{"title":"[Brain stem seizures (author's transl)].","authors":"F L Glötzner","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Paroxysmal symptoms of frequent recurrence and short duration occurring mostly unilaterally and without loss of consciousness have been described under a vast variety of headings. Brain stem origin of these symptoms was presumed. Electroencephalographic recordings usually did not show any paroxysmal discharges. 328 patients were found in the available literature including 9 patients of ours. The seizures were classified by their appearance. Tonic or dystonic, sensory, algetic, ataxic and akinetic-atonic fits were distinguished. The dystonic variety includes the \"paroxysmal kinesiogenic choreoathetosis\". The \"paroxysmal dysarthria and ataxia\" was subsumed under the ataxic type. By etiology, seizures were grouped into the cryptogenic and the symptomatic type. The symptomatic variety is frequently caused by multiple sclerosis, and rarely by tumours of the basal ganglia or by vascular disorders. Cranial computertomography showed subcortical lesions in three out of seven patients. In one case cerebral atrophy was found. All types of seizures respond very well to antiepileptic drugs. Particularly remarkable is the great interest which has been evinced to date on the fate of King Ludwig II of Bavaria. On the other hand, the tragedy of the Belgian Princess Charlotte, who was Empress of Mexico for a few years by the side of her husband, Maximilian of Austria, is hardly known. I could not find anything about her in psychiatric literature, but there are numerous biographical hints in books on history dealing with the dramatic episode of the short-lived Mexican empire. I found some of the most essential information on the personality and psychosis of Empress Charlotte in the book written by Countess H. de Reinach-Foussemagne. Panarteriitis nodosa was diagnosed in 4 cases, Lupus erythematodes in 2 cases. With 5 patients immunovasculitis with cerebral affection was found, which was, however, not to be classified in detail. More or less characteristic features are to be expected in the angiogram; they might harden the suspicion of collagen disease, although they are not likely to prove its diagnosis. An interpretation of the radiological findings should--in addition to the morphology--primarily take into account the distribution type of the vessel wall lesions. Clinically as well as by means of angiography it is difficult to differentiate between collagen disease and cerebral arteriitis of different aetiology; this applies particularly to the alterations in cases of embolic circumscribed encephalitis in sepsis lenta. Other important subgroups are discussed regarding both variation and overlapping. An increased latency of the initial component N1 and a reduced voltage in the beginning of the potential recorded on the side of the thalamic lesion are the most frequent findings in patients with thalamic syndromes. Most dramatic changes of the evoked response occur in cases with lesions of the VPL nucleus and with proprioceptive sensory disturbances.
