{"title":"[Laryngeal schwannoma: a case report].","authors":"R Perouse, B Coulombeau","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The authors report the case of a patient of 67 years, an active teacher active with no relevant history, which noticed several months ago, a gradually progressive dysphonia, though not major (G1R2B0). The stroboscopic examination which proved extremely difficult, could only demonstrate normal laryngeal mobility, while the left ventricular band and left vocal fold appeared affected by a subepithelial tumoral aspect, appearing as a regular swelling. These data were confirmed by naso-fibroscopy, and later by computed tomography. Endoscopy, which was also extremely difficult, brought about the diagnosis of schwannoma in the region of the left ventricular band. {"title":"Phoniatric management of 3 clinical cases of voice disorders involving the recurrent nerve.","authors":"M A Faure","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>These three cases involve aphonia or dysphonia as a consequence of entrapment of one of the recurrent nerves, in the absence of any prior surgical history. Depending on the particular case, one to two months was needed to normalize phonatory function; One case relapsed after a year. The phoniatric management of these cases involved: New functional and acoustic pattern, recorded by the patient as a form of daily maintenance; Laryngeal and perilaryngeal manual therapy, applied according to the \"osteovox\" method. They are characterized by a histological diversity. They can be benign or malignant. The most common histologic type is the hemangioma. Their management is not well codified, it benefited from advances in modern imaging and endoscopic surgery. The objective of the work is to study anatomical and clinical characteristics, therapeutic and outcome.</p><p><strong>Patients and methods: </strong>The observations of 10 vascular tumors of the nasal cavities collected between January 2009 and July 2011 were studied retrospectively. The management of these tumors was based on nasal endoscopy, imaging, biopsy, and some angiography for embolization. The epidemiological parameters, histopathological, clinical, therapeutic and outcome were studied.</p><p><strong>Results: </strong>The average age was 25.4 years, male was marked with a sex ratio of 4. Epistaxis was the revealing sign. All tumors were benign, with a predominance of the hemangioma (4 cases), followed by angiofibroma of the septum (3 cases) and nasopharyngeal angiofibroma (3 cases). All patients were operated, by endonasal technique in 7 cas/10 and transfacial road in 3 cases. Embolization was performed in 5 patients, there were two hemangiomas and 3 nasopharyngeal fibromas. No case of recurrence has been noted so far.</p><p><strong>Conclusion: </strong>The vascular tumors of the nasal cavities are difficult to treat, especially when they reach a large volume and at an extension to adjacent areas, which makes the surgical procedure difficult and bloody. However, little is known about the stimulus of aggressive bony surgery in the development of these tumours.</p><p><strong>Material and methods: </strong>We report the case of a young girl in whom dental extraction was rapidly followed by the occurrence of a localized tumour 11 years after allogeneic haematopoietic stem cell transplantation using total body irradiation (TBI) for a haemophagocytic lymphophistiocytosis (HLH).</p><p><strong>Results: </strong>This tumour involved tooth socket and all the right side of the mandible and was diagnosed as an osteogenic osteosarcoma of the zygomatic bone.</p><p><strong>Conclusion: </strong>This tumour had the characteristics of a radio-induced sarcoma. {"title":"[Kikuchi-Fujimoto's disease: Report of 4 cases and literature review].","authors":"L Lamzaf, H Harmouche, M Maamar, M Adnaoui, M Aouni, Z Tazi Mezalek","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Introduction: </strong>Kikuchi-Fujimoto's disease or histiocytic necrotizing lymphadenitis is a benign disease predominantly occurring in young women which etiology remains unknown and revealed by cervical lymphadenitis and/or prolonged fever.</p><p><strong>Observations: </strong>This report describes a survey of four patients who developed Kikuchi's lymphadenitis occurring concomitantly with LES in one case and actinomycosis in another case. The definite diagnosis is usually made through histopathological examination of a lymph node biopsy. Sinuso-nasal metastases from adenocarcinoma occurring in other localisations are exceptional. We report a case of a patient presenting with an ethmoidal metastasis revealing a pulmonary adenocarcinoma.</p><p><strong>Case report: </strong>A 57 years-old patient consulted for a right fronto-orbital pain which had appeared 3 weeks prior, associated with a central scotoma of the right eye and a third and fifth cranial nerve palsy. MR-imaging revealed a right ethmoidal tumour invading the orbit. Pathology results were in favour of a metastasis from a secondary adenocarcinoma. The CT-scan disclosed a pulmonary tumour of the right apex with liver and spleen metastasis.