Adebo","doi":"10.15761/jccr.1000152","DOIUrl":"https://doi.org/10.15761/jccr.1000152","url":null,"abstract":"Structured abstract Objectives: Rare case of bilateral superior vena cava (SVC) with contralateral atrial drainage in an adult patient with brain abscess and transient ischemic attack diagnosed with cardiac magnetic resonance imaging (MRI). Background: Anomalous systemic venous return is a variable congenital malformation. Furthermore, the consequence of anomalous systemic venous return is also variable. Prior literature has documented drainage of superior vena cava to the left atrium, with right to left shunting. We report a case of bilateral SVC with contralateral atrial drainage in an adult patient. The consequence of the right to left shunt resulted in brain and frequent episodes of transient ischemic attack (TIA). Methods: Evaluation of echocardiography and cardiac MRI demonstrated bilateral SVC with contralateral atrial drainage. Results: Transthoracic echocardiogram revealed dilated coronary sinus. Agitated saline bubble contrast study from right upper extremity showed immediate opacification of the left cardiac chambers suggestive of right SVC draining to left atrium. Transesophageal echocardiogram confirmed dilated coronary sinus; agitated saline contrast injection from left arm revealed complete opacification of the coronary sinus followed by opacification of the right heart, consistent with the presence of the left sided SVC draining to dilated coronary sinus. Cardiac MRI confirmed right SVC draining to the left atrium and the left SVC draining to the right atrium. Conclusions: Anomalous systemic venous return is a relatively common and variable congenital malformation with heterogeneous consequences. Anomalous drainage of systemic venous return to the left atrium is an unusual variant of systemic venous return anomalies, which is characterized by right-to-left shunt. We present extremely rare case of bilateral SVC draining into contralateral atrium, resulting in a right-to-left shunt. This right-to-left shunt can result in persistent hypoxemia, mild cyanosis, paradoxical embolism with brain abscess. Patients may also remain asymptomatic. With accurate diagnosis, this condition can be treated with good short-term results. abstract Anomalous systemic venous return is a variable congenital malformation. The patient had been treated with VKA and low dose of acetylsalicylic acid since surgery. On 28th day of postoperative period an ischemic stroke occurred. In transoesophageal echocardiography we have discovered non-obstructive thrombosis of the artificial mitral annulus and a gap in left atrial appendage (LAA) patch. There was rapid outflow through the gap in LAA directing to mitral annulus in close proximity to visible thrombus. The pathologic flow might have had the contribution in thrombus mobilization and stroke occurrence. We have decided to change anticoagulation to low molecular weight heparin. In control echocardiography, during heparin therapy, thirteen days later, there was remarkable worsening of annulus thrombosis with multiple thrombi present. Re-exchange anticoagulation to warfarin yielded in almost complete resolution of thrombosis after eleven days. If the level is below 90 percent, it is considered low and called hypoxemia. Low concentration of oxygen may cause a large number of pulmonary disorders. It is actually accumulation of large number of phobias relating to fear of heights, close rooms, and dark places etc. Rides phobia can be overcome with the help of certain therapies. Pulse oximetry is an estimate the percentage of oxygen. Oxygen level play a vital role in our body. The most medically significant spiders belong to the genera Latrodectus and Loxosceles . This paper presents a posible case of steatodism in a young woman and her pet. Case description: 26-year-old female patient, who reports a clinical history characterized by paresthesia, malaise, fever, diarrea and a painful papule in the left cheek after being bitten by a spider. Immediately after being bit, the patient hit the spider with the back of her hand and it fell to the ground, where her dog swallowed it. The dog presented with vomiting and general discomfort after ingestion. Symptomatic therapy was given for comfort, and neither the patient nor the dog required antivenin therapy. Both evolved favorably. Discussion: The relevance of this case is the involvement of two mammals (a human and her dog) due to the accidental contact with a spider, possibly of the genus Latrodectus or Steatoda . Conclusion: Two possible cases of steatodism are described. Los errores innatos del metabolismo pueden ocasionar importante morbimortalidad en el periodo neonatal. Dentro de estos trastornos se encuentran los desordenes en la biogenesis del peroxisoma, que originan cuadros clinicos complejos y graves debido a que los peroxisomas estan presentes en todas las celulas nucleadas de los mamiferos. Presentacion del caso. Paciente dismorfica recien nacida quien presento convulsiones al nacer y desarrollo compromiso neurologico, hepatico, renal y cardiaco durante sus 20 dias de vida; en la necropsia se confirmo compromiso hepatico y renal, por lo que se sospecho de una enfermedad peroxisomal. Las alteraciones bioquimicas reportadas en el presente caso fueron compatibles con un trastorno en la biogenesis de los peroxisomas. Como antecedente se registro la muerte neonatal de un hermano y un primo. Discusion. La presencia de dismorfismo y convulsiones al nacer puede indicar la presencia de una enfermedad metabolica. En la paciente reportada, los hallazgos del examen fisico y la demostracion del compromiso hepatico, renal y cardiaco se ajustan a la descripcion inicial del sindrome de Zellweger; por su parte, las alteraciones bioquimicas son concluyentes de un trastorno en la biogenesis de los peroxisomas. Conclusiones. Es necesario definir si el dismorfismo es un hallazgo aislado y si existe afectacion de otros organos o sistemas para establecer un posible diagnostico de biogenesis de los peroxisomas. We report the unusual case of a patient with sickle cell disease who developed symptomatic QT interval prolongation, that occurred despite documented normalization of electrolytes, and absence of any QT prolonging medications.
