{"title":"Resveratrol Delays Diabetic Cardiomyopathy Fibrosis by Regulating Mitochondrial Autophagy.","authors":"Liqun Yang, Zhe Gao, Hang Zhao, Zhimei Zhang, Guangyao Song","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To explore whether resveratrol can postpone the fibrosis associated with diabetic cardiomyopathy (DCM) by modulating the mitochondrial autophagy response through the AMPK/SIRT1-mediated IRE1α/PINK signaling pathway.</p><p><strong>Methods: </strong>A DCM mouse model was established using a high-sugar high-fat diet and streptozotocin. Resveratrol was administered to a subset of the DCM mouse models for comparison. Echocardiography, Masson staining, TNUEL assay, and transmission electron microscopy were employed to evaluate the cardiac status, myocardial fibrosis, myocardial cell apoptosis, and morphological changes of myocardial cells and their internal mitochondria in each group of mice. Western blot staining was performed on myocardial tissues to assess the protein expression levels of p-AMPK, SIRT1, SIRT3, p22, GP91, p-IRE1α, XBP1s PINK, Parkin, LC3I, and Beclin. Mouse myocardial cells were cultured in vitro and intervened with a high-sugar high-fat diet, resveratrol, and GSK690693 (an AMPK inhibitor) to observe the protein expression levels of p-AMPK, p22, XBP1s, and PINK in mouse myocardial cells in each group.</p><p><strong>Results: </strong>Results from echocardiography, Masson staining, TNUEL assay, and transmission electron microscopy showed that resveratrol administration alleviated cardiac damage, myocardial fibrosis, myocardial cell apoptosis, and mitochondrial autophagy in DCM mice. Resveratrol administration promoted the expression of phosphorylated AMP-activated protein kinase (p-AMPK), sirtuin 1 (SIRT1), and sirtuin 3 (SIRT3) in the myocardial tissue of mice, while lowering the elevated protein expression levels of p22 subunit (p22), guanine nucleotide-binding protein q polypeptide 1 (GP91), phosphorylated inositol-requiring enzyme 1 alpha (p-IRE1α), X-box binding protein 1 spliced form (XBP1s), PTEN-induced putative kinase 1 (PINK), Parkin, microtubule-associated proteins light chain 3 isoform I (LC3I), and Beclin (Bcl-2 interacting protein) caused by DCM. {"title":"Diagnostic Value of Transabdominal Ultrasonography in Gastrointestinal Malignant Tumors.","authors":"Jing-Yun Dong, Yan-Bing Gao, Li-Wei Tang, Jing Zheng","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To explore the application value of transabdominal ultrasonography in the diagnosis of gastrointestinal malignant tumors.</p><p><strong>Methods: </strong>This study retrospectively analyzed the transabdominal ultrasound imaging data of 284 patients with gastrointestinal tumors admitted to our hospital from April 2019 to March 2022 and assessed the accuracy of transabdominal ultrasound in diagnosing different types of gastrointestinal tumor diseases. The diagnostic accuracy of transabdominal ultrasonography for TNM staging of gastrointestinal malignancies was calculated.</p><p><strong>Results: </strong>The sensitivity and specificity of transabdominal ultrasonography in the diagnosis of gastric cancer were (82.40% and 83.72%, respectively), colon cancer (77.78% and 88.35%, respectively), gastric stromal tumor (95.45% and 93.65%, respectively), gastric lymphoma (72.22% and 94.66%, respectively), colorectal lymphoma (80.00% and 95.42%, respectively), gastric mucosal hypertrophy (85.71% and 96.69%, respectively), and pyloric hypertrophy (92.59% and 97.79%, respectively). Among the 284 patients included, 152 patients had malignant tumors, including 34 patients with stage I, 30 patients with stage II, 51 patients with stage III, and 37 patients with stage IV. The accuracy of transabdominal ultrasonography for TNM staging of gastrointestinal malignancies was 85.53% (130/152).</p><p><strong>Conclusion: </strong>Transabdominal ultrasonography shows promise as a diagnostic tool for gastrointestinal malignant tumors; however, it is recommended to be used in conjunction with other detection methods such as fibrous gastrointestinal tract examination to minimize the risk of missed diagnoses and misdiagnoses. The study highlights the potential of transabdominal ultrasonography as a non-invasive and accessible diagnostic method for gastrointestinal malignancies. {"title":"Clinical Efficacy of Sun's Abdominal Acupuncture Combined with CBTI in Treating Chronic Insomnia with Anxiety.","authors":"He Zhiyuan, Qin Lina","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>This study aims to evaluate the clinical efficacy of Sun's abdominal acupuncture combined with cognitive behavioral therapy (CBTI) in the treatment of chronic insomnia with anxiety.