surgical technique, observance of postoperative topical steroids ) on postoperative recurrence.</p><p><strong>Results: </strong>Nasal polyposis recurred in 26.6% of patients after an average period of 23 months. Widal disease, asthma and bad observance of the intranasal steroid therapy were significantly associated with postoperative recurrence in the univariate analysis. Examination included an otoscopy and a pure tone audiometry.</p><p><strong>Results: </strong>On 23121 candidates, 1245 had a plug of earwax (5.4%). They were aged from 18 to 25 years old (average age: 22.85 years). Eardrum was pathological in 0.5 % of the cases. Prevalence of hearing loss was 1.5%. Hearing loss included sensorineural hearing loss (56.7%), deafness of transmission (29.4%) and mixed deafness (13.9%).
