{"title":"Value of the preservation of an osseous mandibular rim with a fibula free flap reconstruction.","authors":"S Cortese, B Phulpin, R Mastronicola, P Gangloff, J Guillet, M Roch, L Julien, J L Verhaeghe, G Dolivet","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Background: </strong>Reconstruction of the mandible with microvascularized fibula transplants is actually a well-established procedure, yet the major component is the limited diameter of the diaphysis that can induce oral rehabilitation's failure, especially in dentate patients. In this paper, we report our surgical procedure allowing preservation of the mandibular height. The primary objective was to assess aesthetic and functional improvements of preservation of an osseous mandibular rim with a fibula free flap reconstruction.</p><p><strong>Patients and methods: </strong>Five patients (all males, mean age of 60 years) were treated with this method. Aetiologies were tumour in 3 cases, and osteoradionecrosis in the two others cases. We described all step of our surgical procedure and the functional, aesthetic and carcinologic results were evaluated. The follow up varies from 6 to 30 months.</p><p><strong>Results: </strong>One patient died at 12 days from unrelated affection. For the other patients, both the aesthetics and functional outcomes were better than in case of mandibular interruption surgery. In fact, the mandibular contour of the mandibule was preserved and the height of mandible was restored. One patient is in progress of dental rehabilitation with osseous implants. Carcinologically, no local recurrence was observed.</p><p><strong>Conclusion: </strong>This technique is reliable and enables to optimize oral rehabilitation with endosteal implants. Nevertheless, we consider that the 3D scanner is essential before the intervention to evaluate the osseous reach. Respiratory evolutionary prognosis imposed trans-oral surgical treatment.</p><p><strong>Discussion: </strong>We discuss the diagnostic and therapeutic strategy for the benign tumors of the parapharyngeal space.</p><p><strong>Conclusion: </strong>The schwannoma of the glossopharyngeal nerve is a benign rare lesion. The difficulty lies on the surgical strategy and the choice of the approach. {"title":"[Benign tumors of the nasal cavity, new classification and review of the literature. Report of 54 cases].","authors":"S Kharoubi","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Benign tumors of the nasal cavity is an uncommon disease and very diverse about histological variety. Theses tumors are seen at any age without specificity in semiological aspect. The diagnostic approach is based on nasal endoscopy, imaging (CT-MRI) and biopsy. In some anatomo clinical varieties the result of biopsy is so difficult and we must analyze all the tumor specimen after surgery with modern biological procedure (immunohistochemistry, molecular biology). The treatment is surgery especially with endonasal endoscopic procedure. Our series included 54 benign endonasal tumors between January 1998 and December 2008. The mean age of our population is 28 years with a female sex ratio of 1.16. Endoscopic examination and a CT scan of facial bones were systematic. The tumors of nasal septum were predominant. The histological variety are dominated by vascular tumors (hemangioma) in 36 cases (66.6%) and papilloma (papilloma and inverted papilloma) in 15 cases (27.7%), two cases of fibroma (37%) and one case of leiomyoma. The treatment was surgical with an endonasal approach in 37 cases (68.5%), endonasal and endoscopic in 11 cases (20.3%) and external 6 cases (11.1%). We propose to study here three technologies with contributions to music. All are developed and patented by the Bell Labs: The artificial larynx (and its derivatives, Sonovox and TalkBox), the vocoder and the speech synthesis. After a presentation of the source-filter theory, vital to these innovations, the principle of these three technologies is explained. A brief historical is outlined and is complemented by examples of films and musical selections depicting these processes. Although frequent, it remains largely underdiagnosed by clinicians [1]. We report one case of Eagle syndrome studied at the ENT and radiology departments of the Heinrich Lübke hospital in Dioubrel. Radiologic, therapeutic and clinical data have been documented. The principle of smas plication from two fixed points allows an important tissue ascension. Our purpose was to objectively evaluate the SMAS plication results, although a common critic may be due to its early deep tissular displacement.</p><p><strong>Materials and methods: </strong>We present the case of a 65 y.o. female patient who received a FNL. Miniature metal pins were placed on the external surface of the SMAS in order to ensure radiological monitoring. A cephalometric and photographic monitoring were performed preoperatively and on day 1, months 2, 6 and 16 postoperatively . We performed a computed numerised analysis of the movement of the markers in an orthogonal axis.</p><p><strong>Results: </strong>All our bearings kept a constant position in the radiographic follow-up , demonstrating the fixity of SMAS kinked 16 months postoperatively. Clinically the correction remained a good appearance with a very slight relaxation of the superficial tissues (skin and subcutaneous fat).</p><p><strong>Conclusion: </strong>In the case of our patient, the tensioning of the deep facial plan (the SMAS) objectively appeared to be efficient over time, with an overall delay of 16 months. Our protocole appeared to be coherent without any specific complication. {"title":"Epitympanic osteoma of the middle ear: A case report and literature review.","authors":"M Abouzayd, A Seghir","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Introduction: </strong>Contrary to osteomas and exostosis of the external ear canal, middle ear osteomas are rare benign tumors that are usually revealed by progressive hearing loss.</p><p><strong>Case report: </strong>We here report the case of 56-year-old woman who had a left ear conductive hearing loss. A CT-scan showed a bone-density tumor in the left posterior epitympanum, hanging up to the tegmen tympani. A surgical middle ear exploration showed an epitympanic osteoma, confirmed by a histological exploration. The tumor was left in place and an ossicular prosthesis was inserted, which permitted a significant gain in the audiogram air-bone gap.</p><p><strong>Discussion: </strong>Middle ear osteomas are rare benign tumors (31 cases described from 1964), with male preponderance (sex ratio: 1.75). They always present with a hearing loss, and the diagnosis is made by CT-scan. It also involves other pathological situations such as stuttering and autism seen in Alzheimer's disease. Bilingualism is now a recognized cultural, social, educational and professional substantial asset. Despite the fact that it is a single language system - where grammar, accents, the rhythm of the sentence remain unchanged - it will be interesting to draw some parallels with multilingualism. This will lead us to study the processes of oral and written language acquisition in children.