</p><p><strong>Methods: </strong>A total of 86 patients diagnosed with chronic insomnia and anxiety were selected from October 2022 to September 2023. Patients were divided into two groups: the cognitive therapy group (treated with CBTI alone) and the combined therapy group (treated with Sun's abdominal acupuncture combined with CBTI). Various parameters including insomnia severity (measured by ISI score), sleep quality (assessed by PSQI score), polysomnography (PSG) results (including sleep duration, latency, wake time, and efficiency), cognitive function (MMSE score), anxiety symptoms (HAM-A score), and insomnia efficacy and anxiety efficacy were compared between the two groups.</p><p><strong>Results: </strong>The combined therapy group exhibited significantly lower ISI and PSQI scores compared to the cognitive therapy group (P < .05). PSG results showed increased sleep duration, decreased latency and wake time, and higher sleep efficiency in the combined therapy group (P < .05). Moreover, MMSE and HAM-A scores were significantly lower in the combined therapy group (P < .05). The total effective rate for both insomnia and anxiety efficacy were higher in the combined therapy group compared to the cognitive therapy group (P < .05).</p><p><strong>Conclusions: </strong>Sun's abdominal acupuncture combined with CBTI demonstrates superior clinical efficacy in the management of chronic insomnia with anxiety. This combined approach not only reduces insomnia severity and improves sleep quality but also enhances cognitive function and reduces anxiety symptoms. {"title":"Clinical Efficacy and Adverse Effects of Bevacizumab in Combination with Chemotherapy for Metastatic Colorectal Cancer.","authors":"Ying Wei, Rui Jin, Hui Xue, Ruixuan Zhou, Zhijuan Chen","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>This study aims to investigate the efficacy of bevacizumab (BEV) in combination with chemotherapy for metastatic colorectal cancer (mCRC).</p><p><strong>Methods: </strong>A cohort of 121 patients diagnosed with mCRC and admitted to our hospital from May 2018 to October 2019 were selected for the study. The control group, comprising 64 patients, received chemotherapy alone, while the research group, consisting of 57 patients, underwent a combination of BEV and chemotherapy. Comparative analyses included an assessment of clinical outcomes, monitoring of tumor markers including Carcinoembryonic Antigen (CEA), Cancer Antigen 74-2 (CA74-2), and Carbohydrate Antigen 19-9 (CA19-9) before and after treatment, and a count of adverse effects during the treatment phase. A 3-year post-discharge follow-up was conducted to compare the survival prognosis between the two groups.</p><p><strong>Results: </strong>The research group exhibited a significantly higher objective response rate (ORR) and clinical benefit rate (CBR) compared to the control group (P < .05). Furthermore, CEA, CA74-2, and CA19-9 post-treatment levels were markedly lower in the research group (P < .05). No notable difference in the incidence of adverse reactions was observed between the two groups (P > .05). Importantly, the 3-year overall survival prognosis was superior in the research group (P < .05). Within the research group, patients treated with BEV combined with the XELIRI regimen chemotherapy demonstrated a higher CBR rate (P < .05).</p><p><strong>Conclusions: </strong>The combination of BEV and chemotherapy proves to be highly effective in treating mCRC, significantly enhancing the prognostic survival cycle of patients. {"title":"The Effects of Health Education and the Sunrise Model of Nursing Care on Blood Pressure Control and Psychological Status in Elderly Patients with Hypertension.","authors":"Wenna Sha, Li Ning, Huafang Li, Qiuyan Fu, Meihong Chen","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>Our aim was to investigate the effect of health education and a positive attitude nursing model on blood pressure (BP) control and psychological status in elderly patients with hypertension (HTN).</p><p><strong>Methods: </strong>A total of 80 elderly patients with HTN in the Affiliated Hangzhou First People's Hospital in China were selected as the study participants. They were randomly divided into a control group and an observation group, with 40 patients in each group. The control group received the routine nursing intervention, while the observation group received health education and the positive attitude nursing intervention based on the nursing care in the control group. The BP control effect, changes in systolic and diastolic BP, psychological status and nursing satisfaction were compared in the 2 groups.