[Antiphospholipid antibodies syndrome in dermatology in Dakar: 11 case report]. [达喀尔皮肤病抗磷脂抗体综合征:11例报告]。

All patients were diagnosed according to the criteria of the international consensus statement in 1999 on the antiphospholipid antibodies.</p><p><strong>Results: </strong>Eleven cases (all female) were collected with a median age of 28 years. A systemic lupus was associated in 7 cases, a scleroderma in 1 case and mix connectivity in 1 case. The dermatological manifestations found in all patients were necrosis of the extremities (4 cases), purpuric lesions (4), cutaneous ulcers (3), acrocyanosis (2), livedo (1) and subungual splinter hemorrhage (1). The obstetrical incidents were noted in 82% of cases; there were precocious and repeated abortions (in 5 cases), foetal loss (in 4 patients), precocious delivery (in 2 cases), lateness of growth in uterus (in 1 case). It was an arterial thrombosis in 2 cases. The mean level of anticardiolipin antibodies was 60 UGPL and a dissociation of the syphilitic serology was present in 9 patients. A severe thrombopenia in 1 case was observed. The evolution was favorable in 7 patients. In other cases, we noted foetal loss in 2 cases and distal necrosis relapse in 2 cases.</p><p><strong>Conclusion: </strong>Cutaneous manifestations are diverse. It is especially the fact of late diagnosis of diabetes, at a time where complications are already present. The aims were to estimate the frequency of microangiopathy observed at the time of diabetes diagnosis within diabetic patients received in Conakry University Hospital and to describe the clinical characteristics of patients who had this complication.</p><p><strong>Patients and method: </strong>We prospectively examined 116 patients [76 men (65.5%) and 40 women (34.5%); 11% type 1 diabetes and 89% type 2 diabetes] consecutively received and for whom known evolution of diabetes was lower or equal to three-months. All patients were interrogated (in search of cardiovascular risk factors), had a systematic check for retinopathy (exam done by ophthalmologist) or nephropathy (by measure of creatinine, urea and proteinuria).</p><p><strong>Results: </strong>Diabetic retinopathy was present in 29 cases (24.8%) and diabetic nephropathy in 9 cases (7.8%), one at the stage of chronic renal failure. The patients who had microangiopathy were older than the others (p = 0.003) and have more frequently Type 2 diabetes (p = 0.005). However, glycaemia level and cardiovascular risk factors (nicotine addiction, arterial high blood pressure, obesity and sedentary) were not statistically different between both groups.</p><p><strong>Conclusion: </strong>Diabetic microangiopathy is frequent at the time of diabetes diagnosis in Guinea. In Senegal, a few cases of PH have been previously reported in patients with systemic sclerosis.</p><p><strong>Case reports: </strong>We report two cases of patients with PH that revealed autoimmune diseases (Sharp' syndrome and Sjögren syndrome). Epidemiological, clinical, evolutive and laboratory data were analyzed. Evolution of disease was favourable for one patient and fatal for the other.</p><p><strong>Conclusion: </strong>These two cases show necessity of early diagnosis of systemic diseases in our countries. PH should be screened in each patient with autoimmune disease before installation of irreversible pulmonary arterial lesions that respond to treatment. We report two cases treated between September 1997 and August 2002 at the Surgical Emergency Department of Le Dantec Hospital. The first observation was related to a 26 year old lady. On September 27, 1997, she fell down from a tree and received all the weight of the body on her two arms. She was referred because of anterior chest pains, increasing with respiration associated with injuries of the 2 wrists. On conventional X-ray, a type II manubriosternal joint dislocation (anterior dislocation of the sternal body with respect to the manubrium) was diagnosed. The mechanism of the dislocation was indirect: flexion-compression of the sternum caused by a hyperflexion of the dorsal spine when the patient touched the ground. There was also a Pouteaux-Colles fracture of the 2 wrists. The dislocation was surgically treated: open reduction followed by manubriosternal stabilization using wires. The 2 wrists were treated by Kapandji procedure. At the 21st postoperative day, a traumatic rupture of the wires required a 2nd internal fixation of the sternum by wires. After 9 years, the patient is without complaint and the chest X-ray is normal. The second observation was that of a 19 year old young woman, referred on August 15, 2002 after a frontal crash with a car while crossing the road. She fell down on her back. She was complaining from severe posterior headaches with a normal Glasgow Scale (15), anterior chest and right hand pains. Radiological examinations showed a fracture of the occipital bone without embarrure and a type I manubriosternal joint dislocation (posterior displacement of the sternal body in relation to the manubrium) which mechanism was direct: direct shock against the sternum. There were also an isolated fracture of 4 right sided ribs and a fracture without displacement of the 3 last bones of the right metacarpus. An orthopaedic treatment was carried out for the lesions of the right hand and an abstention-monitoring suggested for the occipital fracture. The manubriosternal joint dislocation was surgically reduced and stabilized by using a braided polyester thread number 2. After 4 years, the patient is asymptomatic and the manobriosternal joint is stable. The difficulty of culture and long-term maintenance of sufficient amount of normal endothelial cells has proven to be the limitation for the understanding of the biological function of the endothelium. Therefore, the aim of this study was to culture and characterize the porcine coronary endothelial cells.