Driven by pioneering breakthroughs, technological advancements, and a deepened understanding of immunology, LT has seen remarkable progress. Some of the most notable breakthroughs in the field include advances in immunosuppression, a revised model for end-stage liver disease, and artificial intelligence (AI)-integrated imaging modalities serving diagnostic and therapeutic roles in LT, paired with ever-evolving technological advances. Additionally, the refinement of transplantation procedures, resulting in the introduction of alternative transplantation methods, such as living donor LT, split LT, and the use of marginal grafts, has addressed the challenge of organ shortage. Moreover, precision medicine, guiding personalized immunosuppressive strategies, has significantly improved patient and graft survival rates while addressing emergent issues, such as short-term complications and early allograft dysfunction, leading to a more refined strategy and enhanced post-operative recovery. Looking ahead, ongoing research explores regenerative medicine, diagnostic tools, and AI to optimize organ allocation and post-transplantation car. Protein biomarkers that can be derived from modern approaches of proteomic analysis of liquid biopsies (serum, urine) represent a promising innovation in the monitoring of kidney transplant recipients.</p><p><strong>Aim: </strong>To investigate the diagnostic utility of protein biomarkers derived from proteomics approaches in renal allograft assessment.</p><p><strong>Methods: </strong>A systematic review was conducted in accordance with PRISMA guidelines, based on research results from the PubMed and Scopus databases. The primary focus was on evaluating the role of biomarkers in the non-invasive diagnosis of transplant-related complications. Eligibility criteria included protein biomarkers and urine and blood samples, while exclusion criteria were language other than English and the use of low resolution and sensitivity methods. The selected research articles, were categorized based on the biological sample, condition and methodology and the significantly and reproducibly differentiated proteins were manually selected and extracted. Functional and network analysis of the selected proteins was performed.</p><p><strong>Results: </strong>In 17 included studies, 58 proteins were studied, with the cytokine CXCL10 being the most investigated. Biological pathways related to immune response and fibrosis have shown to be enriched. Applications of biomarkers for the assessment of renal damage as well as the prediction of short-term and long-term function of the graft were reported. The number of organs available for transplantation is lower than the demand. To provide fair organ distribution, predictive mortality scores have been developed.</p><p><strong>Aim: </strong>To compare the Acute Physiology and Chronic Health Evaluation IV (APACHE IV), balance of risk (BAR), and model for end-stage liver disease (MELD) scores as predictors of mortality.</p><p><strong>Methods: </strong>Retrospective cohort study, which included 283 adult patients in the postoperative period of deceased donor liver transplantation from 2014 to 2018.</p><p><strong>Results: </strong>The transplant recipients were mainly male, with a mean age of 58.1 years. Donors were mostly male, with a mean age of 41.6 years. The median cold ischemia time was 3.1 hours, and the median intensive care unit stay was 5 days. For APACHE IV, a mean of 59.6 was found, BAR 10.7, and MELD 24.2. The 28-day mortality rate was 9.5%, and at 90 days, it was 3.5%. The 28-day mortality prediction for APACHE IV was very good [area under the curve (AUC): 0.85, <i>P</i> < 0.001, 95%CI: 0.76-0.94], <i>P</i> < 0.001, BAR (AUC: 0.70, <i>P</i> < 0.001, 95%CI: 0.58-0.81), and MELD (AUC: 0.66, <i>P</i> < 0.006, 95%CI: 0.55-0.78), <i>P</i> < 0.008. At 90 days, the data for APACHE IV were very good (AUC: 0.80, <i>P</i> < 0.001, 95%CI: 0.71-0.90) and moderate for BAR and MELD, respectively, (AUC: 0.66, <i>P</i> < 0.004, 95%CI: 0.55-0.77), (AUC: 0.62, <i>P</i> < 0.026, 95%CI: 0.51-0.72). All showed good discrimination between deaths and survivors. This study searched for literature on traditional Chinese medicine (TCM) syndromes after kidney transplantation in China, conducted statistical analysis of the results, and sought to identify the underlying patterns.</p><p><strong>Aim: </strong>To understand the TCM syndromes after renal transplantation and associated rules and provide a theoretical basis for further clinical research.</p><p><strong>Methods: </strong>The literature pertaining to TCM syndromes in renal transplantation, published in the China National Knowledge Infrastructure, Wanfang database, and WIP database from 1970 to 2021, was meticulously searched and comprehensively and statistically analyzed.</p><p><strong>Results: </strong>Following the established inclusion and exclusion criteria, 13 studies were selected for analysis. Post-renal transplantation, no significant discrepancy was noted among the groups based on the location of TCM viscera. However, when categorized according to TCM pathogenic factors, the groups with spleen and kidney yang deficiency, as well as liver and kidney yin deficiency, exhibited a statistically significant difference in the frequency.</p><p><strong>Conclusion: </strong>Currently, the research on TCM syndromes pertaining to renal transplantation is in its nascent phase. Nevertheless, circumstances may arise where selecting the RK over the left kidney (LK) is unavoidable. Consequently, it is crucial to thoroughly examine the implications of such a choice on the overall transplant outcome.