{"title":"Perpetual atrial arrhythmias causing a prolonged returning cycle as an expression of cardiac resonant oscillation.","authors":"K Izumi, J Izumi","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A theoretical method of calculating ventricular myocardial voltage and time constant from the findings of 40 portions in electrocardiographic (ECG) tracings of 25 cases, in which atrial premature beats with full returning cycle can be observed, is presented. An resonant electric circuit of an alternating current series element containing resistance (R) of 500 ohms, inductance (L), and capacitance (C) was assumed as a model in this study. The steady-state voltage (Vm) was calculated in terms of the real part of the complex expression L(d2q/dt2) + 500(dq/dt) + (1/C)q = Vmej omega t, where L = 250/ zeta omega n, 1/C = omega n2 L, and Vm = A(zero) omega n2 Lx 100. The effective voltage derived ranged from 0.77 to 0.88 (mean, 0.82) V. The mean value approximated the mean voltage stimulation threshould of the human right ventricle. The time constant RC ranged from 73 to 135 ms. This is the first paper to establish that this method is clinically applicable. The dynamical system regulating the heart rhythm was described as a finite-dimensional matrix representation, which possesses diagonal elements, i.e. eigenvalues, of I/4 square root of 2 pi and 1/2.</p>","PeriodicalId":76124,"journal":{"name":"Materia medica Polona. {"title":"The 5-HT3 blockers in the treatment of the primary fibromyalgia syndrome: a 10-day open study with Tropisetron at a low dose.","authors":"W Samborski, T Stratz, J K Lacki, K Klama, P Mennet, W Müller","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A 10-day open trial with Tropisetron, the selective serotonin type 3 receptor antagonist, in a dose of 5 mg daily was undertaken in 19 patients with the primary fibromyalgia syndrome (FB). Before and after the therapy, pain was assessed using a visual analogue scale and a pain score. All the patients were asked about the presence and intensity of vegetative and functional symptoms. Moreover, the tenderness at the 24 typical tender points at baseline and at the end of the therapy was evaluated. A statistically significant improvement of all examined parameters was observed after the therapy. The efficacy of Tropisetron in a dose of 5 mg daily was comparable with the results obtained in a previously reported trial with a dose of 10 or 15 mg per day, but the frequency of gastric troubles decreased (21% vs. 60%). {"title":"Acute hepatic porphyrias. Detection, prophylaxis and treatment.","authors":"E Kostrzewska, A Gregor","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Between 1962 and 1944, 300 families with acute hepatic porphyrias were seen at the Porphyria Centre in Warsaw. Among members of these families, 443 overt and 707 latent cases were diagnosed, and 211 persons had intermediate values of enzyme activity without clinical symptoms and biochemical changes in urine and stool. Three dual porphyrias were detected. Five children with acute symptoms were also seen. The patients and their relatives were registered and routine prophylactic measures were introduced. The treatment of the attacks was based on glucose and hemearginate infusions, a high carbohydrate diet and maintenance of body homeostasis. {"title":"The natural course of acute gallstone pancreatitis.","authors":"E Małecka-Panas, A Juszyński, E Wilamski","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The aim of the study was to verify the hypothesis of complete recovery after acute gallstone pancreatitis (a.g.p) and to correlate findings of different diagnostic techniques. Thirty patients, aged 23-82 (mean 53.4 +/- 16.7), underwent a secretin-cerulein test (SCT), ultrasound (US) and computed tomography (CT) of the pancreas 6-12 months and 3-5 years after an attack of a.g.p. Exocrine pancreatic function impairment in SCT was found in 19 (63.3%) patients 6-12 months after a.g.p and in 9 (30%) patients 3-5 years after a.g.p. Imaging techniques revealed pancreatic structure abnormalities (pancreas enlargement, pseudocysts, Wirsung duct dilatation or calcifications) in 33.3% patients (US) and in 56.6% patients (CT) 6-12 months after a.g.p and in 13.3% (both US and CT) in patients 3-5 years after a.g.p. Nevertheless, the combination of recurrent attacks of abdominal pain, severe exocrine pancreatic function impairment requiring enzyme supplementation and pancreatic calcifications in imaging techniques 3-5 years after a.g.p. have been observed only in 4 (13.3%) patients. Those were the patients after multiple attacks of clinically severe, necrotic a.g.p., in which cholecystectomy has not been performed. We conclude that in most cases after acute gallstone pancreatitis pancreatic structure and function gradually return to normal. Nevertheless, patients after multiple episodes of clinically severe a.g.p., with gallbladder in situ should undergo a prospective pancreatic function and structure evaluation in order to early recognize and treat the revealed changes.</p>","PeriodicalId":76124,"journal":{"name":"Materia medica Polona.
