An expiratory pressure relief technology called C-Flex+ has been developed to improve the comfort of CPAP \u0000 therapy. It provides a 2 mbar lower exhalation pressure and an additional flow-based pressure relief at the beginning \u0000 of exhalation. The following study was conducted to investigate whether C-Flex+ was as effective as CPAP in treating \u0000 OSA, and the patients’ preference. \u0000Methods: 60 newly diagnosed patients with OSA completed this double-blind controlled crossover-study. \u0000 Patients were randomized to one night of C-Flex+ and one of CPAP under full attended polysomnography (PSG). \u0000 A comfort visual analog scale (VAS) ranging from 0 to 10, with 10 being the highest comfort, was completed by all \u0000 patients immediately after each PSG. \u0000Results: There was no significant difference between the therapy modes in the apnea/hypopnea index (median \u0000 1.5 events/hour (h) with C-Flex+ (interquartile range (IQR) 0.7 to 3.6) vs. 1.7 events/h with CPAP (IQR 0.8 to 3.8), \u0000 p=0.178). The sleep efficiency, sleep architecture and nocturnal oxygenation were also comparable. Most patients \u0000 preferred C-Flex+ over CPAP (65%, p<0.001). Baklan","doi":"10.4172/2167-0277.1000265","DOIUrl":"https://doi.org/10.4172/2167-0277.1000265","url":null,"abstract":"In this study, we aimed to evaluate the effect of CPAP treatment on hemograms of patients with severe OSA in the lack of comorbidities which might affect the real values of hematological parameters. \u0000This retrospective cohort study patients selected from those whose polysomnography reports were compatible with severe OSAS and who underwent PAP titration in our sleep medicine department. 49 patients were enrolled to the study. The baseline hemograms of the participants were collected from one of the patients' visits before they started to use CPAP devices. The control hemograms were evaluated from one of the control visits in the first six months of treatment. We examined the red cell distribution width (RDW), mean platelet volume (MPV), hemotocrit (Hct) and platelet (Plt) count from the hemograms. \u0000The mean age was 58.2 years ± 11.3 years, 38.8% were female and 61.2% were male. 69.4% were non-smokers and 30.6% were ex-smokers. The mean body mass index (BMI) was 31.6 ± 5.8 while the mean apnea hypopnea index (AHI) was 50.2 ± 29. The mean duration of CPAP therapy was 4.8 months ± 3.3 months. The mean RDW, MPV, Hct and Plt count values before CPAP treatment were in normal ranges of laboratory tests. All of the values reduced after CPAP therapy but only the decrease in MPV was statistically significant. \u0000The reduced values supported that CPAP treatment helped to control hypercoagulability and prevented from cardiovascular disorders in OSAS. However, the relationship between pediatric OSA and childhood asthma remains unclear. Here, we conducted a large-scale population-based cohort study for evaluating our hypothesis that childhood asthma is a risk factor for pediatric OSA. \u0000Methods: From 2000 to 2007, children with and without asthma who were frequency matched by age, sex, urbanization level, comorbidities, and baseline year were enrolled from the Taiwan National Health Insurance Research Database (NHIRD). We compared pediatric OSA risk between asthma and non-asthma cohorts by using multivariable Cox regression analysis. \u0000Results: We observed a significant relationship between asthma and OSA. In total, we included 305094 children with asthma and 305094 without asthma. The overall incidence rate ratio of OSA was 3.56-fold higher in the asthma cohort than in the non-asthma cohort (566 vs. 249 per 1000 person-year). After adjustment for potential risk factors, the adjusted hazard ratio (HR) of OSA was 1.82 [95% confidence interval (CI)=1.56-2.13]. Regardless of sex, the asthma cohort had a higher OSA risk than the non-asthma cohort did. Patients with asthma, excluding particular comorbidities, had a significantly increased OSA risk. Scant researches have been \u0000 carried out to explore the efficacy of cognitive therapy on mitigating clinical symptoms and cognitive functions of the \u0000 brain. The novelty of the present study is in combining cognitive method and medical treatment. This research aims \u0000 to compare the effectiveness of continuous positive air pressure (CPAP) therapy and cognitive-behavioral treatment \u0000 (CBT) on improving mental cognitive functions in patients with sleep apnea. \u0000Methods: The study population included all patients referred to the Bahar Sleep Disorders Clinic in Tehran, \u0000 Iran. The sample consisted