</p><p><strong>Material and methods: </strong>The endothelial cells were isolated by collagenase treatment and cultured in culture dishes coated with collagen, prepared from rat tail, containing medium RPMI1640/M199 and 15% fetal calf serum supplemented with antibiotics and fungizon. The cells were maintained to grown at 37 degrees C. The medium was changed one day after and then every two day. The cells were incubated with Dil-labeled-acetylated-LDL for determination of acetylated-LDL uptake. Confluence cultures of cells were examined by phase-contrast and confocal microscopy.</p><p><strong>Results: </strong>After a day of culture, the endothelial cells adhere to the collagen and began to grow. While multiplying themselves, they colonize little by little the body of the surface of culture to form to confluence a monolayer of flat cells relatively homogenous. To confluence, the proliferation of the endothelial cells is inhibited by the contact and the cells present a polygonal aspect. Our results show that all the cultivated cells were strongly positive for acetylated-LDL markers. These hyperthyroidism represent 39% of the thyroid affections, with a female prevalence (97%), 32 years of average age, the urban source of the patients (90% of the cases) and the importance of self medication (73%). They stress the importance of sonography, allowing a classification in diffuse goiter (44%) and multi nodular (56%). They insist on a good preparation of the patient before the intervention and on the interest of the sub total thyroïdectomy. Resources are mainly obtained from the sale of services. The aim of this study is to analyse the operating cost of a community health post and to propose a relevant tariffing that would assure sustainable activities.</p><p><strong>Methods: </strong>We used the method of complete costs. It comes out from our study that the total cost is 20 870 920F.</p><p><strong>Results: </strong>Wages represent 70% of total expenses, operating costs represent 27% and 4% are investment. The public funding represents a value of 12 257 325F (60% of the total) in which 88% correspond to expenses induced by civil servant wages. We are reporting a case characterized among other difficulties by its diagnostic difficulties.</p><p><strong>Observation: </strong>It is about a 29 years old black Senegalese woman patient, without particular antecedents, which presented a systematic chronic syndrome composed of a pharyngitis, a polyarthritis and general symptoms (fever, chills, sweats, change of the general state), a cutaneous eruption, a polyadenopathy, a hepatosplenomegaly. The biological analyses showed among others, an inflammatory syndrome (VS at 115 mm in the 1st hour, CRP at 100 mg/L, WBC at 10,400/mm3 with neutrophilia), a hyperferritinemia in 643 ng/l with collapse of the glycosylated ferritin at 13% (N between 60 in 80%). After elimination of any autoimmune or neoplastic suppurative infectious pathology in the decline of a check up as exhaustive as possible, the diagnosis of a Still disease in adult had been retained. Their was improvement under the combination of prednisone and methotrexate.</p><p><strong>Conclusion: </strong>Although it is exceptional in black African, this pathology shall be however part of the differential diagnoses of any unexplained systematic sign. The aim was to see to what extent echo guided pericardiocentesis could contribute in severe cases.</p><p><strong>Methods: </strong>All patients with abundant pericardial effusion diagnosed by echocardiography with or without compression were considered in a prospective study. They experienced a pericardiocentesis. Some patients did undergo an additional surgical biopsy. The liquid was preliminarily submitted to chemical, bacteriological, cytological analysis. Tissues were observed through an anatomic pathology in biopsy. Heart tuberculosis was diagnosed through histology for following therapeutic testing.</p><p><strong>Results: </strong>Thirty consecutive patients aged 37 in average were included. The sex ratio was 3.75. Prior to draining off, 3 neoplasies (2 of lung bronchitis, 1 of cervix ), 1 leukaemia, 2 extra pericardium tuberculosis and 1 amoebic abscess were diagnosed . Clinical occurrence: the most frequent case, besides the tampon (10 cases, i.e. 30%), was isolated pericardial effusion associated or not with an alteration of the global state (13 cases, i.e. 4%). The average quantity of liquid extracted was 600c c which provoked a kind of haemorrhage in 18 cases, i.e. 60%. Tuberculosis was the major aetiology: 15 cases e i 50% out of which 4 were HIV positive. Three additional surgical biopsies were carried out. Seven patients died. The authors study the clinical, therapeutic and evolutionary aspects of the postpartum depression among eleven Senegalese women.</p><p><strong>Material and method: </strong>They examined the medical reports and hospital registers of eleven patients belonging to a sample of 52 women presenting psychic disorders of the puerperality. The diagnosis was made by using the International Classification of the Diseases.</p><p><strong>Results: </strong>The symptoms of depression were irritability, bad concentration, multiple somatic complaints, of insomnia of drowsiness, loss of the usual interests. All the patients profited from psychotherapy and have received chemotherapy with antidepressant treatment associated tranquillizing in six cases. They improved from 29 to 43 days after their admission.</p><p><strong>Conclusion: </strong>The pregnancy and the childbirth, the parity, personal history of the patient, their social environment and the cultural representations of maternity are factors of depression of the postpartum.
