{"title":"Influence of age on the surgical treatment of cancer of the colon.","authors":"V Păunescu,&nbsp;S Răgălie","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The study of 152 patients with cancer of the colon (CC) showed that age represents a factor of risk in the evolution of disease having a decisive influence on the immediate and delayed results of the surgical treatment. Over two thirds of the cases with advanced clinical stages were detected in subjects over the age of 50. In young agers the evolution of CC was severe and metastases appeared early. In old agers the incidence of disease increased and postoperative mortality was maximum. It was observed that IFN reduced significantly by 25-42% the mean healing period of attacks. This reduction was more important if the period till disappearance of vesicles was considered than if the period till complete epithelialization was taken into consideration. Likewise IFN was proved more effective if administered within the first 2-3 days after onset of attacks than 5 days after. As compared with the group which received placebo the effect of IFN was more marked when the size of the lesions was smaller than 2 sq. cm. IFN did not reduce significantly the number of cases with bacterial superinfections nor the rate of relapses in the first three months after treatment. The case had the following peculiarities: onset at a very early age (8 months); involvement, in some attacks, of the nasal mucosa with nasal obstruction and sinusitis manifestations; unusually long duration (as long as 9 days) of some of the localized edema. The absence of attacks during pregnancy was noted. The family study over 4 generations illustrated the dominant hereditary character of genetical transmission. After this treatment left ventricular end-diastolic diameter (EDD) decreased from 6.4 +/- 0.5 to 6.2 +/- 0.6 cm (p less than 0.05), left-ventricular end-systolic diameter (ESD) decreased from 5.7 +/- 0.5 to 5.4 +/- 0.6 cm (p less than 0.001), the ejection fraction increased from 30.8 +/- 7.0 to 36.2 +/- 6.9% (p less than 0.005) and mean velocity of circumferential fiber shortening (Vcf) increased from 0.51 +/- 0.12 to 0.62 +/- 0.13 circ/sec (p less than 0.001). Captopril markedly relieved dyspnea and fatigue. Three patients improved from class IV to class III, 4 patients from class IV to class II and 3 patients from class III to class II. It has been stated that the prolonged exposure is associated with a reduction in the number of cells having intact acid phosphatase-positive lysosomes. A careful follow up of these subjects during the next year allowed the division of these subjects into a group of healthy not hypertensive subjects and a group of subjects with mild essential systemic hypertension detected in an early phase. The next six years of repeated controls (arterial blood pressure measurement, clinical examination, routine chest X-ray pictures, standard 12 leads ECG and M-mode echocardiography) allowed a further division of the hypertensive subjects into two groups of patients: one correctly treated and the other treated incorrectly or not at all. In the 26 patients of the first group (14 with chronic active hepatitis and 12 with active cirrhosis), their serum was put in contact with a suspension of hepatocytes in a culture of liver from a 5.5 month-old human embryo, in presence of rabbit complement. The hepatocytic lysis was evaluated by the test of microcytotoxicity (MCT), considered positive at values above 60%. Ten healthy subjects and 8 patients with atrophic gastritis were used as controls. In 12 other patients with CALD (2nd group), the authors performed the test of inhibition of the macrophage migration (IMM) and that of lymphoblastic transformation (LT) in presence of an autologous liver antigen, obtained by liver biotic puncture. In the first group, the MCT test was positive in 57.6% of the patients, in correlation with a particular biomorphologic aggressivity and marked immune disturbances, while in controls it was negative in 17 of the 18 cases. In the second group, the extract of autologous hepatocytes induced a marked inhibition of the macrophage migration and a significantly increased lymphoblastic transformation. It is concluded to the existence, in the patients with CALD, of cytotoxic hepatocytic antibodies, as well as to a stimulation of the macrophagic-lymphocytic function in presence of the autologous liver antigen. Among pathogenetic mechanisms which could explain this phenomenon we studied, in some metabolic and endocrine diseases, the possible role of zinc (Zn) in correlation with lipid peroxidation (oxygen radicals). In the patients with Zn depletion (14 cases) the production of malondialdehyde (MDA) (marker for lipid peroxidation) was higher than in controls (10 subjects). Zincemia 57.62 vs 100 micrograms/dl, p less than 0.001, MDA 1.24 vs 0.7 nM/l, p less than 0.01. The clinical tests used were the gustative test for ZnSO4 1% and a clinical severity score. Between Zn and MDA levels a strong correlation was found in hypercortisolism cases, \"r\" = -0.82, p less than 0.01. After Zn therapy the MDA levels lowered by 23.55%, and the clinical score and gustative test improved significantly: chi 2 = 16.98, p less than 0.01 (for gustative test), chi 2 = 31.84, p less than 0.001 (for clinical score). These results suggest that the clinical features could be attributed both to Zn depletion and to oxygen radicals excess. However, only in the cases with hypercortisolemia (endogenous and exogenous) does the Zn-peroxide relationship seem significant.
