Among 40 patients of the use of thermosplint, 27 cases wore it on some places in their hands, and 17 cases did in feet. We observed that 31 cases of the wear of thermosplint had effect on prevention or improvement in their wound, but 3 cases did not. Particularly good effect was obtained in 23 cases of the wear in their hand and 8 cases in foot. In 10 cases, however, we had to stop wearing it because of another damage or patient's uneasy. We found it difficult to apply the use of thermosplint to the plantar ulcer. Some factors such as histological and anatomical ones should be considered in more detail. Several problems of hindrances or inconveniences for daily life in wearing the thermosplint should be solved by further studies including devices for better application of the technique to the leprosy patients. At any rate it should be stressed that both patients and nurses had taken interest in the cause of the injury and could had positively come to grips with prevent of it through this study.</p>","PeriodicalId":76236,"journal":{"name":"Nihon Rai Gakkai zasshi","volume":"63 2","pages":"55-61"},"PeriodicalIF":0.0,"publicationDate":"1994-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"18843539","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Recent investigators suggested that the reaction to the shared antigenic epitope between HSPs may link infections with induction of autoimmune processes. In the present study, antibody level to HSP with 65 kDa (HSP65) of Mycobacterium leprae was investigated with enzyme-linked immunosorbent assay (ELISA) in various skin diseases. Comparing to normal group (n = 9) including patients with nevus cell nevus showing 0.097 +/- 0.039 (mean +/- SD) in anti-HSP65 IgG level (OD492), 20 patients with palmoplantar pustulosis (PPP) and 22 with psoriasis demonstrated elevated (0.170 +/- 0.079, 0.111 +/- 0.053, respectively) level. Among them patients judged as focal infection-related PPP or psoriasis showed significantly higher level of anti-HSP65 (p < 0.01) than those without focal infection. Anaphylactoid purpura (0.125 +/- 0.085, n = 5), Behcet disease (0.178 +/- 0.045), atopic dermatitis (0.218 +/- 0.096, n = 13), urticaria (0.185 +/- 0.079, n = 30), and herpes zoster (0.193 +/- 0.092, n = 13) showed likewise elevated anti-HSP65 antibody. Similar tendency was found in anti-HSP65 IgM level but not in anti-HSP65 IgA. Western blotting confirmed specific immunoreaction bands to HSP65 in blood samples with high titer. Immunomodulation by stress proteins of bacterial or host cells is assumed in pathophysiology of inflammatory skin disorders, especially in relation to focal bacterial infection as observed in cases with PPP and psoriasis.</p>","PeriodicalId":76236,"journal":{"name":"Nihon Rai Gakkai zasshi","volume":"63 1","pages":"3-11"},"PeriodicalIF":0.0,"publicationDate":"1994-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"18924072","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Earlier records have not been found. After the establishment of two private hospitals, first by an English missionary and then by a French Catholic, a public hospital, Kyushu Leprosarium, was started in 1909. In spite of the segregation policy for the management of HD patients, they lived near the Honmyoji temple until the arrest of 157 patients on July 9, 1940. Why they lived there for a long period of time is discussed in this paper. Factors might include the absence of prejudice around the Honmyoji temple where patients and healthy people lived together, economically favorable situations of patients living there, and bitterness of life in the Kyushu Leprosarium. The abolishment of the settlements as well as that of Kaishun Hospital for HD patients, run by an English missionary, might be in preparation for the coming war, but details have not been known.</p>","PeriodicalId":76236,"journal":{"name":"Nihon Rai Gakkai zasshi","volume":"62 3","pages":"118-24"},"PeriodicalIF":0.0,"publicationDate":"1993-11-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19179895","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

As a result, repetitive foot ulcers are frequently seen. When these repetitive shallow ulcers become infected, they result in deep soft tissue ulcers and even osteomyelitis. The treatment of these ulcers in leprosy is a challenging problem both to the patient himself and to medical personnel. Conventional treatment methods for these ulcers include, according to the wound condition, skin graft, local flap, long-term cast immobilization, wedge resection of the joint and even amputation in severe cases. But all these methods have the disadvantages of frequent recurrences, long hospitalization periods and permanent foot deformities. Recently in Korea, despite the decreased occurrences of new patients and the decreased frequencies of facial and hand deformity formation due to the active early diagnosis and treatment, there still exists a large number of foot deformities resulting from decreased foot sensation. Moreover, treatment modalities are not definitely established. In fact, there are many environmental difficulties in applying the free flap transfer to the foot ulcer of leprosy patients in leprosy sanatorium as opposed to treating the non-leprosy patients. And so, from 1990 to 1993, we performed 6 superior-based posterior calf cross leg flap transfers and 1 distal-based flap transfer, under spinal anesthesia, referred by the leprosy sanatorium for deep foot ulcers. We obtained favorable results in 6 patients and partial necrosis in a patient who received distal based flap transfer.</p>","PeriodicalId":76236,"journal":{"name":"Nihon Rai Gakkai zasshi","volume":"62 3","pages":"99-110"},"PeriodicalIF":0.0,"publicationDate":"1993-11-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19179897","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
