The paper focuses attention on: (a) the role in the pathogenesis of unstable angina pectoris, severe arrhythmias and sudden cardiac death, of repeated cycles of formation, disintegration and peripheral embolization of intramural thrombi or of thrombi developed on preexisting atherosclerotic plaques; (b) a still apparently unsolved problem: which comes first, coronary thrombosis or myocardial infarction? {"title":"The therapy with cyclosporin A in rheumatoid arthritis.","authors":"C Georgescu","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Cyclosporin A (CyA) was administered in 6 cases of classical or definite, seropositive rheumatoid arthritis (RA), stages II or III, resistant to the antiinflammatory or nonsteroid therapy, or dependent on high Prednisone doses. An intramuscular daily dose of 1.5 g CyA for a three-week period had good clinical and biologic results. The first signs of improvement, occurring after 14 days of therapy, were manifested by a reduction of the articular inflammation, increase in amplitude of the articular mobility, decrease of the morning stiffness and lowering of the ESR rate. {"title":"The enzymatic deficiencies in lymphocytes of smokers.","authors":"P Moszczyński, S Slowinski, J Lisiewicz","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In the peripheral blood lymphocytes of 85 men aged 18 to 42 years having smoked cigarettes for 2 to 25 years, the activity of various intracellular enzymes was determined using the semiquantitative histochemical methods. The lymphocytes of people having smoked for less than 10 years were characterized by increased acid phosphatase (AP), leucine aminopeptidase (LAP) and lactic dehydrogenase (LDH) activities whereas that of N-acetyl-beta-D-glucosaminidase (NAG) were lowered. In subjects having smoked for more than 10 years lower acid phosphatase, beta-glucuronidase, N-acetyl-beta-D-glucosaminidase and lactic dehydrogenase activities have been observed. When fever appeared, she was treated with antibiotics, but her condition further deteriorated. On laparotomy polycystic liver disease was diagnosed and four liters of pus (due to peptostreptococcal infection) was evacuated from one of the cysts, which was also deroofed. The other cysts were excised and the patient recovered. {"title":"Role of thrombosis in the onset, progression and obstructive character of coronary atherosclerotic plaques.","authors":"C Velican, D Velican, C Petrescu","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A study was carried out on the role of successive microthrombi incorporation and intramural thrombi formation in the onset, progression and obstructive character of coronary atherosclerotic plaques. The coronary arterial tree of 836 apparently healthy subjects 1 to 50 years old who died of accidental causes was investigated. The onset of coronary atherosclerotic plaques having as precursors thrombi could be detected in 10% of advanced lesions. The left anterior descending artery appeared as the vessel of the coronary arterial tree the most susceptible to plaque development induced by thrombi and to the occurrence of thrombi on preexisting plaques. Owing to an important overlap between \"toxic\" and \"nontoxic\" PDC values, the method has a limited value in the diagnosis of digitalis toxicity. However, in patients with toxicity associated with PDC under 2 ng/ml, it allows identification digitalis sensitivity. 2. A dosage selection based on PDC assessment led to a decrease of digitalis toxicity under 4%. {"title":"The impact of advances in radiologic biliary imaging on the diagnosis of obstructive jaundice. Two series of patients hospitalized at an interval of 5 years in the same clinic were prospectively analyzed. In both series, the results of ultrasonography (US) and percutaneous transhepatic cholangiography (PTC) were compared with the intraoperative findings. The performances of US in the assessment of ductal dilatation and choledocholithiasis significantly improved during this period. The fine-needle biopsies taken from the solid masses in liver or pancreatic parenchyma under US guidance in the second series of patients contributed to the greater proportion of correct preoperative diagnoses in this group. The number of PTC investigations increased in the second group (p less than 0.001) given the clinicians' confidence in the safety and easy performance of this method. Using both methods, the etiology of biliary obstruction was correctly diagnosed preoperatively in 85.2 per cent of the second series of patients, significantly more frequently than in the first series of patients (62.7 per cent) (p less than 0.05). {"title":"Pacemaker therapy in a group of 132 patients with sick sinus syndrome--indications and results.","authors":"M Vintilă, R Făgărăşanu, R Luca","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Of the 132 patients with sick sinus syndrome (SSS) studied, 62 (47.8%) required pacemaker implantation thus showing the severity of the arrhythmic syndrome. The pacing requirement varied according to the SSS arrhythmic form with a maximum in the tachy-brady syndrome (53.5%) and in major sinus bradyarrhythmias--S--A block and sinus arrest (54.1%). The presence of syncopes (in 32.6% of the patients) represented the main indication for pacing. Very good results (100% efficiency) were obtained in the bradycardic forms. In the tachy-brady syndrome electrostimulation solved completely the bradycardic component and was able, alone, to suppress the tachyarrhythmic episodes in 18.4% of the patients. By associating pacing with antiarrhythmic drugs favourable results were obtained in an additional proportion of 65.8% of the patients. Transesophageal stimulation was always effective in inhibiting the generator, and it was used when chest-wall stimulation was ineffective.
