Also, the scrotum skin was also transferred to the abdomen side of penis. The normal anatomic position of penis and scrotum was returned.</p><p><strong>Results: </strong>Between Jan 2004 to Oct 2015,34 cases were treated.5 scrotum fascia skin flaps underwent ischemia at the flap tip with delayed healing. All the other flaps survived with primary healing and good appearance.</p><p><strong>Conclusions: </strong>Adhesion between penis tunica albuginea and scrotum sheath capsule should be separated to return the testis and their sheath capsule, as well as scrotum skin to the abdomen side of penis. {"title":"[Nasolabial flap with Facial artery perforator for repairing defects after resection of midface tumor].","authors":"Chi Zhang,&nbsp;Xiujun Tang,&nbsp;Zairong Wei,&nbsp;Dali Wang,&nbsp;Bo Wang,&nbsp;Xueqin Zeng,&nbsp;Wenhu Jin","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To evaluate the effect of nasolabial flap with Facial artery perforator for repairing defects after resection of midface tumor.</p><p><strong>Methods: </strong>Between January 2012 and June 2015,31 cases with midface tumor, including base cell carcinoma(18 cases),squamous cell carcinoma(13 cases).The tumor size ranged from 0.8 cm × 2.0 cm to 2.0 cm × 3.5 cm. The extended resection of the tumor tissue was performed according to the characters of tumor. Intraoperative frozen specimen was used to make sure no invasion at margin and wound base. According to the location and size of the defect, nasolabial flap with perforator was designed to repair the defect, the defect at donor site was directly closed. The flaps size ranged from 1.5 cm × 3.0 cm to 3.0 cm × 5.0 cm, and the pedicle length was 1.0 cm to 2.5 cm.</p><p><strong>Results: </strong>All the 31 flaps survived completely, including 5 cases with vascular crisis at the flap tip, which resolved after active procedure. All the patients were followed up for 6-36 months with unconspicious scar at nasolabial fold and no deformities. Canine models were constructed using gracilis muscles and lower abdominal flap with anterior rectus sheaih, instead of the ALT flap. Seven months later, the erectile function of the neophalli elicited by electrostimulation was evaluated. The length, diameter and stiffness were measured respectively and the results were analyzed statistically using paired-samples t test.</p><p><strong>Results: </strong>The penile reconstruction was success with good appearance in the cadaveric study. The hardness of the reconstructed penis in canine models was graded as 1.2 ± 0.4 at rest. With the nerve pedicles stimulated, it increased significantly and was graded as 3.4 ± 0.5 (P < 0.05),while the length shortened and the diameter did not change significantly. After culturing 72 h,CCK-8 solution was added to record absorbance at 450 nm to test proliferation of NHDFs. Fluorescence quantitative PCR was used for testing mRNA expression of α-SMA and type Ⅰ and Ⅲ collagen. Digestion method was to test the hydroxyproline content in the supernatant liquor. Western Blot was used for testing protein expression of α-SMA, type Ⅰ and Ⅲ collagen and Smad2,Smad3,P-Smad2 and P-Smad3.One-way analysis of variance were used to analyze differences among more than two groups, while LSD-t test as post hoc test were used to make paired-comparisons among the groups. P < 0.05 indicated significant difference.</p><p><strong>Results: </strong>With the stimulation of 2,5,10 ng/ml TGF-β1,the absorbance values(A values),mRNA and protein expression of α-SMA and type Ⅰ and Ⅲ collagen, hydroxyproline content, and protein expression of Smad2,Smad3,P-Smad2 and P-Smad3 were all elevated contrasted with the control group (P < 0.05,P < 0.01).Without TGF-β1 stimulation, SJYHC only increased the absorbance values(A values) from 1.645 ±0.052 to 1.796 ±0.060(P <0.05),while mRNA and protein expression of α-SMA and type Ⅰ and Ⅲ collagen, hydroxyproline content, and protein expression of Smad2,Smad3,P-Smad2 and P-Smad3 were infinitely variable(P > 0.05).With stimulation of 2,5 ng/ml TGF-β1,SJYHC elevated the the absorbance values (A values) from 1.814 ± 0.052,1.970 ± 0.045 to 1.981 ± 0.061,2.133 ± 0.059 (P < 0.05).While stimulated with 10 ng/ml TGF-β1,SJYHC declined the absorbance values(A values) from 2.130 ± 0.050 to 1.958 ± 0.045 (P < 0.05). {"title":"[Reconstruction for dysplastic polydactyly of thumb with an island compound flap].","authors":"Gao Weiyang,&nbsp;Shi Wei,&nbsp;Ding Jian,&nbsp;Wang Anyuan,&nbsp;Wang Long,&nbsp;Zhou Zongwei","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To investigate the necessity and feasibility of digit fusion with an advanced island flap in thumb dysplasia of radial polydactyly.</p><p><strong>Methods: </strong>From February 2007 to January 2015,we collected the patients with radial polydactyly, whose nail width or thumb girth was less than 80% of contralateral side. Among them, some patients were also associated with radial deviation deformities in two thumbs or a shorter radial thumb compared with ulnar thumb. In these cases, an island compound flap, which was based on proper palmar digital artery (PPDA) in radial thumb and contained nail, part distal phalanx and skin, combining with ulnar side of ulnar thumb was used to reconstruct a new thumb. Only the patients with a smaller thumb girth were treated by an island flap to augment the thumb size. There were 26 patients, including 29 sides.Preoperative Doppler ultrasonic examination was assigned to observe the amount and distribution of princepspollicisartery(PPA) and PPDA. Appearances were evaluated by Wang -Gao scoring system and functions were evaluated by Tata scoring system.