{"title":"Colorectal cancer screening program implementation: biennial adherence and participation patterns","authors":"","doi":"10.1016/j.rgmxen.2023.08.010","DOIUrl":"10.1016/j.rgmxen.2023.08.010","url":null,"abstract":"<div><h3>Introduction and aim</h3><p>Adherence to colorectal cancer (CRC) screening is essential for the effectiveness of screening programs. Even though organized screening programs can improve the quality of the process and adherence, there are still challenges to overcome. The aim of the present study, in which we implemented a biennial organized screening program for CRC, was to describe adherence and participation patterns.</p></div><div><h3>Materials and methods</h3><p>A longitudinal, descriptive study was conducted, in which a team of trained patient navigators carried out interventions, with reminders via cellphone communication, to follow a cohort of 301 subjects eligible for CRC screening, utilizing a fecal immunochemical test (FIT). All the follow-up subjects received a FIT kit.</p></div><div><h3>Results</h3><p>A total of 747 cellphone calls were made and divided into three interventions. From the initial cohort, 126 subjects completed their biennial screening process through the FIT, indicating a consistent adherence rate of 41.8% to our program. The participation patterns were: 126 consistent participants (41.8%), 160 inconsistent participants (53.2%), and 15 participants that were never contacted (5%).</p></div><div><h3>Conclusions</h3><p>In conclusion, our study underlines the importance of organized screening programs in the early detection of CRC. The aim of the present study was to evaluate the drying stages, storage, and channel conditions of endoscopes through borescope inspection.</p></div><div><h3>Material and methods</h3><p>The personnel responsible for endoscope reprocessing were interviewed. Storage conditions at 10 endoscopy facilities were inspected and an internal examination of the channels and ports of the stored equipment was carried out, utilizing a borescope. A total of 74 stored endoscope channels were evaluated.</p></div><div><h3>Results</h3><p>Only 10% of the facilities inspected utilized transport cases for storage and only 10% had rooms exclusively used for storage. Sixty percent of the facilities did not perform any shelf-life control. The aim of the present study was to identify whether indocyanine green fluorescence angiography (ICGFA) reduced the anastomotic dehiscence rate in colorectal surgery.</p></div><div><h3>Material and methods</h3><p>A retrospective study on patients that underwent colorectal surgery with colonic resection or low anterior resection and primary anastomosis, within the time frame of January 2019 and September 2021, was conducted. The patients were divided into the case group, in which ICGFA was performed for the intraoperative evaluation of blood perfusion at the anastomosis site, and the control group, in which ICGFA was not utilized.</p></div><div><h3>Results</h3><p>A total of 168 medical records were reviewed, resulting in 83 cases and 85 controls. Inadequate perfusion that required changing the surgical site of the anastomosis was identified in 4.8% of the case group (<em>n</em> = 4). A trend toward reducing the leak rate with ICGFA was identified (6% [<em>n</em> = 5] in the cases vs 7.1% in the controls [<em>n</em> = 6] [<em>p</em> = 0.999]). It can affect the entire gastrointestinal tract and its etiology is unknown.</p></div><div><h3>Objective</h3><p>The aim of this consensus was to establish the most relevant aspects related to definitions, diagnosis, follow-up, medical treatment, and surgical treatment of Crohn’s disease in Mexico.</p></div><div><h3>Material and methods</h3><p>Mexican specialists in the areas of gastroenterology and inflammatory bowel disease were summoned. The consensus was divided into five modules, with 69 statements. Applying the Delphi panel method, the pre-meeting questions were sent to the participants, to be edited and weighted. Prevalence worldwide is estimated at 11%. There is little information on the prevalence of the other functional bowel disorders (FBDs). Our aim was to establish the prevalence of IBS and other FBDs according to the Rome IV criteria, in Uruguay.</p></div><div><h3>Materials and methods</h3><p>An observational, population-based prevalence study was conducted. Data were collected through an online questionnaire, utilizing the Rome IV criteria.</p></div><div><h3>Results</h3><p>Of the 1,052 participants (79% women, mean patient age 44 years), 47.2% met the Rome IV diagnostic criteria for at least one of the FBDs analyzed. Functional constipation (FC) was the most frequent, at 18.7% (16.4-21.1), followed by IBS at 17.1% (14.9-19.4) and functional diarrhea (FD) at 15.4% (13.3-17.6). IBS with constipation (IBS-C) was the most frequent IBS subtype (35%) and the IBS with diarrhea (IBS-D) subtype predominated in men. All FBDs were more prevalent in women and youths. Of the survey participants with a FBD, 59% stated that they perceived no worsening of their symptoms related to the COVID-19 pandemic.</p></div><div><h3>Conclusions</h3><p>Ours is the first prevalence study on FBDs conducted on the Uruguayan general population. Half of the participants surveyed presented with a FBD analyzed in the study. FC was the most frequent, followed by IBS and FD. The reliability, comprehensiveness, and quality of said information in Spanish has not been studied. Our aim was to describe the characteristics of the information about AH on YouTube®.</p></div><div><h3>Methods</h3><p>An analytic observational study evaluated videos in Spanish about AH available on YouTube®, describing their general characteristics, viewer engagement, and information sources. Standardized tools were utilized to analyze reliability (DISCERN), comprehensiveness, and overall quality (Global Quality Score [GQS]).</p></div><div><h3>Results</h3><p>One hundred videos were included, 93% of which provided information from healthcare professionals (group 1), and 7% of which reflected patient opinions (group 2). There were differences in the median reliability (DISCERN 4 vs 2, <em>p</em> ≤ 0.05) and comprehensiveness (4 vs 2, <em>p</em> ≤ 0.05) scores between groups, but equal overall quality (GQS 3 vs 2, <em>p</em> = 0.2). Reliability (DISCERN 4; RIC 3–4) and comprehensiveness (4.5; IQR 3–5) were higher in videos by professional organizations, compared with those by independent users, healthcare information websites, and for-profit organizations (DISCERN 3; IQR 2.5–3.5) (<em>p</em> < 0.001). Reliability (DISCERN 2; IQR 1.5–3), comprehensiveness (2; IQR 1.5–2.5), and quality (GQS 2.5; IQR 1.5–3.5) were lower for videos made by for-profit organizations.</p></div><div><h3>Conclusion</h3><p>The majority of videos about AH in Spanish on YouTube® have good reliability, comprehensiveness, and quality. It is more common in infants, in whom the majority of episodes are short-lived and cause no other symptoms or complications, differentiating it from gastroesophageal reflux disease (GERD). The diagnosis and management of GER and GERD continue to be a challenge for the physician. Therefore, the aim of the <em>Asociación Mexicana de Gastroenterología</em> was to adapt international documents to facilitate their adoption by primary care physicians, with the goal of standardizing quality of care and reducing the number of diagnostic tests performed and inappropriate medication use.</p><p>The ADAPTE methodology was followed, and the recommendations were approved utilizing the Delphi strategy. The executive committee carried out the review of the guidelines, position papers, and international reviews that met the <em>a priori</em> quality criteria and possible applicability in a local context. The recommendations were taken from those sources and adapted, after which they were approved by the working group. The consensus consists of 25 statements and their supporting information on the diagnosis and treatment of GER and GERD in infants.