</p><p><strong>Conclusion: </strong>Ethmoidal metastases are rare and often diagnosed in patients presenting with a previously known tumour. We report the first case of a pulmonary adenocarcinoma revealed by an ethmoidal metastasis. {"title":"[Idiopathic sudden deafness: a report of 96 patients].","authors":"F Gabanou, G Bera, C Vincent","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objectives: </strong>Evaluation of the management of idiopathic sudden deafness indicating the usefulness of biological assessments and the pronostic factors of hearing recovery.</p><p><strong>Patients and methods: </strong>This is a retrospective study of 96 patients with idiopathic sudden deafness referred to a tertiary centre between 2005 and 2009 treated with corticosteroids intravenously at a daily dose of 1 mg/kg. Mean tonal thresholds were assessed (PTA = [500 Hz + 1000 Hz + 2000 Hz + 4000 Hz]/4). Each audiogram was classified as five classes according to its frequency profile.</p><p><strong>Results: </strong>The hearing recovery is significant between D0-D5 and D5-M1 for the frequencies 0.5, 1 and 2 kHz. For 4 kHz, the recovery is significant between 0 and J5. There is no statistically significant correlation between the presence of associated signs (tinnitus, vertigo) and hearing recovery. Hearing recovery according to the five types of audiograms has the same evolution in the follow-up time but with audiograms type E (cophosis or subcophosis) often associated with an hyporeactivity at the videonystagmography. The presence of cardiovascular disease is a predictor of poor hearing recovery. The usefullness of systematic extensive blood tests is low.</p><p><strong>Conclusion: </strong>In sudden deafness, the maximum hearing recovery takes place in the month following the onset of symptoms. {"title":"[Ossiculoplasty with hydroxyapatite bone cement].","authors":"J M Gérard, A Cailliau, D Franceschi, M Gersdorff","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To analyze the results obtained with hydroxyapatite bone cement (HABC).</p><p><strong>Patients: </strong>A total of 73 ossiculoplasties with HABC were evaluated. Ears were divided into 4 groups: Group 1: Reinforcement of the incudo-stapedial joint with HABC; Group 2: Reconstruction of an incus long process defect with HABC; Group 3: Partial ossicular reconstruction between stapes and malleus handle with HABC; Group 4: Reconstruction of stapes with mobile or fixed footplate with moderate or extensive incus long process erosion using a titanium piston glued to the incus remnant using HABC.</p><p><strong>Results: </strong>The mean follow-up was 21 months. The percentage of postoperative average air-bone gap (ABG) < or = 20 dB for groups 1, 2, 3 and 4 were 100%, 95%, 83% and 91% respectively; for ABG < or = 10 dB: 90%, 71%, 50% and 50%. No complications related to HABC and extrusion occurred.</p><p><strong>Conclusion: </strong>Today, the use of HABC seems to help us improve our functional results and also to avoid extrusion. In our experience, ossiculoplastie with HABC seems to provide better and more stable functional results. HABC is safe and easy to use. The use of cement with or without biocompatible ossicular prostheses allows us to repair different types of ossicular defects whilst trying to conserve an anatomical and physiological ossicular chain. {"title":"[Middle ear barotraumas due to rhinopharyngeal scar tissue: tubomanometry diagnostic and therapeutic contribution].","authors":"M Collin, M Coulange, A Devèze, M Montava, D Estève, J P Lavieille","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>Evaluation of tubomanometry contribution to diagnosis middle ear barotraumas in relation with rhinopharyngeal scar tissue, and contribution to check postoperative effectiveness of scar tissue surgical resection.</p><p><strong>Methods: </strong>Clinical cases study of two stewardesses who have lost their flight fitness consecutively to barotraumatic otitis during landing, engendered by rhinopharyngeal scar tissue.</p><p><strong>Results: </strong>Pre-operative tubomanometric parameters were abnormal in both cases: decrease of intratympanic pressure in one case, variability of tube opening latency index and lengthening of intratympanic pressure rising time in the other case. Surgical section during endonasal endoscopy results in initially abnormal tubomanometric parameters normalization, allowing resumption of flight fitness without any barotrauma.</p><p><strong>Conclusion: </strong>Some abnormal tubomanometric parameters help to establish causality link between middle ear barotrauma and rhinopharyngeal scar tissue for which surgical section is thus indicated. Post-operative tubomanometric parameters normalization prove surgical effectiveness.
