However, the SBP thresholds have not been standardized for DR prevention. We analyzed data from a cross-sectional study (December 2017-November 2018, n = 426, mean age 59.15±13.68) from the Department of Endocrinology, Guangdong Provincial People's Hospital in patients with type 2 diabetes mellitus. DR severity was assessed by retinal photographs. The International Clinical Diabetic Retinopathy and Diabetic Macular Edema Disease Severity Scale was used to classify DR severity and divide it into two groups: with DR and without DR. SBP was analyzed as a continuous variable. Multivariate logistic regression models, smoothed curve fitting, threshold analysis, and interaction tests were used to assess the relationship between BP and DR. Prevalence of DR in the study population was 39.20%. After adjusting for age, sex, DM duration, HbA1c, BUN, HDL, LDL, TRIG, CHOL, TP, DBP, PP, eGFR, and CKD stage, the association between SBP and DR appears as a threshold effect, with a inflection point of 132mmHg. The risk of DR did not change significantly when SBP ≤132mmHg (OR: 0.86; 95% CI: 0.63 to 1.17, p=0.3400). When SBP ≥132mmHg, each 10 mmHg increase in SBP raise the risk of developing DR by 28% (OR:1.28; 95% CI:1.07 to 1.54, P=0.0081). In addition, a stronger association of SBP with DR in patients with TP≤60g/L (OR=1.58, 95% CI: 1.19- 2.08, P=0.001) compared to those with TP>60g/L (OR=1.15, 95% CI: 1.03, 1.27, P=0.012) was discovered, with a P value for interaction=0.023.","PeriodicalId":72287,"journal":{"name":"Archives of nephrology and urology","volume":"71 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134889427","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Among the many reasons associated with LUTS, numerous studies have shown an association between LUTS and the emotional state of patients. Various researches in different Arch Nephrol Urol 2021; 4 (4): 143-150 DOI: 10.26502/anu.2644-2833046 Archives of Nephrology and Urology 144 geographical locations have been conducted to assess this relationship between LUTS and mental health in different ways. Findings from previous studies were highly dependent on the methodologies and criteria used. The purpose of this study is to evaluate any relationship between LUTS and the emotional state of patients in a sample of clinic attendees of General Urology (GU) clinic in Teaching Hospital, Peradeniya, Sri Lanka where the emotional state is assessed through criteria for depression, anxiety and stress. Here, International Prostate Symptom Score (IPSS) is used as the screening tool to evaluate LUTS and Depression Anxiety Stress Scale-21 (DASS-21) is used to quantify the emotional state. Methodology: This is a descriptive, cross-sectional study performed among patients attending the GU clinic at Teaching Hospital Peradeniya, Sri Lanka to identify the relationship between LUTS and the emotional state. Study samples were collected from July 2020 to October 2021. Subjects were asked questions using a questionnaire on the presence of Lower Urinary Tract Symptoms and the emotional state. Data analysis was performed using SPSS version 20. The association between IPSS and DASS-21 scores was analyzed using simple linear regression analysis. Results: The study group consisted of 161 patients who attended the GU clinic at the Teaching Hospital, Peradeniya. 84.5% of the sample was male while 15.5% was female. The mean age of the studied group was 62 ± 14.51 (p<0.005). The highest prevalent symptom was nocturia (81%). The mean total IPSS score was approximately 17. Among 161 patients 36.17%, 50.35% and 30.5% people had mild – extremely severe depression, anxiety and stress respectively. According to the coefficients of linear regression analysis, the most significant correlation was observed among LUTS and depression (p = 0.001). When IPSS score was increased by 1, depression score is predicted to be increased by .292. Similarly, when IPSS score was increased 1, anxiety score is predicted to be increased by .221 (p = 0.003) and stress score is predicted to be increased by .257 (p = 0.008). Conclusion: A clear association of stress with the LUTS was observed in a sample of 161 patients attending the GU clinic at Teaching Hospital, Peradeniya. The most prevalent LUT symptom was nocturia. Anxiety was the most prevalent emotional issue out of stress, anxiety and depression. Maximum correlation was observed among depression and LUTS with a higher significance.","PeriodicalId":72287,"journal":{"name":"Archives of nephrology and urology","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69344684","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This study was aimed to evaluate the association of Socioeconomic Status (SES) with glomerular diseases among patients attending at a tertiary care hospital in Bangladesh. Methods: This cross sectional study was conducted at Department of Nephrology, Shaheed Sheikh Abu Naser Specialized Hospital, Khulna, Bangladesh from January 2016 to December 2020. A total of 184 diagnosed patients of glomerulonephritis (GN) were enrolled. Patient’s socioeconomic status (SES) was assessed using Modified BG Prasad socioeconomic classification. Morphological pattern of glomerular diseases were assessed according to the histopathology report. Relationship of glomerular diseases with particular socioeconomic status (SES) was identified. Results: Among 184 study patients 99 were male and 85 were female, their mean age was 31.18 ± 13.55 years. It was observed that, in proliferative variety; Mesengial Proliferative Glomerulonephtis (Mes PGN) was the leading morphological variant (34.7%) followed by Membranoproliferative Glomerulonephtis [MPGN (16.8%)], Lupus Nephritis [LN (7.1%)], Focal Segmental Proliferative Glomerulonephritis [FSPGN (7.1%)], IgA Nephropathy (6.5%), IgM Nephropathy (4.3%) and Crescentic Glomerulonephtis [Cres GN (2.2%) serially. In the non-proliferative glomerulonephtis; Membranous Nephropathy (MN) was the commonest form (10.9%) followed by Focal Segmental Glomerulosclerosis [FSGS (7.6%)] and Minimal Change Disease [MCD (2.7%)]. All types of GN were significantly prevalent in the lower socioeconomic classes (Class- III, IV, V), while the frequency of Mes PGN and MPGN were significantly higher in Class- IV and Class- V socioeconomic classes. The CKD patients suffer from various types of anemia including iron deficiency anemia. Measurement of reticulocyte hemoglobin content (CHr) has been proposed as a measure of available iron stores. In this study CHr and serum iron profile were compared with stainable iron in the bone marrow to evaluate the CHr as a marker of iron deficiency in pre-dialytic CKD patients. This cross sectional study was conducted at the Department of Nephrology, from January 2018 to December 2018. A total of seventy (70) pre-dialytic CKD patients were selected. All participants had undergone bone marrow study for detection of iron deficiency anemia and it was observed that, 60% CKD patients had iron deficiency by bone marrow iron stain. Serum iron profile [serum iron, total iron binding capacity (TIBC), serum ferritin, transferin saturation (TSAT)] and reticulocyte haemoglobin content (CHr) of each patient were determined accordingly. Data analysis reveals that CHr was significantly low in iron deficient group (p<0.001). The sensitivity and specificity of CHr at a cut-off 28 pg/cell was 81% and 75.0% respectively (p<0.001); sensitivity and specificity of serum ferritin at a cut-off 100 ng/ml was 40.6% and 67.9% respectively (p=0.051); while sensitivity and specificity of TSAT at a cut-off 20% was 54.2% and 57.1% respectively (p=0.465).