</p><p><strong>Aim: </strong>To compare transplant outcomes between recipients of RK and LK while examining the factors that influence these outcomes.</p><p><strong>Methods: </strong>We retrospectively analyzed data from adult patients who received LD kidney transplants involving meticulous patient selection and surgical techniques at our center from January 2020 to December 2023. We included all kidney donors who were over 18, fit to donate, and had undergone diethylenetriamine pentaacetic acid split function and/or computed tomography based volumetry. The variables examined comprised donor and recipient demographics, and outcome measures included technical graft loss (TGL), delayed or slow graft function (SGF), and post-transplant serum creatinine (SC) trends. We used a logistic regression model to assess the likelihood of adverse outcomes considering the donor kidney side.</p><p><strong>Results: </strong>Of the 250 transplants performed during the period, 56 (22%) were RKs. The recipient demographics and transplant factors were comparable for the right and LKs, except that the donor warm and cold ischemia time were shorter for RKs. TGL and SGF each occurred in 2% (<i>n</i> = 1) of RKs and 0.5% (<i>n</i> = 1) of LKs, the difference being insignificant. These complications, however, were not related to the venous anastomosis. One RK (2%) developed delayed graft function after 48 hours, which was attributable to postoperative hypoxia rather than the surgical technique. Liver transplantation (LT) offers the most effective treatment. HCC recurrence is the strongest risk factor that decreases post-LT survival in patients transplanted for HCC. The rate of HCC recurrence is generally reported as 8%-20% in the literature. Many predictors of HCC have already been researched, however, to our knowledge there are no published studies on this topic using Australian data.</p><p><strong>Aim: </strong>To determine the rate and identify predictors of HCC recurrence in a contemporary Western Australian LT cohort.</p><p><strong>Methods: </strong>We performed a retrospective cohort study of all liver transplants in patients with HCC at Sir Charles Gairdner Hospital between 2006 and 2021. Data was collected from various health record databases and included recipient demographics, serum biochemistry, radiology, operation notes, explant histopathology and details of recurrence. Overall survival of HCC patients post-LT, stratified for recurrence, was calculated by Kaplan Meier analysis. Univariate and multivariate Cox regression was used to determine predictors of HCC recurrence post-LT.</p><p><strong>Results: </strong>Between 1/1/2006 and 12/31/2021, 119 patients were transplanted with HCC. 8.4% of subjects developed recurrent HCC after LT with median follow-up time of 5.4 years. The median time to recurrence was 2.9 years ± 0.75 years. When comparing baseline characteristics, a greater proportion of subjects with recurrence had common characteristics on explant histopathology, including > 3 viable nodules (<i>P</i> = 0.001), vascular invasion (<i>P</i> = 0.003) and poorly differentiated HCC (<i>P</i> = 0.03). Unadjusted survival curves showed lower 1-year, 3-year, 5-year and 10-year survival rates in subjects with HCC recurrence compared to those without HCC recurrence (90% <i>vs</i> 92%, 70% <i>vs</i> 88%, 42% <i>vs</i> 80%, 14% <i>vs</i> 76%, respectively; log rank <i>P</i> < 0.001).</p><p><strong>Conclusion: </strong>HCC recurrence was low at 8.4% in this contemporary Australian cohort, however it significantly impacted post-LT survival. Endothelial dysfunction (ED), characterized by impaired vasodilation, inflammation, and thrombosis, triggers future cardiovascular (CV) diseases. Chronic kidney disease, a state of chronic inflammation caused by oxidative stress, metabolic abnormalities, infection, and uremic toxins damages the endothelium. ED is also associated with a decline in estimated glomerular filtration rate. After kidney transplantation, endothelial functions undergo immediate but partial restoration, promising graft longevity and enhanced CV health. However, the anticipated CV outcomes do not happen due to various transplant-related and unrelated risk factors for ED, culminating in poor CV health and graft survival. ED in kidney transplant recipients is an under-recognized and poorly studied entity. CV diseases are the leading cause of death among kidney transplant candidates with functioning grafts. ED contributes to the pathogenesis of many of the CV diseases. Various biomarkers and vasoreactivity tests are available to study endothelial functions. Differences in ethnicity, culture, religion, and education shape local practices and infrastructure, making the establishment of a single global entity unfeasible. Even with these social disparities aside, logistical factors such as time and distance between organ procurement and transplantation sites pose significant challenges. While technological advancements have extended organ preservation times, they have yet to support the demands of transcontinental transplantations effectively. This review presents a comparative analysis of the structures, operational frameworks, policies, and legislation governing various transplant organizations around the world. Key differences pertain to the administration of these organizations, trends in organ donation, and organ allocation policies, which reflect the financial, cultural, and religious diversity across different regions.