</p><p><strong>Results: </strong>The BP control rate in the control group was 77.5% and was 95.0% in the observation group, which was significantly better (P < .05). Before the intervention, there was no significant difference in average systolic and diastolic BP between the 2 groups (P > .05); after the intervention, average systolic and diastolic BP in the observation group were significantly lower than in the control group (P < .05). Before the intervention, there was no significant difference in Self-rating Anxiety Scale (SAS) and Self-rating Anxiety Scale (SDS) scores between the 2 groups (P > .05). After the intervention, the SAS and SDS scores in the observation group were significantly better than those in the control group (P < .05). Among them, 57 patients received MEWS-based graded nursing and were regarded as the research group (RG), while the other 46 patients received routine nursing and were regarded as the control group (CG). The Symptom Check List-90 (SCL-90), the Visual Analogue Scale (VAS), and the Post-traumatic Stress Disorder (PTSD) Checklist-Civilian version (PCL-C) scoring surveys were administered before and after care, respectively. Nursing satisfaction was investigated when patients were discharged from the hospital. Then, patient outcomes were followed up for one year to evaluate patients' QoL by the Generic Quality of Life Inventory-74 (GQOL-74).</p><p><strong>Results: </strong>SCL-90, VAS, and PCL-C were lower, and satisfaction with care was higher after RG treatment compared to CG (P < .05). The incidence of adverse events during treatment was lower in RG than in CG (P < .05). {"title":"Clinical Efficacy of Adhesion Release Under Brachial Plexus Block Plus Silver Needle Warm Acupuncture on Frozen Shoulder and Recovery of Limb Function.","authors":"Haiyang Wang, Daohua Fan, Qiang Lan, Huaqu Gong","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>This study aims to evaluate the clinical efficacy and safety of combining adhesion release under brachial plexus block with silver needle warm acupuncture for the treatment of frozen shoulder, compared to adhesion release under brachial plexus block alone.</p><p><strong>Methods: </strong>A total of 72 patients with frozen shoulder were randomly assigned to receive either adhesion release under brachial plexus block (control group) or adhesion release under brachial plexus block plus silver needle warm acupuncture (treatment group). Outcome measures included changes in shoulder pain scores assessed using the Visual Analog Scale (VAS), shoulder mobility scores evaluated using the Constant-Murley Score, shoulder function scores measured using the Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaire, and overall clinical outcomes based on a comprehensive evaluation. Both groups received functional exercise 2 days after the operation, and the treatment duration was 3 weeks.</p><p><strong>Results: </strong>The treatment group exhibited a mean reduction in shoulder pain scores of 3.2 points on the Short-form McGill pain scores, while the control group showed a reduction of 1.5 points. Shoulder mobility scores, assessed by the Constant-Murley Score, increased by an average of 18.6 points in the treatment group, compared to 9.2 points in the control group. Moreover, the treatment group achieved better shoulder function scores on the Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaire, with an average improvement of 25.4 points, compared to 13.8 points in the control group.</p><p><strong>Conclusion: </strong>This study provides valuable insights into the clinical efficacy of adhesion release under brachial plexus block plus silver needle warm acupuncture for frozen shoulder, there are still areas that warrant further investigation. {"title":"Effects of Continuous Quality Improvement on Complication Rate and Treatment Outcome of Extremely Premature Infants.","authors":"Yang Chen, Tingting Cheng, Huaping Zhu, Shiwen Xia","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Background: </strong>Extremely premature infants (EPIs) are those less than 32 weeks of gestational age. Preterm birth is the leading cause of neonatal death and poor prognosis, accounting for 25% of neonatal deaths, with extremely premature births accounting for 50% of all premature deaths. Continuous quality improvement (CQI) improves patient outcomes by changing and optimizing clinical practice including increasing participation of neonatologists in prenatal consultation, maintenance of normal body temperature in preterm infants, early use of pulmonary surfactant, reduction of mechanical ventilation time and intensive breastfeeding to reduce clinically avoidable adverse events.</p><p><strong>Objective: </strong>The risk of death and disability is high for very preterm infants, with a mortality rate of 30-50% and a risk of at least 20-50% for survivors. This study aimed to investigate the effect of CQI on the incidence of complications and treatment outcomes in very preterm infants.</p><p><strong>Design: </strong>This was a retrospective study.</p><p><strong>Setting: </strong>This study was conducted in the Maternal and Child Health Hospital of Hubei Province.</p><p><strong>Participants: </strong>A total of 140 EPIs born in our hospital and transferred to the neonatal intensive care unit between August 1, 2020, and July 31, 2022, were enrolled. The EPIs were divided into two groups: before improvement (n=79, 56.4%) and after improvement (n=61, 43.6%) according to the week of birth, and the gestational age ranged from 26 weeks to 26 weeks 6 days into the 26 weeks group.