of 45 people who were randomly placed into three groups of 15 people (CPAP \u0000 therapy, cognitive behavioral therapy, and control groups). All three groups were psychologically assessed prior \u0000 to the intervention. Next, individuals in the cognitive-behavioral group underwent 12 sessions of cognitive training, \u0000 progressive muscle relaxation, mental visualization, and sleep hygiene. The other group went through CPAP \u0000 therapy. After the intervention, both groups were assessed psychologically. In humans, 209 amino acids with one disulfide bond form a primarily alpha-helical prion protein structure with a molecular mass of 35 to 36 kDa. The specific role and function of the prion protein elude research efforts and remains a controversial topic. Misfolding of the native prion protein leads to a protein structure with increased proportion of alpha-helices to beta-sheets. Advancing our understanding of the role of the prion protein as it relates to sleep and sleep disturbances presents an appealing avenue into diagnosing and more effectively treating a devastating and debilitating disease. New research into multiple system atrophy further validates evidence of a direct association between the prion protein and sleep. This reinforces previous observations regarding changes in sleep patterns noted with patients affected by Creutzfeldt-Jakob Disease and Fatal Familial Insomnia. From these earlier studies, a more focused approach to identifying and defining the role of the prion protein appears possible. A clearer understanding of the functional prion protein in its native role within the cell membrane allows identification of the potential signaling pathways and the aberration that likely occurs that leads to misfolding at the thermodynamic level. A total of 373 medical students (mean age=20.6 years ± 2.3 years) from a medical school in Haiti participated in this study. Participants were administered a questionnaire assessing their sleep health and cardiovascular outcomes. The rate of sleep apnea symptoms was: snoring (13.2%), excessive daytime sleepiness (73.7%), and difficulty maintaining sleep (25.3%). Many reported falling asleep while watching television (68.2%) or while driving (7.8%). Based on logistic regression analysis, reported nocturnal breathing pauses was the most important predictor of the likelihood of reporting a history of cardiac disease (14.96; 95% CI=1.27-76.07). [1] provided information about sleep quality in the United States. From the findings, it was concluded that sleep quality varies by occupation and location of state, in general. In terms of occupations, Healthcare Practitioners ranked the as the most group commonly experiencing poor sleep. The major occupation group with high prevalence of poor sleep was the firefighters and prevention workers. A second major general point of the Shockey et al. [1] was the locations in the United States where sleep quality was poorest in southeastern United States and in states along the Appalachian Mountains. These regions have been identified as areas with higher estimates of obesity, diabetes, and death rates from heart disease and stroke [1]. In this review, we discuss the growing epidemic of obesity and OSA, highlighting the common pathogenic hypotheses linking these risk factors to CVD. The \u0000 main objective of the study was to compare the OSLER values versus EEG-determined Sleep Onset latency (EEGSOL). \u0000Methods: Patients referred for assessment of daytime vigilance were recruited from a tertiary sleep clinic. \u0000 Patients underwent the OSLER (4 x 40 minutes trials; if 7 consecutive stimuli are missed, the trial is terminated and \u0000 sleep onset is concluded to have occurred) with simultaneous EEG recordings. Determination of EEG-SOL using \u0000 American Academy of sleep Medicine (AASM) criteria to score sleep during daytime testing was compared to \u0000 OSLER values. \u0000Results: 65 OSLER were performed in 65 subjects for a total of 260 trials (65X4 trials/OSLER). In 136 out of the \u0000 260 trials (52.3%), subjects remained awake according to the OSLER, while EEG-SOL was scored in 5 of the 136 \u0000 trials (3.7%). Of the 124 trials (47.7%) with sleep onset, (i.e. 7 consecutive missed stimuli) the mean sleep onset \u0000 value was 14.5 ± 10.9 min and EEG-SOL was recorded before the end of the trial in 37 trials (29.8%) (Mean \u0000 difference EEG-SOL vs. OSLER 4.1 ± 5.8 min). \u0000Conclusion: Using current AASM criteria for daytime testing, EEG-determined sleep onset latency is unlikely to \u0000 occur in subjects with no sleep onset in the OSLER.
