However, some patients remain unresponsive, necessitating further investigation.</p><p><strong>Objective: </strong>To investigate the association between systemic inflammatory response index (SIRI) and tadalafil unresponsiveness in erectile dysfunction patients.</p><p><strong>Methods: </strong>A total of 106 male patients who applied to the Andrology outpatient clinic with ED complaints between January and June 2024 were included in the study. Patients were started on daily tadalafil 5 mg therapy, and response was assessed after one month using the International Index of Erectile Function Erectile Function domain (IIEF-EF). SIRI values, calculated using neutrophil, monocyte, and lymphocyte counts, were compared between tadalafil-responsive and unresponsive groups.</p><p><strong>Results: </strong>Tadalafil unresponsiveness was observed in 48.1% of patients. Non-responders had significantly higher mean age(57.44 ± 12.52 vs. 47.22 ± 11.49, <i>p</i> < 0.001), BMI(27.22 ± 3.17 vs. 25.85 ± 2.92, <i>p</i> = 0.023), and SIRI values(1.33 ± 0.82 vs. 1.02 ± 0.40, <i>p</i> = 0.016) compared to responders. Multivariate analysis identified age(OR = 1,641, <i>p</i> = 0.001) and SIRI(OR = 2.420, <i>p</i> = 0.014) as independent predictors of tadalafil failure. Associations between ED and beverage consumption were analyzed using univariate and multivariate logistic regression. We also stratified by age and adjusted for potential confounders using propensity score matching (PSM).</p><p><strong>Results: </strong>After fully adjusting for covariates, logistic regression showed that regular grape juice consumption (≥ 5 times/week) was related to a lower prevalence of ED (OR = 0.21, 95% CI: 0.08-0.54, <i>p</i> = 0.001). Subgroup analyses found that this correlation remained significant for those over 40 years of age (all <i>p</i> < 0.05). After PSM adjustment, the association between grape juice consumption and ED remained statistically significant (OR = 0.12, 95% CI: 0.03-0.44, <i>p</i> = 0.001). However, most research on male infertility has primarily focused on the testis, often ignoring the critical role of the prostate. As the largest male reproductive gland, the prostate plays a vital role in semen quality. Prostatic fluid, a major component of semen, significantly influences sperm function and overall male fertility. Prostatitis, the most common prostate disorder among men, particularly with advancing age, can severely impair reproductive health. This article aims to explore the fundamental physiological functions of the prostate, the etiology of prostatitis, and its impact on sperm health. It also discusses potential treatment strategies for prostatitis. The aim was also highlighting the impact of age on both DFI and CMA3 staining and on sperm quality, and their associations with male infertility.</p><p><strong>Methods: </strong>In this retrospective study, conventional semen tests, including sperm motility and morphological evaluations and DFI and disturbed chromatin condensation indexes (DFI, CMA3) were performed according to the World Health Organization (WHO) 2021 criteria. DFI and CMA3 were evaluated using sperm chromatin dispersion (SCD) and chromomycin A3 (CMA3) staining assays and then correlation and regression analysis were done.</p><p><strong>Results: </strong>By analyzing SCD and CMA3 results, notable differences were found in sperm parameters among different DFI and CMA3 groups (all <i>p</i> < 0.05). It was found that in the male fertility quality sperm concentration, progressive (PR)/non-progressive (NP) motility, immobility, and morphology were significantly associated with sperm DFI and CMA3, but not with age (<i>p</i> < 0.05).</p><p><strong>Conclusions: </strong>Sperm molecular index DFI and CMA3 negatively affect male fertility quality through semen parameters (sperm concentration, motility, and morphology). Given that elderly patients with ED often have a complex combination of systemic and sexual health risk factors, the safety and efficacy of PDE5i in such a context are hereby reviewed.</p><p><strong>Materials and methods: </strong>A thorough examination of existing literature has been conducted on PubMed.</p><p><strong>Results: </strong>PDE5i has good safety and efficacy, but the situation is more complex for patients with hypogonadism than those with normal testosterone levels, with reduced responsiveness to PDE5i. This study explored ED in Korean OSA patients and assessed the impact of continuous positive airway pressure (CPAP) therapy on ED.</p><p><strong>Methods: </strong>A total of 87 male patients with OSA from four different sleep centers underwent physical measurements and completed sleep and mental health (MH) questionnaires, including the Korean version of the International index of erectile function (IIEF), before and three months after initiating CPAP therapy.</p><p><strong>Results: </strong>After three months of CPAP therapy, the patients demonstrated a significant improvement in ED as measured on the IIEF. However, the study found no significant correlation between the duration of CPAP use and the improvement in IIEF score. It did identify the SF36 quality of life assessment as a significant factor influencing ED improvement after CPAP.</p><p><strong>Conclusions: </strong>ED is a prevalent issue that escalates with age and is associated with OSA. The Masturbation Premature Ejaculation Diagnostic Tool (MPEDT) was created to address this gap, but its effectiveness for men who only engage in self-masturbation has not been studied. This research aimed to determine the frequency of self-reported PE patients who do not have vaginal intercourse and to evaluate the diagnostic accuracy of MPEDT for this group.</p><p><strong>Method: </strong>The study involved 446 male patients aged 18 to 40, and 40 non-self-reported-PE and non-vaginal-intercourse healthy males. Participants completed the MPEDT questionnaire and reported their recent six-months sex frequency.</p><p><strong>Result: </strong>Among the patients seeking treatment for PE, 21.75% had not engaged in vaginal intercourse in the past six months. Of the PE patients who completed the MPEDT questionnaire (86 patients), 90.7% were diagnosed with masturbatory-PE (MPE). The sensitivity for self-reported MPE and specificity for self-reported non-MPE were 93.02% and 72.5%, respectively.</p><p><strong>Discussion: </strong>For patients who have not had vaginal intercourse in the past six months but engage in masturbation and seek treatment for PE, the PEDT may not effectively assess their ejaculatory function. The MPEDT, however, can effectively evaluate their ejaculatory function. We used Review Manager version 5.4 for data analysis and management.</p><p><strong>Result: </strong>After applying inclusion and exclusion criteria for screening, a total of 4 studies involving 413 patients were finally included in our meta-analysis. According to our analysis, the PRP group showed significant advantages over the placebo group in terms of MCID at the first month (<i>p</i> = 0.03) and sixth months (<i>p</i> = 0.008), while there was no significant difference between the two groups at the third month (<i>p</i> = 0.19). This study aimed to examine healthcare professionals' perceptions, attitudes, and behaviors regarding the AFHS and 4Ms before they were implemented.</p><p><strong>Methods: </strong>This study was a questionnaire-based survey of 252 healthcare professionals in geriatrics and long-term care departments, Rumailah Hospital, Acute Care Services in Hamad General Hospital, and home healthcare services to assess their perceptions, attitudes, and behaviors regarding 4Ms from November 1, 2022, to July 31, 2023.</p><p><strong>Results: </strong>Most respondents acknowledged the benefits of providing care through AFHS. However, only 62% of respondents reported using the 4Ms framework. The most commonly used types of age-friendly care provided by health care professionals were reviews of high-risk medication use (64.2%) and screening for mobility limitations (55.8%).</p><p><strong>Conclusion: </strong>The findings suggest that there is a need for more training and education regarding the 4Ms framework for health care providers.