</p><p><strong>Interventions: </strong>From August 2021, the hospital implemented the CQI method, which included neonatologists' participation in consultations before birth, the care of a professionally trained resuscitation team after birth, and the introduction of transport heating tanks and ventilators during transport.</p><p><strong>Primary outcome measures: </strong>(1) Apgar score (2) body weight (3) duration of invasive ventilation (4) length of stay (5) treatment expense (6) incidence of complications and (7) survival rate of EPIs.</p><p><strong>Results: </strong>The application of CQI methods resulted in significant improvements in body weight (1305 g vs 1404 g) and duration of invasive ventilation (4.64 d vs 7.40 d) in EPIs (P = .036 and P = .040), reduced the time of invasive mechanical ventilation decreased significantly, from 7.4 days to 4.64 days (P < .01), increased the median temperature of newborn infants (36.2°C vs 35.7°C) (P = 0), increased the proportion of newborn infants with a temperature greater than 36°C (67.2% vs 35.4%) (P < .001), reduced the incidence of complications in EPIs (32.79% vs 45.57%) (P < .05).</p><p><strong>Conclusion: </strong>The application of the CQI approach significantly increases the body temperature, improves the incidence of complications of EPIs, and is conducive to the survival of EPIs. {"title":"Impact of FMEA-Based Nursing on Postoperative Condition of Appendicitis Patients.","authors":"Yang Yang","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Backgrounds: </strong>Appendicitis (AP) is a prevalent and sudden disease, and although surgery can be effective in treating it, post-surgical care is of great importance in improving the patient's recovery.</p><p><strong>Objective: </strong>To analyze the impact of Failure Mode and Effect Analysis (FMEA)-based nursing on patients with AP after surgery.</p><p><strong>Methods: </strong>A total of 104 patients with AP who received treatment in Fuyang Shi Hospital of TCM from December 2021 to February 2023 were enrolled and randomly divided into a control group (n = 52) who received routine nursing and a research group (n = 52) who receive FMEA-based nursing. The time to recovery of bowel sounds, time to defecation by anus, time to anal exhaust, and length of stay were statistically analyzed in both groups after surgery. Pain, sleep, and psychological status of patients before and after surgery were assessed using the visual analog scale (VAS), Pittsburgh Sleep Quality Index (PSQI), and self-rating anxiety scale/self-rating depression scale (SAS/SDS). Patients' satisfaction with nursing was also investigated, and the incidence of postoperative complications was recorded.</p><p><strong>Results: </strong>After surgery, the research group showed shorter time to recovery of bowel sounds, time to defecation by anus, time to anal exhaust, and length of stay, and lower scores of VAS, PSQI, SAS, and SDS than those of the control group (P < .05). {"title":"Umbilical Cord Blood Betaine in Infants Born Small for Gestational Age.","authors":"Guicun Yang, Nianrong Wang, Hao Liu, Lina Si, Yan Zhao","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Background: </strong>Birth weight is of importance due to its relation to fetal health, and it's also a predictor of the subsequent development of the child.</p><p><strong>Objective: </strong>This study aims to assess whether betaine is associated with poor fetal growth.</p><p><strong>Design: </strong>A case-control study was used in this study.</p><p><strong>Setting: </strong>The study took place at the Chongqing Maternal and Child Health Hospital, in Chong Qing, China.</p><p><strong>Participants: </strong>A total of 141mother-infant pairs were recruited from the Department of Obstetrics of our hospital between June 2021 and December 2021. According to gestational age and birth weight, themother-infant pairs were divided into small-for-gestational-age and appropriate-for-gestational-age groups.</p><p><strong>Primary outcome measures: </strong>Cord plasma concentrations of betaine were measured by high-performance liquid chromatography tandem mass spectrometry. Plasma levels of triglycerides, low-density lipoprotein, high-density lipoprotein and total cholesterol were determined using commercially available assays on an automatic biochemical analyzer (BS-240 VET, Mindray Medical, Shenzhen, China) using reagent from Mindray Medical company.</p><p><strong>Results: </strong>Cord plasma betaine concentrations were higher in small-for-gestational-age relative to appropriate-for-gestational-age newborns, and were not correlated to lipid levels. Adjusting for maternal and neonatal characteristics, birth weight and birth length were negatively correlated with the levels of betaine.
