The methodology applied included a questionnaire, acoustic analysis of the records, a videolaryngostroboscopic examination, analysis of objective and subjective measures of vocal function. The results confirmed the existence of voice risk factors and symptoms of vocal fatigue that may justify a preventive intervention. The adaptations that program sellers carry out follow the characteristics of what is called \"Lombard Speech\". {"title":"[Aesthetic augmentation of the dorsum, mid-term results].","authors":"C Winter, O Gerbault, P Kestemont, L Castillo","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Purpose of the study: </strong>Dorsum aesthetic augmentation can be divided according to their objective, total and partial increase or camouflage. The objective of this original article is to define the valid techniques in each indication through a cohort study, clinical cases, and current data from the literature.</p><p><strong>Patient and method: </strong>It is a monocentric mono operator retrospective study from 2005 to 2010 included. On 171 rhinoplasties, 57 were augmentation rhinoplasties of which 40 were of interest to the dorsum. Excluded patients were bone grafts, lost and one patient operated on a active Wegener desease. Thus 26 rhinoplasties were analyzed by an independent observer.</p><p><strong>Results: </strong>All grafts confused there were 11.5% of resorption which corresponds to the data from the literature, 17% of resorption in the camouflage indications and 7% in augmentation, as well as a higher resorption for crushed cartilage (33%) rate. There was more mobility in augmentation (28%) than in the camouflage (8%) and greater visibility of the banks of the graft in augmentation (35%) compared to the camouflage (8%).</p><p><strong>Conclusion: </strong>In the mild to severe saddle nose, the DCF is greater than cartilage monobloc or crushed in terms of stability and visibility, its indications could be expanded to harmonisation. Camouflage crushed cartilage is not sustainable and the temporal aponeurosis could it be preferred. The mean age was 7.5 years ranging from 03 months to 15 years. MIF plays a key role in cell cycle regulation and in the pathogenesis of many cancers. Recently, MIF has been studied in the upper aerodigestive tract cancer for its involvement in tumor progression, invasion, proliferation and cell motility. In addition, MIF appears to be a mediator in angiogenesis and in the development of metastasis and locoregional lymph node, which are often associated with a poor prognosis. The mechanisms of action responsible for MIF involvement in tumor progression are not completely elucidated. However, the main effects of MIF are mediated by the CD74 receptor. MIF binding to its receptor is responsible for the activation of several signaling pathways (ERK1/2 - MAPK, JAB1 - CSN5, PI3K - Akt), the inhibition of p53 and the stimulation of angiogenic factors including VEGF and IL-8. The overexpression of MIF also causes a reduction of the anti-tumor activity of the immune system. Finally, MIF could be an interesting biomarker in the diagnosis and monitoring of upper aerodigestive tract cancers. Most of them (about 70%) are diagnosed at an advanced stage (stage III or IV) while they are associated with a poor prognosis (only 40% five year survival). The objective of our study was to analyze the care pathway of patients with cancers of uppers aero-digestives tracts in order to target efforts to improve the survival of these patients.</p><p><strong>Methods: </strong>It was a descriptive and retrospective study, on medical files, on the health care pathway of patients with cancers of uppers aero-digestives tracts cared in the Head and Neck surgery department of Val de Grâce in Paris and Percy in Clamart between January 2004 and December 2006. The patients were adults with squamous cell carcinoma of uppers aero-digestives tracts.</p><p><strong>Results: </strong>One hundred thirty-eight files of patients were analyzed. Fifty-five percent of patients were diagnosed at an advanced stage. On average patients have waited two months and twenty-one days before consulting a doctor for the first time. The time interval between the specialist consultation and the start of treatment was on average 7 weeks. The overall 5-year survival rate was 61%.</p><p><strong>Conclusion: </strong>Squamous cell carcinoma of uppers aero-digestives tracts remains serious and has a poor diagnosis, even in a population with a high social-cultural level. The long time interval before the first consultation may be reduced by improving health education among the general practitioner (primary and secondary prevention), and by establishing health care public campaigns. {"title":"[Post-operative evaluation of 63 cases of rhinoseptoplastie by nasal frameworks].","authors":"C Hardy, D Goga, F Disant","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Introduction: </strong>The rhinoplasty is a difficult operation in plastic surgery of the face. The principles of the extracorporeal septoplasty were launched in the 50's, consisting of the treatment of the septal framework outside the nose. The most recent evolution, which is the creation of nasal frameworks required a precise evaluation of the results.</p><p><strong>Patients and methods: </strong>The patients having benefited from a rhinoseptoplasty by nasal frameworks are included. Eleven criteria are studied. Every patient is rated before and after rhinoplasty procedure, one point if improvement of the criterion, zero if no modification and -1 if degradation of the criterion.</p><p><strong>Results: </strong>Sixty three patients were operated, 54 women and 9 men. The postoperative average follow-up is of 5 months.</p><p><strong>Conclusions: </strong>The cartilaginous nasal frameworks, allows on complex noses (diverted, traumatic), an improvement of the righteousness of the dorsum (88%), of the projection of the tip (82.5%), of the naso-labial angle (84%), lines of Sheen (63%). We report the case of a 17-month baby who has a lingual lesion. MRI shows a well-defined cystic lesion. Treatment consisted of a complete resection and histology found a pseudostratified respiratory type epithelium. Only 10 pediatric cases of bronchogenic cyst of the tongue have been reported in the literature. MRI is the imaging modality of choice and treatment is always surgical. Were analyzed: demographics, quality of life questionnaires (QLQ-C30 and H&N 35 EORTC) and quality of swallowing questionnairex (MDADI, DHI and EAT 10) with an average of 1 year after the end of the treatment.</p><p><strong>Results: </strong>Fifteen patients (10 males and 5 females), mean age of 65 years (47-73 years) were included. The tumours were classified as: cT1: 4; cT2: 10 cT3: 1. Five of the 15 patients received postoperative chemoradiotherapy as histo-pathological studies showed multiple metastatic cervical lymph nodes +/- capsular rupture. For the analysis of the quality of life and quality of swallowing, the patients were divided into two groups. Group A included patients who underwent CTARs and group B, patients operated by CTAR with adjuvant chemoradiotherapy. At 12 months of the procedure, all patients had a deglutition considered as normal without feeding tube nor tracheostomy. With the three scales used, the quality of swallowing was satisfactory for all patients. It was better for patients in group A than for patients in group B. In terms of quality of life, the EORTC QLQ-C30 scale showed our patients had a high rate of satisfaction scale in \"health and overall quality of life\". For the EORTC H&N35 questionnaire, mean scores for \"specific symptoms\" were also low except for the following three items \"dry mouths\", \"sticky saliva\" and \"sexuality problem\". The first two items were statistically more frequent (p = 0.02 and p = 0.001) in group B patients.</p><p><strong>Conclusion: </strong>After transoral robotic-assisted oropharyngectomy, patients have a good quality of life and swallowing. Postoperative chemoradio-therapy compromises the quality of swallowing. The aim of our study was to analyze the prognosis of patients operated on for CCM and having no lymph node involvement after analysis of sentinel lymph node (SLN).</p><p><strong>Material and method: </strong>This was a retrospective study of 10 cases of CCM stage I or II who received the SLN technique in our institution between 2003 and 2010. RESULTS Among the 10 patients, 7 had an identification of at least one SLN, among which the node could be surgically removed in 6 cases. In cases of failure of SLN technique, a neck dissection +/- parotidectomy was performed. No lymph node metastasis was found in our series. Mean follow-up was 38 months. The overall survival rate was 100% at 2 years and 50% at 5 years.