</p><p><strong>Results: </strong>There were two basic artery forms through preoperative ultrasonography and surgical exploration in 29 hands: both thumbs only had one PPDA; the main thumb had two while the extra thumb still had one. The former was responsible in 82.76% (24 hands).All cases were operated according to preoperative design using an island compound flap.26 hands in 23 patients were followed up more than 6 months. Flap results included survival in 22 patients, tension vesicle in 2 patients and tip necrosis in 2 patients. The incision was infected in one patient with the treatment of changing dressing regularly. The function results were excellent in 21 patients, good in 4,fair in 1.Aesthetic results were excellent in 13 patients, good in 4 and fair in 1.</p><p><strong>Conclusions: </strong>There is always one developed PDAA in extra thumb, which can be used to design a flap. The longitudinal advanced flap can also eliminate the tension caused by unequal length in two thumbs. {"title":"[The anatomy basis and clinical application of the horn fasciocutaneous perforator flaps for the reconstruction of the defects on finger soft tissue].","authors":"Yu Wang,&nbsp;Xiaojing Li,&nbsp;Xinyi Li,&nbsp;Jinlong Ning,&nbsp;Demeng Ma,&nbsp;Huairui Cui,&nbsp;Jun Zheng","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To discuss the clinical effect of horn perforator flaps for the treatment of finger soft tissue defect.</p><p><strong>Methods: </strong>① Lead oxide-gelatin mixture injection and CT scanning were performed in 4 fresh adult cadavers and the data was put into mimics 17.0 to achieve the three-dimensional reconstruction. The regulation and origins of the perforator vessel in proper palmar digital artery was observed. ② From July 2012 to July 2015,9 cases with finger soft tissue defects were treated with horn perforator flaps. The defect areas ranged from 0.6 cm × 0.8 cm to 1.2 cm × 1.5 cm and the flap size ranged from0.8 cm×2.5 cm to 1.6 cm×4.2 cm. We put the head end of flaps at defect side. The horn perforator flap was designed with 1-2 perforator vessel and rotated to cover the defect. The defect at donor site was closed directly.</p><p><strong>Results: </strong>We got 4 symmetric arteries in palmar and back surfaces of fingers,2 for proper palmar digital arteries and 2 for dorsal digital arteries. The proper palmar digital arteries have a larger diameter which is the main source of blood supply for fingers. All 9 flaps survived after the operation. Both the recipient area and donor areas received sound healing. With 2 months to 1.5 years of follow-up visit, the shape, color and luster, texture and function all had good healing.</p><p><strong>Conclusions: </strong>The horn perforator flaps for repair finger soft tissue defect have advantages of flexible design, reliable perforator vessels. The defect at donor site can be closed directly with no skin graft. KFb and NFb were cultured under normoxia or hypoxia (2% O2).Differences of cell proliferation were detected by CCK-8.Flow cytometer was used to detect the content of mitochondria and reactive oxygen species (ROS) in KFb and NFb. Ultra-structures of mitochondria in KFb and NFb were observed by transmission electron microscope (TEM).Mitochondria fusion/fission related genes MFN1,MFN2 and FIS1 were detected by RT-PCR. Oxygen consumption rate, lactate production and ATP contents were determined by spectrophotometry.</p><p><strong>Results: </strong>KFb showed a higher proliferation rate compared with NFb, especially under hypoxia. The oxygen consumption rate, ATP content, lactate production and ROS of KFb were lower than NFb under normoxia. After incubated under hypoxia, there was a significant increase in oxygen consumption, ATP content, lactate production and ROS in KFb, while NFb showed less increase compared with KFb. KFb had 15.33% more mitochondrion than NFb, and expressions of MFN1, MFN2, FIS1 in KFb were 33.27%,113.39% and 20.34% higher compared with NFb. All 15 flaps survived</p><p><strong>Conclusions: </strong>Emergency debridement and reconstruction with lateral arm free-flap is a very effective method for hand thermal-crush injuries. The adipofascial tissue was used to fill the dead space, the propeller flap was used to cover the superficial wound. The defect at donor site was closed directly.</p><p><strong>Results: </strong>The size of the propeller flap based on peroneal artery perforator ranged from 4 cm × 10 cm to 7 cm × 16 cm, the size of the adipofascial flap ranged from 2 cm × 4 cm to 5 cm × 7 cm. All flaps survived completely and the wounds healed primarily. The defects at donor sites got primary healing without complications. All patients were followed up for 5 to 21 months with an average of 8 months without recurrence of infection during the follow-up period. The appearance, texture, and color of the flaps were similar to those of the recipient sites. The donor sites were covered with free skin grafts. The skin and soft tissue defects size ranged from 7.0 cm × 10.0 cm to 9.5 cm × 18.0 cm (average 9.2 cm× 12.0 cm).</p><p><strong>Results: </strong>The flaps size ranged from 9.0 cm × 12.0 cm-11.5 cm × 20.0 cm (average 10.3 cm × 13.5 cm).Except 2 cases of small area necrosis occurred in the distal end of the flap, which healed after dressing change; the other 7 flaps survived completely. The donor wounds healed primarily. Postoperative patients were followed up for 12-26 months (mean 14.5 months). Flap appearance, texture and thickness were similar to the surrounding normal tissue. According to the sensory function evaluation standard formulated by the British Medical Research Council in 1954,the result of skin flap sensory evaluation was assessed as S2 in two cases,S3 in four cases,S3 in three cases, with excellent and good rate as 77.8%.According to the assessment standard for upper limb function issued by Chinese Medical Association of hand surgery, the limb function were assessed as excellent in 6 cases, good in 2 cases, poor in 1 case, with the excellent and good rate as 88.9%.</p><p><strong>Conclusions: </strong>The large flowthrough venous flap has the advantages of simple operation, minimal donor site morbidity and satisfactory appearance.
