Typically defined by non-response to at least two sequential, adequate trials of antipsychotics, treatment-refractory schizophrenia represents a significant clinical challenge. Currently, clozapine (Clozaril) is the only evidence-based treatment strategy available for treatment-refractory schizophrenia. Nonetheless, clozapine has long remained underutilized due to administrative barriers such as the Food and Drug Administration (FDA) Risk Evaluation and Mitigation Strategy (REMS) program, a historical requirement for clozapine prescribing. The FDA has eliminated the clozapine REMS program effective February 2025. {"title":"Effects of therapeutic music listening on depression and sleep quality in people with chronic schizophrenia: Pilot randomized controlled trial","authors":"Dilek Avci , Berfin Coskun","doi":"10.1016/j.apnu.2025.151865","DOIUrl":"10.1016/j.apnu.2025.151865","url":null,"abstract":"<div><h3>Objective</h3><div>This pilot study aimed to determine the effects of experimenter-selected recorded music listening on depression and sleep quality in people with chronic schizophrenia.</div></div><div><h3>Methods</h3><div>The single-masked, randomized controlled trial was conducted with 56 outpatients in a community mental health center in western Turkey, between January 2023 and July 2023. The music listening intervention was carried out for eight weeks, two sessions per week and 60 min per session.</div></div><div><h3>Results</h3><div>The post-test and follow-up test depression and sleep quality scores of chronic schizophrenia patients in the music listening group were lower than the control group and the difference was statistically significant. In addition, there was a significant decrease in the depression levels of the patients in the music listening group from pre-test to post-test and from post-test to follow-up test. There was a significant improvement in sleep quality in the post-test compared to the pre-test, but there was no significant difference between the follow-up test and post-test scores.</div></div><div><h3>Conclusion</h3><div>Listening to pre-recorded Turkish rast makam music is a feasible, economical, safe, and non-invasive intervention, able to improve both depression and sleep quality in outpatients with chronic schizophrenia. In Spain, early care centers focus on providing care to this population. However, little is known about parents' experiences during this process, the daily challenges they face due to their children's social skills deficits, and how their context and values influence this process. The aim of this study was to explore and describe the experiences of parents of children diagnosed with ASD in early care services, focusing on their children's social skills and the family's cultural values. For this purpose, a semi-structured interview was conducted with 17 parents and a qualitative thematic analysis was made based on the principles of Gadamer's hermeneutic phenomenology. Three main themes could be identified: the emotional process of the parents upon receiving the diagnosis, the unanticipated obstacles and coping strategies they use, and the role of cultural family values that maintain the family dynamics. Despite its prevalence, only a fraction of affected individuals receive intervention, emphasizing the need for comprehensive healthcare strategies. The CRAFFT is a common tool used to evaluate substance use in youths, but limited studies have examined the innovative CRAFFT+N, which includes a nicotine screen. Thus, this study aimed to evaluate the feasibility and fidelity of pediatric SBIRT in an outpatient psychiatric setting through retrospective chart review, and secondary aims were to explore rates of pediatric substance use and associations with psychiatric variables and modality of visits, in addition to trends over time.</div></div><div><h3>Methods</h3><div>A retrospective chart review of electronic health record data was completed. Chi-squared analyses were used to compare which psychiatric diagnoses and psychotropic medications were associated with positive CRAFFT+N scores.</div></div><div><h3>Results</h3><div>A total of 455 patient encounters were reviewed, revealing concerning rates of substance and nicotine use, with approximately 20 % screening positive on the CRAFFT+N. Depressive disorders (p < 0.001), OCD (p = 0.012), and neurodevelopmental disorders (p = 0.015) were associated with positive CRAFFT scores. Positive nicotine scores were associated with bipolar disorders (p < 0.001), psychotic disorders (p = 0.001), and other psychiatric disorders (p = 0.039).
